BACKGROUND: A painful and swollen shoulder has an extensive differential diagnosis, with Milwaukee shoulder syndrome (MSS) being diagnosed infrequently. Analysis of crystals in the synovial fluid is an important diagnostic step in patients with a swollen shoulder.
CASE DESCRIPTION: A 69-year-old female presented at the orthopaedic outpatient clinic with pain, limited mobility and swelling of the left shoulder. The diagnosis MSS was made based on the characteristic features on X-ray and MRI combined with the analysis of the synovial fluid, including a positive alizarin-red staining.

BACKGROUND: Hemiplegic shoulder pain (HSP) following stroke significantly affects the individual's function and quality of life. The mechanism of HSP is not clearly understood; hence, it is unclear why HSP resolves spontaneously or following routine care in some patients, while in others it becomes persistent. A painful shoulder is a frequent complaint in general practice. In this clinical vignette, we discuss the case of an elderly patient with pain and lack of strength in the right shoulder one month after a hospital stay. The diagnosis is an injury of the accessory nerve after the placement of a central venous catheter in an emergency setting. The different causes of non-orthopaedic causes of shoulder pain are reviewed with emphasis on neurological etiologies.

We present a case of a 19-year-old man with right shoulder pain lasting for several months. Abdominal imaging revealed a right adrenal mass directly invading vascular structures into the right atrium. Widespread metastatic adrenocortical carcinoma was confirmed on biopsy. He opted for palliative mitotane treatment with home hospice care. This case emphasises the importance of considering abdominal masses in the differential diagnosis of persistent right shoulder pain after common causes have been ruled out. Early diagnosis could be potentially life-saving.

In this case report we present a 55-year-old female patient with acute-onset shoulder pain. X-rays of the affected shoulder showed clear levels of resorbed calcium in an enlarged subacromial bursa which is typical for the resorptive phase of calcific tendinitis of the rotator cuff.

Shoulder pain and weakness are common presenting Emergency Department complaints. Patients are often diagnosed with fractures, dislocations, bursitis, and muscle strain. AIM: To evaluate the magnetic resonance imaging (MRI) findings in painful hemiplegic shoulder (PHS) in hemiplegic post-stroke patients.
MATERIALS AND METHODS: Patients with hemiplegia following their first cerebrovascular accident who were admitted to the Sarah Network of Hospitals for Rehabilitation were studied. Forty-five patients with pain in the hemiplegic shoulder and 23 post-stroke patients without shoulder pain were investigated. MRI and radiographic findings of the hemiplegic and contralateral asymptomatic shoulders were evaluated.

The authors have observed 23 cases of painful and unstable shoulders in athlets which could be related to unknown recurrent dislocations. The diagnosis was based on a radiological study demonstrating a deformity of the humeral upper extremity and of the glenoid. The patients were incapacited and had often been treated for "tendinitis". Most of the cases have been operated by a grafting of the anterior edge of the glenoid with success. It is thought that most of the glenoidal lesions are consequences and not causes of the dislocations.
