Risks and benefits of the single versus the triple MMR vaccine: how can health professionals reassure parents?

Measles, mumps and rubella (MMR) are all preventable but infectious diseases caused by viruses. A particular study by Wakefield et al suggests that there are potentially adverse effects of having the triple MMR vaccine. This has been reported widely by the media and has caused alarm to parents of young children, probably contributing to the decline in its uptake. In order to provide the context for the debate regarding the single versus the triple vaccine, this paper briefly appraises firstly, the Wakefield et al research paper that has led to public health concerns and secondly, a more rigoro

The More Than Words® parent-delivered program for autistic children and those with social communication challenges: a single-arm pragmatic feasibility trial.

BACKGROUND: Pediatric speech-language pathologists (SLPs) frequently use parent coaching interventions such as the More Than Words® (MTW) program to support young autistic children and others with social communication challenges. While some promising evidence exists for MTW, outcomes from previous studies have been mixed and research to date has primarily been conducted in controlled settings, limiting generalizability to real-world practice. To support future pragmatic randomized controlled trials (RCTs), this study aimed to assess the feasibility of a MTW evaluation protocol that had been co

Diversity of single small subcortical infarctions according to infarct location and parent artery disease: analysis of indicators for small vessel disease and atherosclerosis.

BACKGROUND AND PURPOSE: Single small subcortical infarctions (SSSIs), so-called lacunae, are typically caused by lipohyalinosis of a perforator artery. However, SSSIs can be caused by underlying large parent arterial disease or microatheroma of the proximal portion of a perforator artery. We sought to investigate whether indicators for small vessel disease (SVD) and atherosclerosis in patients with SSSI differ according to lesion location and the presence of parent artery disease. METHODS: We assessed 449 patients who had an SSSI (lesion diameter ≤ 20 mm) in the perforator territory of middle

Pharmacokinetics of chlorimipramine, chlorpromazine and their N-dealkylated metabolites in plasma of healthy volunteers after a single oral dose of the parent compounds.

A single oral dose of 0.7 mg kg-1 chlorimipramine (n = 18) and chlorpromazine (n = 16) was given to each subject 45 days apart and plasma concentrations of parent drugs and their monodesmethyl and didesmethyl metabolites were measured by GC. Ingestion of chlorimipramine resulted in an area under the plasma concentration-time curve (AUC0-24) for parent drug plus metabolites 5-fold higher than that observed in the same subjects following chlorpromazine intake (600 +/- 87 and 124 +/- 14 ng mL-1, respectively). Plasma chlorimipramine levels reached a mean peak value of 43.8 ng mL-1, which occurred

Loneliness, problematic digital media use, and nutritional-metabolic vulnerability in adolescents from single-parent households: an integrative review of eating- and sleep-related pathways.

Adolescents from single-parent households may experience household time, economic, supervisory, and routine-related constraints that are relevant to psychosocial well-being, digital media use, eating behavior, sleep, and metabolic health. The objective of this integrative review was to synthesize evidence on the associations of loneliness and problematic digital media use with eating- and sleep-related behaviors in adolescents and to examine how single-parent household characteristics may influence these relationships. The review applies a contextual-behavioral vulnerability model in which hou

Efficacy of a parent-based treatment for children and adolescents with obsessive-compulsive disorder: Protocol of a multiple baseline, single-case experimental design study.

BACKGROUND: Pediatric obsessive-compulsive disorder (OCD) is a severely impairing disorder, associated with high levels of family accommodation (FA). Approximately 40 % of youth do not benefit from first-line treatment options (cognitive behavioral therapy or pharmacotherapy). Supportive Parenting for Anxious Childhood Emotions (SPACE) is a parent-based treatment, teaching parents to reduce FA and increase supportive parenting, thereby aiming to improve the child's OCD. This article presents the protocol of a multiple baseline single-case experimental design (SCED) study to test the efficacy o

Effects of a Digital Parent-Child Single-Session Growth Mindset Intervention on Adolescent Depression and Anxiety Symptoms: A Three-Arm Waitlist Randomized Controlled Trial.

Adolescent depression and anxiety are prevalent, yet brief and scalable parent-child digital interventions remain understudied. This study evaluated the effects of a digital parent-child single-session growth mindset intervention targeting beliefs about intelligence, failure, and emotion on adolescent internalizing of symptoms. In a three-arm waitlist cluster randomized controlled trial, 390 parent-child dyads from seven secondary schools in Hong Kong were assigned to a parent-child intervention group, a child-only intervention group, or a waitlist control group. Students were assessed at base

Implementation of virtual parental presence during induction of anesthesia: a single-site descriptive study at a pediatric academic center.

BACKGROUND: Preoperative anxiety in children can cause distress for patients, families, and care teams. Virtual Parental Presence during the Induction of Anesthesia (VPPIA) offers a novel way to maintain emotional support during inhalational induction, particularly in the wake of COVID-19-related restrictions on in-room parental presence. While prior studies suggest VPPIA is feasible and acceptable, practical insights on real-world implementation and coaching strategies remain limited. METHODS: This single-site, descriptive implementation study at a pediatric academic center evaluated the feas

Internet-Delivered Psychological Treatment for Parents With Health Anxiety by Proxy: Replicated Randomized Single-Case Experimental Design.

BACKGROUND: Health anxiety by proxy is characterized by ruminations about severe illness in one's child that can cause severe distress in affected parents. Health anxiety by proxy may lead to repeated unnecessary medical consultations and checking the child's body for symptoms, as well as heightened attention to their child's behavior, sign of illness, and bodily symptoms. It has been hypothesized that health anxiety by proxy may pose a risk for transmission of maladaptive symptom coping and health anxiety from the parent to their child. In spite of this, no targeted treatment has previously b

The impact of audiovisual information on parental anxiety levels prior to hypospadias surgery: A prospective single center cohort study.

BACKGROUND: Most parents have anxiety before a pediatric surgical procedure. Parental anxiety may impair the parents' ability to cope with new or stressful situations while their children are undergoing surgery. By effectively improving the education of parents regarding the diagnosis, treatment, and potential complications of hypospadias and surgical repair, it might be feasible to reduce their anxiety during this process. OBJECTIVE: To determine whether structured audiovisual information would reduce parents' anxiety levels compared to classic verbal information. MATERIAL AND METHODS: The

Assessment of parental mosaicism in SCN1A-related epilepsy by single-molecule molecular inversion probes and next-generation sequencing.

BACKGROUND: Dravet syndrome is a severe genetic encephalopathy, caused by pathogenic variants in SCN1A. Low-grade parental mosaicism occurs in a substantial proportion of families (7%-13%) and has important implications for recurrence risks. However, parental mosaicism can remain undetected by methods regularly used in diagnostics. In this study, we use single-molecule molecular inversion probes (smMIP), a technique with high sensitivity for detecting low-grade mosaic variants and high cost-effectiveness, to investigate the incidence of parental mosaicism of SCN1A variants in a cohort of 90 fa

Participation in a single-blinded pediatric therapeutic strategy study for juvenile idiopathic arthritis: are parents and patient-participants in equipoise?

BACKGROUND: Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials (RCT) need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. METHODS: We compared preferences of parents

Investigation of Parental Transmission of RUNX2 Single Nucleotide Polymorphism and Its Association With Nonsyndromic Cleft Lip With or Without Palate.

OBJECTIVE: To investigate the association and parental transmission of RUNX2 single nucleotide polymorphisms (SNPs) with risk of nonsyndromic cleft lip with or without cleft palate (NS-CL±P). DESIGN: Four RUNX2 SNPs in 142 Korean NS-CL±P families (nine cleft lip, 26 cleft lip and alveolus, and 107 cleft lip and palate; 76 trios and 66 dyads) were genotyped. The minor allele frequency, heterozygosity, and chi-square test for Hardy-Weinberg equilibrium at each SNP were computed between parents. Pairwise linkage disequilibrium was computed as D' and r(2) for all SNPs. Both allelic and genotypic

Characterization of human serum-resistant and serum-sensitive clones from a single Trypanosoma brucei gambiense parental clone.

A protocol was developed to select clones of Trypanosoma brucei gambiense having different levels of resistance to normal human serum. Human serum-resistant clones were selected from a single parental clone by continuous serum treatment of infected immunosuppressed mice. Human serum-sensitive revertant clones were also obtained by continuous passage of resistant clones in immunosuppressed mice but without human serum pressure. It has been demonstrated that our trypanosome clones express distinct but stable levels of resistance. The variant antigenic type of each clone was characterized serolog

Psycho-emotional intervention with parents of very preterm babies during the first year: A single-arm pilot study.

Studies of intervention programs that aim to improve the emotional state of parents of children admitted to the neonatal intensive care units (NICU) are scarce in Spain. The aims of this single-arm pilot study are to get to know the emotional profile of parents of high-risk preterm newborns, and to explore parents' patterns of emotional well-being before and after a psychological program called the Parental Empowerment Program, to increase parental readiness levels. The sample was made up of 100 parents (50 couples) who participated in the program. Measurements were taken of post-traumatic str

Evaluation of a Parenting Program for Mothers With a Borderline Personality Disorder: A Multiple Baseline Single-Case Experimental Design Study.

Parents with borderline personality disorder (BPD) experience deficiencies in parenting skills leading to increased family burden, stress, mental health problems in children and even family violence. The present study evaluated a Dutch version of the parenting training based on Dialectical Behavior Therapy (DBT) for mothers with BPD. A multiple baseline single-case experimental design study was conducted with standardized and idiosyncratic assessments of parenting behavior and parental stress before, biweekly during, and after training. Participants (N = 10) were mothers with a BPD diagnosis w

Single subcortical infarction associated with parental arterial disease: important yet neglected sub-type of atherothrombotic stroke.

BACKGROUND: Traditionally, single subcortical infarction has been considered to be caused by lipohyalinosis of small perforating arteries. However, atherosclerosis in the parental artery produces similar single subcortical infarction by blocking the orifice of branching arteries. The purpose of this paper is to review the evolving concepts, clinical characteristics, and classification issues of single subcortical infarction associated with parental artery disease. SUMMARY OF REVIEW: Single subcortical infarction associated with parental artery disease is an important cause of subcortical infa
