Evaluation of serum and cerebrospinal fluid copeptin as biomarkers in children with febrile seizures, febrile illness without seizures, and meningitis: an exploratory study.

BACKGROUND: This study aimed to compare serum and cerebrospinal fluid (CSF) copeptin levels among children with febrile seizures, febrile illness without seizures, and meningitis; to investigate the relationship between serum and CSF copeptin levels; and to assess the potential diagnostic utility of these biomarkers. METHODS: This study included children aged 1 month to 18 years who presented to the pediatric emergency department with fever or seizures and underwent lumbar puncture because of signs of meningeal irritation or the absence of an identifiable source of fever. The patients were cl

A controlled trial of diazepam administered during febrile illnesses to prevent recurrence of febrile seizures.

BACKGROUND: Phenobarbital, once widely prescribed to prevent febrile seizures, is now in disfavor because of its side effects and lack of efficacy. Diazepam, administered only during episodes of fever, may be a safe, effective agent to prevent the recurrence of febrile seizures. METHODS: We conducted a randomized, double-blind, placebo-controlled trial among 406 children (mean age, 24 months) who had at least one febrile seizure. Diazepam (0.33 mg per kilogram of body weight) or placebo was administered orally every eight hours during all febrile illnesses. RESULTS: During a mean follow-up o

Risk factors for acute encephalitis and early seizure recurrence in complex febrile seizures.

The purpose of this study is to elucidate risk factors for central nervous system infection and early seizure recurrence in children with febrile seizures (FSs) and thus facilitate outpatient management of complex FS. This single-center, retrospective cohort study investigated 688 children (6-60 months old) with FSs in Japan during 2011-2021. We investigated the incidence and clinical manifestations of children with acute encephalitis or bacterial meningitis. Logistic regression modeling was used to examine risk factors for seizure recurrence within 24 h. Among children with recurrent FSs, the

Clinical study of epilepsy with severe febrile seizures and seizures induced by hot water bath.

We followed five cases with severe febrile seizures (FS) with early onset and seizures induced by hot water bath, indicating severe myoclonic epilepsy in infancy (SME) or the peripheral form of SME. These cases, however, had far better clinical courses than that of SME or the peripheral form of SME. These cases were characterized by: (a) severe FS with early onset and seizures induced by hot water bath; (b) a lack of myoclonic seizures; (c) no psychomotor deterioration; (d) no underlying disorders or signs of brain insult on neurologic and laboratory examination; (e) relatively successful seiz

Intermittent oral diazepam prophylaxis in febrile convulsions: its effectiveness for febrile seizure recurrence.

In order to evaluate the effectiveness of diazepam for the reduction in the recurrence of febrile seizures we carried out a prospective study in two groups of children; Group A: 45 children (25 female, 20 male), receiving oral prophylaxis with diazepam, and Group B: 65 children (35 female, 30 male) who did not receive any oral prophylaxis. All subjects of both groups were followed for at least 4 years and finally re-evaluated at the mean age of 6.7+/-1.4 years. Among the patients of Group A, recurrent febrile seizures (FS) occurred in five of the 45 children (11.1%). Among the 65 children of G

Comparison of Laboratory Findings for Simple and Complex Febrile Seizures.

Febrile seizures (FS) are the most common convulsive disorder in childhood and are classified into simple and complex subtypes. Although blood tests are not routinely required, certain laboratory parameters may provide additional information for distinguishing these subtypes. We conducted a single-center retrospective study of 67 children, including 41 with simple FS and 26 with complex FS. Laboratory and clinical findings were compared between groups. Venous blood gas analysis revealed significant differences in pH (P = .039) and pCO2 (P = .042). The area under the receiver operating characte

[Febrile seizures in infants and children : know how to respond].

Febrile seizures are the most common neurological disorder in infants and children. They affect children aged 6 months to 5 years with a frequency of 2 to 4 %. The diagnosis is clinical, and additional tests are mainly used to rule out other differential diagnoses. A distinction is made between simple febrile seizures and complex febrile seizures. Most febrile seizures stop spontaneously within a few minutes. If not, emergency treatment with benzodiazepines or even antiepileptic drugs is necessary. The prognosis is usually favourable. Recurrences are estimated at around 30 %. The risk of neuro

Immunological mechanisms and prevention strategies for febrile seizures in children.

Febrile seizures (FS) affect 2-5% of children globally, causing significant caregiver anxiety and healthcare utilization. Emerging evidence implicates neuroinflammation and T-cell-mediated immunity in FS pathogenesis, suggesting potential targets for future investigation. This Review synthesizes current evidence on FS prevention, emphasizing a paradigm shift from universal pharmacological approaches toward risk-stratified, personalized strategies. The COVID-19 pandemic provided unique insights: non-pharmaceutical interventions reduced FS incidence by 54-70%, while the Omicron variant emerged a

Study on the correlation between Interleukin 4 and febrile seizures.

OBJECTIVE: Febrile seizures (FS) are common in pediatric epilepsy, but their precise etiology remains unclear. Although cytokines such as interleukin (IL)-4, IL-1, and IL-6 are known to influence FS fever responses, their specific role is not fully understood. This study aimed to clarify the correlation between IL-4 levels and febrile convulsions, exploring molecular mechanisms through bioinformatics and animal experiments to enhance the understanding of FS pathogenesis. MATERIAL AND METHODS: With the GSE28674 dataset, the K-means clustering algorithm was used to select key genes that regulat

Subacute sclerosing panencephalitis in a child with recurrent febrile seizures.

Subacute sclerosing panencephalitis (SSPE) is a devastating disease of the central nervous system (CNS) caused by persistent mutant measles virus infection. The diagnosis of SSPE is based on characteristic clinical and EEG findings and demonstration of elevated antibody titres against measles in cerebrospinal fluid. Subacute sclerosing panencephalitis can have atypical clinical features at the onset. Herein, we report an unusual case of subacute sclerosing panencephalitis in a child with recurrent febrile seizures. The disease progressed with an appearance of myoclonic jerks, periodic high amp

Technical report: treatment of the child with simple febrile seizures.

OVERVIEW: Simple febrile seizures that occur in children ages 6 months to 5 years are common events with few adverse outcomes. Those who advocate therapy for this disorder have been concerned that such seizures lead to additional febrile seizures, to epilepsy, and perhaps even to brain injury. Moreover, they note the potential for such seizures to cause parental anxiety. We examined the literature to determine whether there was demonstrable benefit to the treatment of simple febrile seizures and whether such benefits exceeded the potential side effects and risks of therapy. The therapeutic app

Human herpesvirus-6 infection in children with first febrile seizures.

We evaluated febrile convulsions prospectively in 42 children to investigate the association between acute human herpesvirus-6 (HHV-6) infection and first-time febrile convulsions, using both virologic and serologic methods. Eight children had primary HHV-6 infection documented by viral culture and an additional three by acute- and convalescent-phase serologic studies. These findings indicate that acute HHV-6 infection is a frequent cause of febrile convulsions in young children.

gamma-Aminobutyric acid in CSF of children with febrile seizures.

Previous studies have suggested that levels of cerebrospinal fluid (CSF) gamma-aminobutyric acid (GABA) may be decreased in children with febrile seizures. We used gas chromatography and mass spectrometry to measure CSF GABA levels in 14 children with febrile seizures. The results were compared with the GABA levels in six children with first-time afebrile seizures, three with recurrent febrile seizures, and 13 controls (febrile children undergoing lumbar puncture to rule out meningitis). Children with central nervous system infections or known neurologic disease were excluded. The CSF GABA lev

Long-term white matter tract reorganization following prolonged febrile seizures.

OBJECTIVE: Diffusion magnetic resonance imaging (MRI) studies have demonstrated acute white matter changes following prolonged febrile seizures (PFS), but their longer-term evolution is unknown. We investigated a population-based cohort to determine white matter diffusion properties 8 years after PFS. METHODS: We used diffusion tensor imaging (DTI) and applied Tract-Based Spatial Statistics for voxel-wise comparison of white matter microstructure between 26 children with PFS and 27 age-matched healthy controls. Age, gender, handedness, and hippocampal volumes were entered as covariates for vo
