Anatomical basics, variations, and degenerative changes of the shoulder joint and shoulder girdle.

This paper summarizes the anatomical basics of the shoulder, their variations, and precise definitions, including differential diagnoses. It also describes the characteristic degenerative changes caused by aging. A typical variation (7-15%) is the os acromiale, which forms the triangular epiphysis of the scapular spine. This abnormality must be differentiated from a fracture of the acromion or a pseudarthrosis. Because ossification of the acromion is complete after age 25, the os acromiale should be diagnosed only after this age. The shape of the acromion is a further important feature. In a r

[Indications and limitations of conservative and invasive treatment of the inflammatory shoulder joint and the shoulder girdle].

An inflammatory rheumatic shoulder can be assessed as a forgotten joint. Apparent problems and deformities of the hands and feet are prioritized in the perception of rheumatic patients. In contrast, however, involvement of the shoulder joint in the context of an inflammatory rheumatic disease is very high with up to 85% [2]. Loss of shoulder function and pain can be well compensated for a long time. This means that further diagnostics are only carried out when the joint is already destroyed and joint-preserving treatment options are obsolete. The various tools of evidence-based conservative tr

Sensory innervation of the human shoulder joints in healthy and in chronic pain shoulder syndromes.

BACKGROUND: Afferent innervation of shoulder joints plays a fundamental role in nociception and mechanoception and its alteration result in shoulder´s disease that course with pain and functional disability. METHODS: Joints shoulder from healthy subjects (n = 20) and with chronic pain shoulder syndromes (n = 17) were analyzed using immunohistochemistry for S100 protein to identify nerve structures (nerve fibers and sensory corpuscles), coupled with a quantification of the sensory formations. Sensory nerve formations were quantified in 13 distinct areas in healthy joint shoulder and in the ava

Shoulder Joint Range of Motion Related to Dementia.

INTRODUCTION: Dementia is caused by various diseases, including Alzheimer's disease dementia (ADD) and dementia with Lewy bodies (DLB). We often encounter patients with dementia who have limited shoulder joint range of motion (ROM), especially those with behavioral and psychological symptoms of dementia (BPSD). But the relationship between the diseases of dementia and restricted shoulder joint ROM is currently unclear. METHODS: We examined cognitive function and shoulder joint ROM in 234 new outpatients at 7 memory clinics in Japan. We assessed cognitive function using the Mini-Mental State E

[Arthroscopic resection of the shoulder joint (Mumford operation)].

Based on results of anatomical studies, we established an arthroscopic procedure to resect all degenerative changes within the joint without endangering the stabilizing structures. Since November 1989 we operated on 26 patients with this technique. There were no major complications. The mean score significantly increased from 64.9 (+/- 12.8) to 86.8 (+/- 11.5). Analysis of postoperative x rays revealed an average length of resection of 21.3 mm at the inferior border of the clavicle, of 15.4 mm in the middle, and of 10.2 mm at the superior border of the clavicle. 21 out of 26 patients were comp

[Osteochondromatosis in shoulder joint arthrosis--a case report].

We report on a 59 year old patient with repetitive shoulder trauma due to numerous grand mal convulsions and consequent severe osteoarthritis of the shoulder joint with formation of multiple loose bodies. We discuss the aetiology of the loose bodies as being different to those found in synovial chondromatosis, as well as the radiological, histological, clinical and therapeutical features of this entity.

Arthrography of the shoulder joint: study of the findings in adhesive capsulitis of the shoulder. Study of the findings in adhesive capsulitis of the shoulder.

1. Arthrography of the shoulder demonstrates the gross pathological changes in adhesive capsulitis of the shoulder. In true adhesive capsulitis, or frozen shoulder, arthrography shows a very definite decrease in joint capacity with obliteration of the reflected axillary fold. In many instances the subscapularis bursa is decreased in size or not visualized at all but, as a rule, the bicipital sheath is well outlined. 2. This procedure can serve as a diagnostic aid to differentiate true adhesive capsulitis from stiff and painful shoulder due to other causes. 3. A mixture of twelve cubic centimet

Electromyographic study relating to shoulder motion: control of shoulder joint by functional electrical stimulation.

The purpose of this study is to create the standard stimulation patterns of shoulder motion from electromyographic (EMG) data in 13 healthy human volunteers in order to control the movement of the paralyzed shoulder in quadriplegic and hemiplegic patients by functional electrical stimulation (FES). Simultaneous EMG measurement was made at 24 points of 17 major muscles relating to shoulder motion. Since the number of the output channels in the portable FES apparatus is limited, 12 major muscles were selected from statistically processing these EMG data and stimulation patterns were created base

[Imaging methods in traumatology of the shoulder- and elbow joint].

Radiological diagnostics (diagnostic imaging) of the traumatised shoulder and elbow joint has expanded considerably and has become increasingly meaningful in recent years. The introduction of computed tomography and magnetic resonance imaging has enabled a significantly better assessment of soft parts and of the inner spatial structure of the joints. Hence, focus of diagnosis has now shifted from the assessment of the bones (which was previously the main aspect of diagnosis) to a different analysis of the joint as a functional unit of bone and soft part structures, an analysis that can also de

[Anatomical bases of ultrasonic examination of the shoulder joint].

Authors present the parts of the shoulder that can be represented by ultrasound. Beside the rotator cuff of accentuated significance (within this, first of all the tendon of the supraspinatus muscle) the subacromial bursa, the tendon of the long head of the biceps, the muscle-tendon junction of the infraspinatus muscle are analysed. It is stated that the ultrasonography is suitable for a reliable representation of the soft tissues, surrounding the shoulder joint, further in possession of literary data they see possibility for a very exact judgement of the pathological alterations, too.

[The value of arthroscopy for diagnosis of instability of the shoulder joint].

As a glenohumeral instability is defined each case of transient recurrent and permanent decentration of the joint. We propose to differentiate primary and secondary glenohumeral instabilities. Arthroscopical diagnostic is helpful to classify the luxation and subluxation and for verify transient instabilities. In cases clinically snapping phenomena were noticed, arthroscopic findings were documented. A relation between those phenomena and arthroscopic findings is discussed.

[Possibilities of sonography in the diagnosis of instabilities of the shoulder joint].

We present ultrasound investigation as a diagnostic tool for shoulder instability. The following structures are visible in the posterior transversal plane: subcutis, deltoid muscle, infraspinatus muscle, infraspinatus tendon, humeral head, scapula, dorsal glenoid rim, dorsal labrum, and the joint capsule. The relation of the humeral head to the glenoid can be judged statically and dynamically. Anterior and posterior instabilities and secondary signs of dislocation such as Hill-Sachs lesion and effusion can be seen. This technique is easy to learn. By comparing the injured with the normal shoul

Injection-induced pyogenic arthritis of the shoulder joint.

This study reports the clinical results of treatment for injection-induced pyogenic arthritis of the shoulder joint. We followed up 13 patients who underwent operation for the treatment of pyogenic glenohumeral arthritis that developed after injections around the shoulder joint. Cultures identified Staphylococcus aureus in 6 patients and methicillin-resistant S aureus (MRSA) in 4. Finally, the University of California, Los Angeles Shoulder Score improved from 12.7 to 28.3 points. Four of the 9 patients with open surgery and 1 of 4 with arthroscopic surgery had good results; however, the other
