Deep infections caused by Scedosporium prolificans. A report on 16 cases in Spain and a review of the literature. Scedosporium Prolificans Spanish Study Group.

Scedosporium prolificans, a mold morphologically similar to Scedosporium apiospermum, may cause asymptomatic colonization or localized or disseminated infection following trauma, surgery, and immunosuppression. S. prolificans is normally resistant to available antifungal agents, and prognosis depends largely on the host's immune status, extent of infection, and feasibility of surgical debridement. We report on 16 patients with deep S. prolificans infections, focusing on predisposing factors, clinical characteristics, outcome, postmortem findings, and antifungal susceptibility testing to 6 anti

Skull base osteomyelitis secondary to Scedosporium apiospermum infection.

Scedosporium apiospermum is a common environmental mold which is increasingly reported in the literature as a cause of infection, particularly in the immunocompromised patient population. We present a case of malignant otitis externa due to S apiospermum, complicated by spread of infection causing skull base osteomyelitis, internal carotid artery vasculitis and subsequent stroke. Despite the multiple complications encountered, prompt diagnosis and initiation of appropriate antifungal treatment resulted in patient survival. Multiple imaging modalities were used to aid the establishment of the d

Scedosporium cerebral abscesses after extra-corporeal membrane oxygenation.

BACKGROUND: Scedosporial infections are usually encountered in the immunocompromised patients. However, they are now emerging in the immunocompetent population and have an affinity for the central nervous system. They represent a therapeutic challenge, since they are highly resistant to most antifungal medications. METHODS: We report the case of an immunocompetent patient with multiple cerebral abscesses secondary to Scedosporium apiospermum following extracorporeal membrane oxygenation (ECMO) and review the literature about this challenging cerebral infection. RESULTS: A previously healthy

Implantable cardioverter-defibrillator infection due to Scedosporium apiospermum.

We report a case of implantable cardioverter-defibrillator infection due to Scedosporium apiospermum. This infection persisted despite systemic antifungal treatment with voriconazole and was controlled after removal of the foreign device and replacement of the involved tricuspid valve. This case underscores the importance of a combined surgical and medical approach for this complicated infection. Scedosporium species should be considered in the differential diagnosis of cardiac or intravascular device-related infections.

Scedosporium apiospermum traumatic endophthalmitis successfully treated with voriconazole.

We describe a case of traumatic endophthalmitis due to Scedosporium apiospermum that was successfully treated with a combination of intravitreal and systemic voriconazole. A high dose of 200 microg was used intravitreally without any apparent complications. A brief overview of voriconazole use for treatment of ocular infections is presented, and a comparison is made with antifungal agents traditionally used in ophathalmology.

Scedosporium apiospermum pneumonia after autologous bone marrow transplantation.

Although opportunistic infections after bone marrow transplantation (BMT) are very common, only five cases of Pseudallescheria boydii infection have been reported in the literature, two of which were autopsy findings. A case of Scedosporium apiospermum infection after BMT, treated initially with amphotericin B (total dose of 2.5 g) and then with itraconazole (for 25 days), is reported here. When the patient failed to improve, Scedosporium apiospermum pneumonia was diagnosed and therapy was changed. The patient was treated successfully with miconazole (600 mg/8h for 32 days) and ketoconazole (2

Scedosporium spp. rhinitis in 4 dogs from Northern California.

Four dogs were presented for sneezing, nasal discharge, and epistaxis. All were mesocephalic or dolicocephalic and age ranged from 1.5 to 7 years. Computed tomographic imaging findings included focal destructive rhinitis. Endoscopic findings included turbinate and septum loss, mucopurulent discharge and fungal plaques. All dogs were treated with endoscopic debridement and clotrimazole infusion. One dog had resolution of clinical signs, 2 dogs developed subsequent nasal aspergillosis, and 1 dog had persistent nasal signs associated with polyposis.

Chronic Canaliculitis Due to a Novel Fungal Organism: Scedosporium.

Canaliculitis is a rare lacrimal disorder often caused by Streptococcus species, Staphylococcus species, or Actinomyces . In cases associated with punctal plugs, Pseudomonas species are often implicated. Fungal etiology of canaliculitis has been exceedingly uncommon. The authors present the first report of canaliculitis secondary to Scedosporium , a group of filamentous fungi containing the 2 medically pathogenic species Scedosporium prolificans and Scedosporium apiospermum. Scedosporium spp. are found in soil and water and often infect immunocompromised hosts. The patient responded to punctop

Scedosporium apiospermum: a rare cause of malignant otitis externa.

A 79-year-old man, with a history of well-controlled diabetes mellitus, presented with left-sided otalgia. With an initial diagnosis of simple otitis externa, he was discharged on topical drops. He represented 2 months later with worsening otalgia and discharge. A diagnosis of malignant otitis externa was made based on clinical and radiological findings. Intravenous Tazocin and Gentamicin were given based on previous bacterial culture from ear swabs. The patient failed to improve and developed left-sided facial nerve palsy. His condition stabilised following a change in antimicrobial therapy a

[Invasive fungal disease due to Scedosporium, Fusarium and mucorales].

The number of emerging organisms causing invasive fungal infections has increased in the last decades. These etiological agents include Scedosporium, Fusarium and mucorales. All of them can cause disseminated, virulent, and difficult-to treat infections in immunosuppressed patients, the most affected, due to their resistance to most available antifungal agents. Current trends in transplantation including the use of new immunosuppressive treatments, the common prescription of antifungal agents for prophylaxis, and new ecological niches could explain the emergence of these fungal pathogens. Thes

Scedosporium prolificans osteomyelitis following penetrating injury: A case report.

Scedosporium prolificans are opportunistic moulds that can cause mycetoma following penetrating injuries. This fungus is more virulent than other species and treatment options are limited. Here we describe the first known case in the UK of S. prolificans osteomyelitis, in a 4 year old following penetrating injury. Successful outcome with limb salvage and foot function is achieved after repeated surgical debridement, and combination chemotherapy with voriconazole/terbinafine.

Dermatic Scedosporium apiospermum infection after autologous bone marrow transplantation.

Infection with Scedosporium apiospermum (S. apiospermum) is rare, although it is associated with a high fatality rate, especially in immunosuppressed patients. A 23-year-old man with acute myelogenous leukemia (AML) (M2) who was pretreated with chemotherapy for autologous bone marrow transplantation developed a skin ulcer on the left groin. After a culture study demonstrated the presence of S. apiospermum, voriconazole was administered and the lesion rapidly improved. Since a diagnosis of S. apiospermum continues to depend on the results of a fungal culture and most isolates of S. apiospermum

Disseminated Scedosporium prolificans infection in a German Shepherd dog.

Disseminated Scedosporium prolificans infection in a 1-year-old female spayed German Shepherd dog is described. Clinical signs were predominantly associated with fungal pyelonephritis and the organism was cultured from the urine. The dog was treated with itraconazole and later, terbinafine was added. Subsequent antifungal susceptibility testing of the isolate showed it to be resistant to all available antifungal drugs. The dog was euthanased because of acute abdominal haemorrhage and associated clinical deterioration. Postmortem examination revealed extensive pyogranulomas containing fungal or
