Reversible atrioventricular blocks in thyroid storm.

Atrioventricular blocks or sinoatrial blocks are rarely described in patients with thyrotoxicosis or thyroid storm. The mechanism of these blocks remains obscure. Thyroid storm, being an emergency situation requires early diagnosis and management because if left untreated, it may prove fatal. Usually patients with AV blocks require pacing (temporary or permanent). Here we describe a case who developed AV blocks, did not undergo pacing, but recovered only on antithyroid treatment.

"Paradoxical" prolongation of sinus nodal recovery time after atropine in the sick sinus syndrome.

A case of symptomatic sick sinus syndrome is presented with confirmation of sinus nodal dysfunction established by functional testing. The validity of such provocative testing and the criteria for abnormality are discussed. A newly recognized, seemingly "paradoxical" and potentially detrimental effect of atropine noted in this patient is examined. Despite an increase in sinus rate and an improvement in sinoatrial conduction time after administration of atropine, a markedly prolonged sinus recovery time after rapid atrial pacing occurred, and atrial quiescence for more than 10 seconds was seen.

Clinical significance of prolonged sinoatrial conduction time.

Prolonged (greater than 152 msec) calculated sinoatrial conduction times (SACT) were found in 24 of 470 patients studied by the atrial extrastimulus technique, ranging from 155 to 220 msec (180+/-4.4; mean+/-SEM). There were 18 males and six females with ages of 29 to 85 (mean 65+/-2.6). Electrocardiographic monitoring revealed significant sinus or atrial dysrhythmias in 19 (79%) patients. Of these 19, 15 had persistent sinus bradycardia and/or sinoatrial block, three had sinus bradyarrhythmia with paroxysmal atrial tachycardia, and one had isolated atrial tachycardia. Additional electrophysio

A case report of sinoatrial arrest caused by temporal lobe epilepsy in subclinical glioblastoma.

BACKGROUND: Atrial fibrillation with symptomatic bradycardia, higher grade atrioventricular block, and sinus node disease are all common indications for permanent pacemaker implantation. The most frequent causes of sinus node disease treated with pacemaker implantation involve degenerative structural changes of the sinus node; less often, extrinsic causes (such as damage due to myocardial infarction or heightened parasympathetic nervous system activity) lead to pacemaker implantation. CASE PRESENTATION: A 50-year-old patient with syncope and documented sinoatrial arrest was referred. Neurolog

[Syncopal episodes in healthy young athletes. Report of 3 cases in the course of ergometric evaluation].

We report the occurrence of syncope in three healthy athletes during exercise stress testing. In two subjects the syncope occurred in the late recovery stage and was due to inhibition of sinus activity. In the third case a Mobitz type II A-V block developed during hyperventilation, which was followed by a complete A-V block with ventricular asystole and subsequent sinoatrial arrest. We discuss the probable pathophysiological mechanisms of these arrhythmias and their implications in sports practice.

[Physiopathologic mechanism of ventricular fibrillation induced by atropine. Report of two cases (author's transl)].

The authors report on two cases of ventricular fibrillation induced by bolus of atropine injected for diagnostic purpose. The electrophysiologic effect of adrenergic stimulation following the parasympathetic blockade, is probably the responsible mechanism rather than myocardial ischemia in the first patient, affected by complete a-v block with narrow QRS. The second patient, affected by coronary heart disease and suspected sick sinus syndrome, had more probably a ventricular fibrillation because of a worsening of the underlying myocardial ischemia due to the increase of the cardiac rate. the r

Automaticity of subsidiary pacemakers of patients with dysfunction of the sinus node.

Automaticity (postpacing impulse recovery time) of the sinus node or subsidiary pacemakers (or both) was determined by intracardiac recordings and atrial overdrive pacing in three patients with disease of the sinus node. Their electrocardiographic manifestations included sinus bradycardia, sinoatrial block, sinus arrest, atrioventricular junctional rhythm, and atrioventricular dissociation. One patient, who initially had syncope, demonstrated prolonged time for recovery of the sinus node (maximum corrected value, 3,485 msec) and impaired atrioventricular junctional automaticity (maximum correc

A clinicopathological study on the sick sinus syndrome.

A total of 6 cases of sick sinus syndrome were presented, including 2 cases of sinoatrial (SA) block and 4 cases of bradycardia-tachycardia syndrome. There were 2 men and 4 women, and their ages ranged from 69 to 91 years. Electrophysiological studies were performed in 3 cases, showing positive overdrive suppression in 1 and normal atrioventricular (AV) conduction in 3. Syncopal attacks were found in 4 cases, and 2 cases died suddenly. Histological examination of the SA node and AV conduction system disclosed (1) marked reduction of the SA nodal muscle fibers by 70 t0 80%, (2) moderate to mark

[Adult familial idiopathic binodal block].

Four brothers with a maximum age difference of 20 years, the eldest of whom has been re-examined after a 10 year interval, have sinoatrial block, a supra-hisian atrioventricular block and paroxysmal atrial arrhythmias which have led to partial atrial standstill in the eldest: left anterior hemiblock is also present in the two youngest brothers. The condition is very well tolerated. This family is compared to the other 12 reported cases of familial idiopathic binodal block in the adult, an autosomal dominant condition with variable penetration. The diagnosis is reserved and justifies endocavita

The Anatomical Substrate for Sick Sinus Syndrome in Dogs.

The hearts of 28 dogs, clinically diagnosed as having symptomatic sick sinus syndrome (SSS), were examined post mortem, with a particular focus on the sinus node (SN) region. The affected dogs were divided into two groups according to the findings of ambulatory electrocardiography: 16 dogs with severe sinoatrial (SA) block and/or sinus arrest (group A) and 12 dogs with long sinus or atrial pauses due to SA block and/or sinus arrest accompanied by atrial tachyarrhythmias (group B). The most significant histopathological changes found in both SSS groups were extensive destruction of the SN chara

Sinoatrial node function and the role of sinoatrial conduction in the typical atrial flutter substrate.

BACKGROUND: Sinoatrial node (SAN) activation and sinoatrial conduction pathways (SACPs) have been assessed in animals but not in humans. OBJECTIVES: We used ultrahigh-density mapping and simulated models to characterize the SAN and to investigate whether slowed SAN conduction may contribute to the atrial flutter (AFL) substrate. METHODS: Twenty-seven patients undergoing electrophysiologic procedures had right atrial mapping. SAN activation patterns and conduction block were analyzed. The interaction between the SAN and the intercaval line of block (LOB) was analyzed, and right atrial simulat

Myocardial perforation due to temporary transvenous pacing catheters in pediatric patients.

Over a 13-year period, 18 pediatric patients received temporary transvenous pacing catheters for symptomatic bradycardia due to complete atrioventricular block or sinoatrial node dysfunction. Right ventricular wall perforation occurred in two patients. The utility of two-dimensional echocardiography in the detection and assessment of myocardial perforation by temporary pacing catheters is discussed.

[Bradyarrhythmias: diagnostics].

Bradyarrhythmias cause symptoms such as syncope, dizziness, or exercise intolerance and lead to the implantation of > 100 000 pacemakers in Germany annually. They are caused either by sinus node dysfunction (sinus bradycardia, sinoatrial block, sinusarrest, preautomatic pauses in paroxysmal atrial fibrillation, chronotropic incompetence) or by atrioventricular (AV) conduction abnormalities (AV block, complex bundle branch block). Optimal therapy of bradycardia crucially depends on ECG documentation which frequently requires Holter monitoring in the case of intermittent bradycardia. The differe

[Disorders of heart rhythm in patients with mitral valve prolapse detected by continuous electrocardiography].

Prolapse of the mitral valve is a frequent occurrence. It is usually benign. However, because of frequent arrhythmias that can provoce sudden death, its detection and treatment are of great importance. In 20 patients with prolapse of the mitral valve the authors detected arrhythmias on a 24-hour electrocardiogram. The results were as follows: ventricular arrhythmias in 50% of patients; auricular arrhythmias in 50% of cases; atrioventricular and sinoatrial block in 5% of subjects, and block of the left bundle-branch in 2% of patients.
