Cochlear and vestibular lesions in capsular otosclerosis as seen in microdissection.

In 24 temporal bones from patients with otosclerosis prepared by the method of microdissection and surface preparations, otosclerotic foci could be clearly seen during removal of the otic capsule. The state of activity of each focus was estimated on the basis of its consistency and vascularity. Small anterior foci constituted the most common form of involvement of the otic capsule. All were judged to be inactive, and none of them appeared to have caused obvious sensorineural degeneration. No cases of "pure cochlear otosclerosis" were seen. Sensorineural degeneration was associated with large a

Magnetic resonance imaging of the inner ear after both intratympanic and intravenous gadolinium injections.

CONCLUSION: Using magnetic resonance imaging (MRI), endolymphatic hydrops could be visualized on both sides after intratympanic (IT) injection of gadolinium contrast agents (Gd) in one symptomatic ear and subsequent intravenous (IV) Gd injection. The MRI revealed a difference of intracochlear Gd distribution between the IT injection side and the contralateral IV side. OBJECTIVES: Although the IT method allows greater enhancement of the perilymph, many patients feel reluctance in receiving the IT injection in asymptomatic ears. We attempted to evaluate endolymphatic space size on both sides wi

Temporal bone histopathology of a patient with a nucleus 22-channel cochlear implant.

The temporal bone histopathology of a patient with profound sensorineural hearing loss attributable to cochlear otosclerosis is presented. The patient had a Nucleus 22-channel cochlear prosthesis implanted in the right ear prior to his demise. Unintentional scala media insertion of the electrode array resulted from labyrinthitis ossificans obliteration of the scala tympani. Inadvertent facial nerve stimulation necessitated program exclusion of electrodes 11 to 16 located in proximity to the labyrinthine and geniculate segments of the facial nerve. The patient received significant benefit from

Communicatory routes connecting the middle ear, the inner ear and the subarachnoid space via perineural space.

Twenty-one rabbits received injection of Indian ink into the stylomastoid foramen. Observation of the serial sections of their temporal bones revealed Indian ink infiltration into the subarachnoid space, along the facial nerve, the auricular branch of the vagus nerve and the tympanic nerve. Along the acoustic nerve, the infiltration was found as far as the fundus of the internal auditory meatus. In some cases, however, Indian ink reached the subarachnoid space and infiltrated into the scala tympani, scala vestibuli and around the saccule by way of the cochlear aqueduct. These routes are suspec

Scala vestibuli cochlear implantation in patients with partially ossified cochleas.

Partial cochlear obstruction is a relatively common finding in candidates for cochlear implants and frequently involves the inferior segment of the scala tympani in the basal turn of the cochlea. In such patients, the scala vestibuli is often patent and offers an alternative site for implantation. The current report describes two patients with such partial obstruction of the inferior segment of the basal cochlear turn, caused in one case by systemic vasculitis (Takayasu's disease) and in the other by obliterative otosclerosis. A scala vestibuli implantation allowed for complete insertion of th

Frequency dependence and harmonic distortion of stapes displacement and intracochlear pressure in response to very high level sounds.

Previous reports have suggested that intracochlear pressures (PIC) measured at the base of the cochlea increase directly proportionally with stapes displacement (DStap) in response to moderately high (<130 dB SPL) level sounds. Consistent with this assumption, we have reported that for low frequency sounds (<1 kHz), stapes displacement and intracochlear pressures increase linearly with sound pressure level (SPL) for moderately high levels (<130 dB SPL), but saturate at higher exposure levels (>130 dB SPL). However, the magnitudes of each response were found to be frequency dependent, thus the

Magnetic resonance imaging of the membranous labyrinth during in vivo gadolinium (Gd-DTPA-BMA) uptake in the normal and lesioned cochlea.

MRI with a T1 contrast agent was used to investigate the normal and noise-damaged cochlea. The time course and distribution of the in vivo uptake of the gadodiamide chelate bound paramagnetic Gd ion (GdDTPA-BMA) throughout the membranous labyrinth of normal and impulse noise-damaged guinea pig cochleae were measured by MRI at 4.7T. Simultaneous signal enhancement of the basal, medial and apical scala tympani (ST) and scala vestibuli (SV) was observed within 10 min following i.v. injection, reaching maximum levels at around 100 min. ANOVA and post hoc paired t-tests showed statistically signifi

Instrumental perforation of the round window. Animal experiments using cochleography and ERA.

The round window membrane of the inner ear of the guinea pig was perforated under Ketanest anaesthesia. A very rapid and almost total loss of cochlear microphonics, auditory nerve action potential and brain-stem response developed. Replacement of the perilymph with Ringer's solution and surgical closure of the membrane damage had no effect on auditory nerve action potential or brain-stem response. Further decline in the amplitude of cochlear microphonics was halted, however. When perfusion of the round window membrane was carried out whilst the bulla was full with Ringer's solution, no essenti

Micro-magnetic resonance imaging of the inner ear in the guinea pig.

We applied a magnetic resonance microscopy at 7.05 T with a gradient coil unit to image the fine structure of the guinea pig cochlea. First, a three-dimensional MR image of the surface of the cochlea was obtained to select the location of cross-sectional images. Then, cross-sectional images of the basal turn, the second turn and the apical turn of the cochlea were obtained. Based upon the different protein concentrations of the endolymph vs the perilymph, the scala vestibuli, scala tympani and the cochlear duct could be clearly distinguished. This allowed a determination of the location of bot

[Comparative studies of lactate concentration in the perilymph, blood and cerebrospinal fluid of normal and sound exposed guinea pigs (author's transl)].

The paper deals with comparative studies of lactate concentration in the perilymph (PL) of scala tympani and of scala vestibuli, arterial and venous blood, serum and cerebrospinal fluid (CSF) of normal and sound exposed guinea pigs, special consideration having been given to possible sources of error in the methods employed. Lactate was determined enzymatically using a micromodification of the Boehringer UV-test combination adapted to 1 mul PL. The lactate concentrations in the PL of scala tympani and scala vestibuli did not differ significantly. The mean values amounted to 4.5-5.2 mM/l in the

3 Tesla delayed contrast magnetic resonance imaging evaluation of Ménière's disease.

OBJECTIVE: To determine whether 3 Tesla (T) magnetic resonance imaging (MRI) with delayed contrast imaging has sufficient anatomic resolution to image the intracochlear fluid spaces (i.e., the scala tympani, scala media, and scala vestibuli) of the inner ear and identify endolymphatic hydrops in vivo. STUDY DESIGN: Four normal subjects underwent 3T MRI scanning after the administration of gadodiamide intravenous (IV) contrast. MRI region of interest signal intensity was used to determine the diffusion of gadodiamide into the perilymphatic fluid spaces over time. Seven subjects with unilateral

MR imaging of intralabyrinthine schwannoma, labyrinthitis, and other labyrinthine pathology.

This article reviews recent imaging developments in various diseases of the membranous labyrinth. Membranous labyrinth is the initial receptor for acoustic stimuli. Exciting progress has been made in the imaging evaluation of the normal and diseased labyrinth. Structures of the membranous labyrinth, such as the cochlea, vestibule, semicircular canals, and endolymphatic duct and sac can be visualized on routine MR scans. Using high-resolution MR imaging techniques, researches have been able to visualize the cochlear duct, scala tympani, scala vestibuli, and even the otolithic mass. The sensitiv

Experimental observation of particulate flow along cranial nerves.

The purpose of this study was to investigate the communicative routes via the cranial nerves among the middle ear, the auditory tube, the inner ear, and the subarachnoid space under nearly physiological conditions. Various volumes of Indian ink were slowly injected into the facial nerve trunk of rabbits at the stylomastoid foramen, then serial sections of the heads were observed under the light microscope. This study yielded the following results: (i) Indian ink penetrated into the subarachnoid space through the internal auditory canal and/or the jugular foramen along the nerve sheaths of the

Supporting-cell and extracellular responses to acoustic clicks in the free-standing region of the alligator lizard cochlea.

1. Acoustic clocks were delivered to the tympanic membrane of anesthetized alligator lizards, and electric responses were measured within the free-standing region of the cochlea using glass micropipettes. Responses were recorded intracellularly in supporting cells and extracellularly in the receptor organ and in the scalae. Gross responses were also recorded with wire electrodes in scala tympani. 2. Intra- and extracellular responses contain two components: (1) an early (with latent period less than 0.15 ms after the onset of the click), rate-independent component presumed to originate in the

Safety and audiological outcome in a case series of tertiary therapy of sudden hearing loss with a biodegradable drug delivery implant for controlled release of dexamethasone to the inner ear.

BACKGROUND: Intratympanic injections of glucocorticoids have become increasingly common in the treatment of idiopathic sudden sensorineural hearing loss (ISSHL). However, due to their fast elimination, sustained applications have been suggested for local drug delivery to the inner ear. MATERIALS AND METHODS: The study is based on a retrospective chart review of patients treated for ISSHL at a single tertiary (university) referral center. We included patients who were treated with a solid, biodegradable, poly(D,L-lactic-co-glycolic acid) (PLGA)-based drug delivery system providing sustained de

An optically-guided cochlear implant sheath for real-time monitoring of electrode insertion into the human cochlea.

In cochlear implant surgery, insertion of perimodiolar electrode arrays into the scala tympani can be complicated by trauma or even accidental translocation of the electrode array within the cochlea. In patients with partial hearing loss, cochlear trauma can not only negatively affect implant performance, but also reduce residual hearing function. These events have been related to suboptimal positioning of the cochlear implant electrode array with respect to critical cochlear walls of the scala tympani (modiolar wall, osseous spiral lamina and basilar membrane). Currently, the position of the

Threshold sensitivity of the auditory pathway: effects of worsening signal-to-noise ratio in the auditory nerve.

Perfusions of scala tympani with high potassium solutions increase spontaneous activity of auditory nerve fibres without affecting their threshold sensitivity. In these circumstances, however, the signal-to-noise ratio of the auditory nerve response to a given stimuli is worsened. Recordings from cochlear nucleus neurones during such perfusions indicate that this worsening is critical for threshold sensitivity at higher levels in the auditory pathway.

Speech recognition with cochlear implants as a function of the number of channels: Effects of electrode placement.

This study investigated the effects of cochlear implant (CI) electrode array type and scalar location on the number of channels available to CI recipients for maximum speech understanding and sound quality. Eighteen post-lingually deafened adult CI recipients participated, including 11 recipients with straight electrode arrays entirely in scala tympani and 7 recipients with translocated precurved electrode arrays. Computerized tomography was used to determine electrode placement and scalar location. In each condition, the number of channels varied from 4 to 22 with equal spatial distribution a

Histologic evaluation of temporal bones with cochlear implants.

Ectopic bone in the scala tympani is characteristic of cochlear otosclerosis and meningitis, which are the two most frequent etiologies of deafness in cochlear implant patients. The ectopic bone in a multiple-electrode implanted bone is probably caused by disease (syphilis) rather than by the electrode or electrical stimulation. The sensory elements being stimulated by intracochlear electrodes probably are the ganglion cells.

Cochlear implant soft surgery: fact or fantasy?

A basic surgical principle is to be as gentle as possible to accomplish the goals of the operation. The concept of "soft surgery" for cochlear implants consisted of a small, localized cochleostomy and gentle electrode insertion, the hope being that by limiting damage to the inner ear, superior hearing results might be obtained. The technique includes deferring the cochleostomy until immediately before electrode insertion, use of a large burr to flatten the promontory, followed by a smaller burr to expose the endosteum, preservation of the endosteum of the scala tympani, smoothing of the bony e
