Repetitive top-performance of overhead athletes induces a tremendous stress for the throwing shoulder. Throwers reach rotational speed of their arm of more than 7000°/s with joint compression and distraction forces of more than 1000 N. This performance is tributary to adaption of the shoulder muscles and the joint itself. These adaptions may, however lead to two specific problems of the throwing shoulder: 1. The posterosuperior internal impingement of the rotator cuff and labrum between glenoid and humeral head during late cooking phase; 2. A decreased internal rotation of the glenohumeral joi

Shoulder arthrodesis is an end-stage salvage option for the failing, painful joint that cannot undergo or has failed reconstruction. It is indicated for irreversible and nonreconstructible massive rotator cuff tears and deltoid muscle denervation as well as for detachment of the deltoid from its origin. Rarely, arthrodesis is done to stabilize the glenohumeral joint after many failed attempts at shoulder reconstruction. Arthrodesis for failed prosthetic arthroplasty or tumor resection presents additional challenges because of the associated bone loss on the humeral and/or glenoid side of the j

Ultrasound of the shoulder is a powerful and accurate method for the examination of the rotator cuff, biceps mechanism and the synovium. This article describes the anatomy, standard examination technique, indications, pitfalls and potential errors. It lists suggested imaging algorithms for a range of shoulder pathology.

The shoulder joint takes a special position among all the other joints of the human body because of its special requirements of stability and mobility. Knowledge of the biomechanics of the shoulder joint forms the basis for the development of modern concepts of reconstructive surgery and arthroplasty. Most of the biomechanical findings are the result of research performed on cadaver shoulders using increasingly sophisticated methods of measurement. These studies elucidate the interaction of the static and dynamic factors which contribute to the delicate balance of the glenohumeral joint. Recen

Because disability and pain may persist or recur after shoulder surgery, visualization of the postoperative shoulder before further treatment is of great interest. Postoperative status is reevaluated most frequently after rotator cuff repair, capsulorrhaphy in patients with chronic instability, and acromioplasty, where postoperative symptoms are clinically difficult to distinguish from rerupture or inadequate surgical results. Postoperative evaluation or follow-up after surgical treatment of tumors or surgical repair of shoulder injuries are other potential indications. Whereas surgical proced

Except for instability, a variety of causes has been described in the painful shoulder. A thorough clinical examination may help establish the cause of shoulder pain. Even if modern imaging modalities (ultrasonography, computed tomography, MR imaging) allowed a precise diagnostic and therapeutic approach, plain radiographs are still necessary. Performed by an experienced radiologist, ultrasonography may be useful for the assessment of rotator cuff tears which are a common problem. CT arthrography and MR imaging are only recommended in preoperative assessment. In calcific tendinitis plain radio

61 shoulders of rheumatoid diseases, 23 of ankylosing spondylo-arthritis, 22 of psoriatic rheumatism and 30 of hyperostoses (Forestier's disease) were analysed and compared. There is a very significant difference between rheumatoid disease, on the one hand, and the other diseases, on the other hand. In ankylosing spondylo-arthritis, psoriatic rheumatism and hyperostosis, enthesophytes are observed; it is not the case in rheumatoid disease (unless associated with hyperostosis). The most frequent entesophyte is the one at the acromial insertion of the acromio-coracoid ligament ("bearded acromion

Many sports activities that involve the upper extremity entail similar patterns of movement. Analysis of these activities, a better understanding of the throwing motion, and an awareness of shoulder diseases or injuries have led to a rational plan for investigation and management of shoulder problems. Evaluation often extends beyond the usual medical boundaries and must be based on information obtained from an analysis of sports mechanics, a review of training methods, and a physical examination directed at determination of flexibility, strength, endurance, and the presence of inflammation. A systematic study of shoulder joints in 30 DISH patients demonstrates the frequency of shoulder involvement in this disease and its characteristic radio-clinical features. Radiological abnormalities observed in DISH shoulders are contrasted with those encountered in rotator cuff tears. However, both disorders can occur together, producing a "complex shoulder" syndrome. The most typical aspect of this is the presence of an osteoarthritic pseudoarthrosis between the greater tuberosity and the acromion which has developed from the subacromial enthesopathy.

The operative technique and indications for shoulder arthroscopy have been reviewed, with a description of proper technique and instrumentation. Visualization within the joint is usually superior to visualization with arthrotomy. With the refinement of techniques and equipment, it is expected that the frequency of transarthroscopic procedures will increase in the near future.

BACKGROUND: The criticisms of the current patient-reported outcome measure (PROM) scores are that they are laborious, may be difficult for the patients to understand, and some may require a license for their use. Therefore, the purpose of the study was to validate a new PROM scoring system.
METHODS: A new shoulder scoring questionnaire was developed after interviewing 50 patients and refined after testing in 20 additional patients. The new score, "Mumbai shoulder score (MSS)," consisted of 10 questions with 4 response options. The score evaluates shoulder pain and difficulties in tasks such a

OBJECTIVES: To determine whether positioning of the arm in adduction and internal rotation would improve the confidence in the diagnosis of Bankart lesions in first time shoulder dislocators.
METHODS: Eleven patients were imaged on an open bore MRI within 1-6 days of traumatic shoulder dislocation with the arm adducted and internally rotated, and subsequently the patients were reimaged with the arm adduced and externally rotated. Two blinded musculoskeletal radiologists determined the confidence of diagnosing labral tears in each of the two positions.
RESULTS: An anterior-inferior labral tea

1. Arthrography of the shoulder demonstrates the gross pathological changes in adhesive capsulitis of the shoulder. In true adhesive capsulitis, or frozen shoulder, arthrography shows a very definite decrease in joint capacity with obliteration of the reflected axillary fold. In many instances the subscapularis bursa is decreased in size or not visualized at all but, as a rule, the bicipital sheath is well outlined. 2. This procedure can serve as a diagnostic aid to differentiate true adhesive capsulitis from stiff and painful shoulder due to other causes. 3. A mixture of twelve cubic centimet

Stemless humeral components for shoulder arthroplasty represent the fourth generation of modern prosthetic shoulder implants. Because of their metaphyseal fixation, the implantation technique is rather straightforward and preserves the humeral canal from violation. Substantial benefits have been highlighted with such a design, including less perioperative morbidity, independence from the proximal humeral anatomy, preservation of bone stock, ease of potential revision surgeries, and limited risk of complex periprosthetic fractures. Initially conceived to better re-create the center of rotation

Posttraumatic stiffness of the shoulder joint is a frequent and socioeconomically relevant result of injury; however, prior to making the diagnosis as the only cause of a persisting impairment of motion, it is necessary to rule out other sequelae of trauma. Even intensive conservative treatment is mostly accompanied by the stagnation of the impairments of movement. In many cases the treatment of choice is early arthroscopic arthrolysis with a standardized approach and intensive follow-up treatment. In the surgical treatment of proximal humeral fractures with humeral head preservation, arthrosc

Posterior shoulder instability is a relatively uncommon condition, occurring in ∼10% of those with shoulder instability. Because of the rarity of the condition and the lack of knowledge in treatment, it is often misdiagnosed or patients experience a delay in diagnosis. Posterior instability typically affects athletes participating in contact or overhead sports and is usually the result of repetitive microtrauma or blunt force with the shoulder in the provocative position of flexion, adduction, and internal rotation, leading to recurrent subluxation events. Acute traumatic posterior dislocation
