Infectious diseases in clinical practice (Baltimore, Md.)2016eng
A 46-year-old man with HIV infection and active intravenous drug use presented with approximately two weeks of fevers and body aches. On physical examination he was somnolent, had a new systolic murmur, bilateral conjunctival hemorrhages, diffuse petechiae, and left-sided arm weakness. Echocardiography revealed a large mitral valve vegetation and brain imaging demonstrated numerous embolic infarctions. Blood cultures grew Serratia marcescens. Despite aggressive treatment with meropenem the patient died due to intracranial hemorrhage complicated by herniation. Serratia marcescens is an uncommon

Serratia marcescens is a common nosocomial infection but a rare cause of osteomyelitis and more so of vertebral osteomyelitis. Vertebral osteomyelitis caused by this organism has been reported in few studies. We report a case of S. marcescens vertebral discitis and osteomyelitis affecting multiple non-contiguous vertebras. Although Staphylococcus aureus is the most common cause of vertebral osteomyelitis, rare causes, such as S. marcescens, need to be considered, especially when risk factors such as intravenous heroin use, post-spinal surgery and immunosuppression are present. Therefore, blood

Serratia marcescens is an extremely rare cause of necrotizing fasciitis. We report the first case of necrotizing fasciitis of the chest wall due to infection with S. marcescens that initially manifested as bilateral breast necrosis. The patient had a fulminant course leading to death within 72 h of presentation. Literature pertinent to S. marcescens-mediated necrotizing fasciitis is also reviewed.

PURPOSE: To describe the first reported case of endophthalmitis after 20-gauge vitrectomy caused by Serratia marcescens.
METHODS: Retrospective case report.
RESULTS: An 86-year-old woman underwent a standard 20-gauge vitrectomy for repair of a chronic rhegmatogenous retinal detachment. All sclerotomies were sutured at the completion of the case. Within 2 days after surgery, she developed severe endophthalmitis. A bacterial culture of her aqueous sample was positive for S. marcescens. Despite the culture-demonstrated sensitivity of this organism to the antibiotics given intravitreally, system

Necrotizing fasciitis colli is a rare, but life-threatening disease. A 43-year-old female patient was admitted to our department because of increasing dysphagia and slight swelling of the left side of her neck due to severe necrotizing pharyngolaryngitis. Initial treatment with cephalosporin I was ineffective. The patient developed a rapidly spreading necrotizing fasciitis of the neck requiring open surgery for débridement and drainage as well as tracheostomy. Microbiological analysis revealed Serratia marcescens, thus allowing targeted and ultimately successful antibiotic therapy. The case sh

PURPOSE: To report a Serratia marcescens infection that clinically simulated a conjunctival neoplasm.
METHOD: Case report.
RESULTS: A healthy 80-year-old man without contact lenses presented with a pink-yellow conjunctival mass that resembled a solid neoplasm. Stains and cultures of material that exuded from the mass during surgery revealed S. marcescens. Histopathology disclosed an epithelial-lined cyst with macrophages containing S. marcescens.
CONCLUSION: Although S. marcescens usually affects the eye as a keratoconjunctivitis in patients with contact lenses, it can also present as a mas

Serratia marcescens (S. marcescens) keratitis is a rare, sight-threatening infection primarily seen in contact lens users. A 22-year-old female developed sudden ocular symptoms two days after contact lens use. Examination revealed significant conjunctival injection, anterior chamber inflammation, and a corneal abscess with an epithelial defect. Despite initial moxifloxacin treatment, her condition worsened, requiring a switch to fortified vancomycin, ceftriaxone, and voriconazole. Cultures confirmed S. marcescens, allowing treatment to be streamlined to fortified ceftriaxone. The infection gra

Serratia species are opportunistic human pathogens found in diverse environmental habitats. Here, we report the first isolation of Serratia nevei from food samples in Nigeria. During a two-month epidemiological surveillance at a local food market in Dutsin-Ma, Katsina State, Nigeria, a total of 180 food samples were collected, and isolation and species identification were performed using chromogenic agar and MicroScan autoSCAN-4, respectively. Antimicrobial susceptibility and minimum inhibitory concentrations (MICs) were determined using the MicroScan autoSCAN-4 system. Strain F129B, recovered

A 66-year-old female patient presented with chronic medial eyelid swelling and discharge of the right eye. She was initially treated with tobramycin eye drops and oral amoxicillin-clavulanic acid. However, the persistence of signs and symptoms accompanied by grainy punctal discharge on follow-up prompted a diagnosis of lacrimal canaliculitis. She underwent punctum-sparing right superior and inferior canaliculotomy under local anesthesia and completed topical antibiotic treatment with moxifloxacin. Serial follow-up examinations showed complete resolution of swelling, discharge, and epiphora. Mi

Serratia marcescens, an opportunist pathogen mainly isolated in healthcare-associated infections, is a rare cause of infective endocarditis (IE) that generates an increased mortality rate compared to the usual agents. We report a case of a 70-year-old male patient who underwent a mitral valve replacement and was readmitted two months later with a high-grade continuous fever and deterioration of the general status. The diagnosis of early IE due to S. marcescens was established upon further investigation. The patient was treated with ertapenem and underwent surgery. Nevertheless, the prognosis w

A 79-year-old patient was hospitalized due to recurrent Serratia marcescens bacteremia. An implantable cardioverter-defibrillator (ICD) electrode infection with septic pulmonary emboli and vertebral osteomyelitis were diagnosed. In addition to antibiotic therapy, the ICD system was completely extracted. In patients with cardiac implantable electronic devices (CIED) and bacteremia that cannot be adequately explained or recurs, regardless of the pathogen involved, a CIED-associated infection always needs to be ruled out.

A 93-year-old male presented with left eye pain, fever and loss of vision two days after complicated cataract surgery. A diagnosis of Serratia marcescens endophthalmitis and systemic bacteremia was made after the organism was identified on vitreous and peripheral blood cultures. This case demonstrates that an aggressive intraocular infection can lead to bacteremia.

BACKGROUND: To report a case of Serratia marcescens endophthalmitis after pterygium surgery using the bare sclera technique with mitomycin C (MMC).
CASE PRESENTATION: A 69-year-old male patient underwent pterygium excision surgery using the bare sclera technique and 0.02% MMC. The patient presented with decreased visual acuity and pain from the day after the operation. Trans pars plana vitrectomy was performed and intravitreal antibiotics were administered. Cultures from the aqueous humor and intraocular lens were all positive for S. marcescens, which was sensitive to an empiric antibiotic re

The Gram-negative bacterium and opportunistic pathogen Serratia marcescens causes ocular infections in healthy individuals. Secreted protease activity was characterized from 44 ocular clinical isolates, and a higher frequency of protease-positive strains was observed among keratitis isolates than among conjunctivitis isolates. A positive correlation between protease activity and cytotoxicity to human corneal epithelial cells in vitro was determined. Deletion of prtS in clinical keratitis isolate K904 reduced, but did not eliminate, cytotoxicity and secreted protease production. This indicated

This study reports a case of Serratia marcescens cellulitis following a snakebite in a 50-year-old woman. The bite was on the dorsum of the right hand with symptoms of envenomation. She developed swelling and cellulitis with tissue necrosis. Wound debridement was performed. Pus and tissue biopsy cultures yielded Serratia marcescens sensitive to fluoroquinolones, aminoglycosides, third-generation cephalosporins and carbapenems. The patient responded to anti-snake venom (ASV) therapy, ciprofloxacin, local wound management and recovered uneventfully.

OBJECTIVE: Define the applicability of a rapid molecular typing scheme to study the epidemiology of a Serratia marcescens outbreak.
DESIGN: With the assistance of a simple bacterial lysis technique, isolates of S marcescens from a putative outbreak were genotyped with the polymerase chain reaction technology for which primers were chosen on the basis of previously defined enterobacterial repetitive intergenic consensus sequences.
SETTING: Pediatric ICU.
PATIENTS: Intensively monitored patients who were found to yield S marcescens from any body site during the epidemic period.
RESULTS: Over

PURPOSE/METHODS: After bilateral simultaneous radial keratotomy, Serratia marcescens keratitis, which involved multiple incisions of both eyes, developed in a 46-year-old physician. The keratitis was treated with repeated wound debridement, fortified topical antibiotics, and topical povidone-iodine.
RESULTS/CONCLUSIONS: Six months after radial keratotomy, uncorrected visual acuity was R.E.: 20/25 and L.E.: 20/60, both eyes correctable to 20/20. Health-care workers who undergo refractive surgery may be at increased risk of acquired postoperative infections because of their work environment. Al
