Atraumatic Scala Tympani Cochleostomy; Resolution of the Dilemma.

OBJECTIVES: While an accurate placement in cochleostomy is critical to ensure appropriate insertion of the cochlear implant (CI) electrode into the scala tympani (ST), the choice of preferred cochleostomy sites widely varied among experienced surgeons. We present a novel technique for precise yet readily applicable localization of the optimum site for performing ST cochleostomy. MATERIAL AND METHODS: Twenty fresh frozen temporal bones were dissected using the mastoidectomy-posterior tympanotomy approach. Based on the facial nerve and the margins of the round window membrane (RWM), the cochleo

Active middle ear implants-Enhanced efficiency of oval window stimulation with larger piston size.

INTRODUCTION: Direct piston-stimulation of the oval window with a middle ear implant, following a stapedotomy or stapedectomy, is one treatment option for patients with otosclerosis. Here, we experimentally investigated whether increasing the surface area of the actuated piston enhances transmission efficiency in oval window (OW) stimulation. METHODS: Fresh-frozen human cadaveric temporal bones (N = 14) were used to evaluate the output of different oval window couplers (OWCs) of A = 0.38 mm², 1.26 mm2, 3.06 mm2 by intracochlear pressure difference measurements between scala vestibuli (PSV) an

Temporal bone findings in a case of otopalatodigital syndrome.

The principal features of the otopalatodigital syndrome are hearing loss, cleft palate, and skeletal dysplasia of the hands and feet. The right temporal bone was acquired from a boy with this syndrome who died at the age of 2 1/2 years. Behavioral audiometry had indicated a conductive hearing loss, with probable near-normal sensorineural function; brainstem evoked response audiometry indicated a mild sensorineural hearing loss. Histologic studies of the temporal bone revealed dysmorphic features in both the middle ear and the bony labyrinth. The ossicles were deformed, the stapes was fixed, an

Salicylate, mefenamate, meclofenamate, and quinine on cochlear potentials.

The perilymphatic spaces of guinea pig cochleae were perfused with artificial perilymph, with and without drug, at a rate of 2.5 microliters/minute for 10 minutes. The compound action potential of the auditory nerve, cochlear microphonics, and the summating potential evoked by 10 kHz tone bursts of varying intensities were recorded from a wire inserted in the basal turn scala vestibuli. The endocochlear potential was recorded from the scala media. Sodium salicylate (1.25 to 10 mmol/L) reduced the magnitude of the compound action potential evoked by low-sound intensities without affecting the c

Prognostic Factors in Cochlear Implant in Otosclerosis: Radiologic and Clinical Variables: Cochlear Implant Predictors in Otosclerosis.

OBJECTIVE: This study aimed to determine whether radiologic and clinical data can predict the cochlear implant (CI) benefit in patients with otosclerosis, with a focus on the prognostic value of computed tomography (CT) features and relevant clinical variables. METHODS: A retrospective single-centre analysis was conducted on 126 otosclerotic patients (143 implanted ears) who underwent CI between 2009 and May 2025. Preoperative and postoperative CT scans were reviewed and classified using established grading systems (Symons/Fanning, Rotteveel, Veillon). Clinical variables included age at impla

[Multi-frequency impedance measurement and Meniere's disease. Analysis of the results of TEFLAG].

We present today our preliminary results of the TEFLAG test performed on 58 patients suffering from Meniere's disease. This test completed in 1993 has already led to a 4-stage classification of noise induced hearing losses. On the one hand, it is based on the morphological study of susceptance B curves, and on the other hand, on the study of the resonant frequency (RF) of the annular ligament of the stapes, provided by a multifrequential impedancemeter. The endolymphatic hydrops coming with the Meniere's disease practically always cohabits in the intercritical period with the decrease of the p

Revision Cochlear Implantation via Middle Turn Insertion in Post-Traumatic Basal Turn Ossification : Revision cochlear implantation.

BACKGROUND: Complete ossification of the basal turn of the cochlea represents a major challenge inrevision cochlear implantation, particularly in post-traumatic cases. Otic capsule violating temporal bone fractures need to be carefully managed to avoid future possible meningeal infections. OBJECTIVE: To describe our surgical technique of middle turn cochleostomy combined with subtotal petrosectomy for revision cochlear implantation in the setting of total basal turn ossification due to an otic capsule-violating temporal bone fracture. METHODS: We report a case of failed primary cochlear impl

Characterizing Insertion Pressure Profiles During Cochlear Implantation: Simultaneous Fluoroscopy and Intracochlear Pressure Measurements.

BACKGROUND: Combined electrical-acoustical stimulation (EAS) has gained popularity as patients with residual hearing are increasingly undergoing cochlear implantation. Preservation of residual hearing correlates with hearing outcomes, but loss of hearing occurs in a subset of these patients. Several mechanisms have been proposed as causing this hearing loss; we have previously described high amplitude pressure transients, equivalent to high-level noise exposures, in the inner ear during electrode insertion. The source of these transients has not been identified. METHODS: Cadaveric human heads

Evaluation of round window stimulation using the floating mass transducer by intracochlear sound pressure measurements in human temporal bones.

HYPOTHESIS: Round window (RW) stimulation with a floating mass transducer (FMT) can be studied experimentally and optimized to enhance auditory transduction. BACKGROUND: The FMT (MED-EL Vibrant Soundbridge) has been recently implanted in patients with refractory conductive or mixed hearing loss to stimulate the RW with varying degrees of success. The mechanics of RW stimulation with the FMT have not been studied in a systematic manner. METHODS: In cadaveric human temporal bones, measurements of stapes velocity with laser vibrometry in response to FMT-RW stimulation were used to optimize FMT

Intact blood-perilymph barrier in the rat after impulse noise trauma.

CONCLUSION: The permeability of the blood-labyrinth barrier for radioactive mannitol was unchanged after impulse noise trauma. The present findings are contradictory to the theory of an increased permeability in the blood-labyrinth barrier as a result of extensive noise exposure. OBJECTIVE: Noise trauma is reported to cause multiple effects on the cochlea including mechanical and metabolic damage. The aim of the study was to observe the effects of impulse noise on cochlear homeostasis. MATERIALS AND METHODS: A well-established rat model was used for evaluation of the early effects of impulse

Surgical technique protocol for cochlear implantation in post-meningitic ossified cochlea.

INTRODUCTION: Labyrinthitis ossificans is a known complication of bacterial meningitis which results in severe to profound hearing loss. Historically, labyrinthitis ossificans was considered a contraindication for cochlear implantation. However, with advances in technology, cochlear implantation is an accepted treatment for hearing loss in labyrinthitis ossificans. MATERIALS AND METHODS: 17 cases with varying degrees of ossification in the cochlea were evaluated and treated at our tertiary referral hospital and cochlear implantation center out of a total of 832 profoundly deaf patients evalua

Performance considerations of prosthetic actuators for round-window stimulation.

Round-window (RW) stimulation has improved speech perception in patients with mixed hearing loss. In cadaveric temporal bones, we recently showed that RW stimulation with an active prosthesis produced differential pressure across the cochlear partition (a measure related to cochlear transduction) similar to normal forward sound stimulation above 1 kHz, when contact area between the prosthesis and RW is secured. However, there is large variability in the hearing improvement in patients implanted with existing modified prosthesis. This is likely because the middle-ear prosthesis used for RW stim

Surgical treatment for congenital absence of the oval window with facial nerve anomalies.

Three patients with a middle ear malformation characterized by the absence of the oval window and complicated by facial nerve anomalies were treated by surgery. Hearing ability of all the patients was improved. A variety of facial nerve anomalies were observed. Fenestration of the vestibule above the facial nerve was performed in the case of 2 of the 3 patients. In the other patient, fenestration of the scala vestibuli below the facial nerve was performed. In the case of all the 3 patients, hearing ability improved after the surgery and remained stable for a long time. Surgery for this disease

High-resolution 3-D T2-weighted imaging in the diagnosis of labyrinthitis ossificans: emphasis on subtle cochlear involvement.

BACKGROUND: Meningitis is the most common cause of acquired sensorineural hearing loss (SNHL) in children. Labyrinthitis ossificans develops in a significant number of patients with meningitis-related SNHL. Reduced T2 signal on MRI within the membranous labyrinth is often noted in the fibrous and ossifying stages of labyrinthitis ossificans. OBJECTIVE: The purpose of this study is to demonstrate the distribution and extent of involvement of the cochlea in children being evaluated for labyrinthitis ossificans; using high-resolution 3-D T2-weighted imaging, and to evaluate for subtle involvemen

Using the CLARION cochlear implant in cochlear ossification.

This paper is a retrospective review of 5 patients with various degrees of cochlear ossification who were implanted with the CLARION Multi-Strategy Cochlear Implant. Preoperative computed tomography scans, intraoperative findings, surgical technique, and hearing outcomes are discussed in a case report format. Full implantation was achieved in all cases by a systematic approach that included drill-through of proximal obstruction (2 cases), scala vestibuli insertion (2 cases), and complete drill-out (1 case). The only complication was delayed wound healing in a patient with sickle cell disease,

An overview of cochlear implant electrode array designs.

Cochlear implant electrode arrays are designed with specific characteristics that allow for the preservation of intra-cochlear structures during the insertion process, as well as during explantation. Straight lateral wall (LW) electrode arrays and pre-curved modiolar hugging (MH) electrode arrays are the two types that are commercially available. Although there is a third type of electrode array called the mid-scala (MS), which is positioned in the middle of the scala tympani (ST), and is usually considered as an MH type of electrode. Different lengths of straight LW electrode arrays are curre

Various Surgical Techniques for Cochlear Implantation in an Ossified Cochlea: A Systematic Review.

BACKGROUND: Cochlear ossification, otherwise known as labyrinthitis ossificans, is not an uncommon encounter in candidates for cochlear implantation (CI) that may necessitate alternative surgical approaches due to the insertion difficulties. We aimed to review the literature for the documented approaches for CI in cases with cochlear ossification. METHODS: This review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines on articles that described their surgical approach for CI in cases with cochlear ossification. Case report of mite infestation of the external ear in Carollia perspicillata: Histological features of the ear.

Bats host a diverse assemblage of ectoparasites that occupy specific anatomical microhabitats, yet the histological effects of these associations remain poorly documented. Here, we report a case of mite infestation of the external ear in the Neotropical bats species Carollia perspicillata and Carollia brevicauda, with a detailed histological characterisation of the auricle and inner ear of C. perspicillata. Bats were captured during biodiversity monitoring programmes in Antioquia, Colombia, and mites were collected and identified morphologically. Histological and histochemical analyses of ear
