Skull base in trigonocephaly.

The authors present aspects of the skull base in trigonocephalic children. Different patterns on clinical anthropometric investigation, bidimensional computer tomography (CT) scan and three-dimensional (3D) CT scan were studied. We present a series of 27 cases of trigonocephaly operated on in the department of pediatric neurosurgery at the Hôpital des enfants La Timone in Marseille since 1975. The skull base has been studied with CT scan in 12 patients and in 5 a 3D CT reconstruction was performed. A control series of 27 children without synostosis has been studied with the same patterns. We h

Intrauterine growing skull fracture.

Growing skull fractures with development of leptomeningeal cysts are rare complications of head injuries and have not been described in the perinatal period. The case history of a newborn with bilateral parietal fractures and the formation of a leptomeningeal cyst on one side detected at birth is presented. The importance of radiographic evaluation, including skull films, computered tomography, and magnetic resonance imaging, as well as the associated subarachnoid cyst and the age of presentation are discussed.

Extracranial skull base chondrosarcoma.

Chondrosarcoma of the skull base is a rare neoplasm. The most common presenting symptoms are hoarseness, dysphagia, and diplopia, which is associated with palsies of cranial nerves X, IX, and VI. The temporal bone is the most common site of tumor origin, followed by the sphenoid bone. These tumors must be differentiated from chordoma, chondroid chordoma, osteogenic sarcoma, enchondroma, and meningioma. Diagnosis is made by patient history, radiologic imaging, and biopsy. The treatment is surgical excision followed by radiation therapy. Five-year survival rates for grades I, II, and III are 90,

Compound depressed skull fracture.

Although compound depressed skull fractures are common and their management and diagnosis have been described at length, their occurrence following dog bites is unusual. The case of an 18-month-old infant with this injury as a complication of multiple dog bites in the facial and head regions is described with the treatment employed. The necessity of searching for skull fractures in children following dog bites in this region is emphasized.

Aspergillus Osteomyelitis of the Skull.

Osteomyelitis of the craniofacial skeleton is rare, with fungal pathogens least commonly implicated. The authors present 2 patients of osteomyelitis of the skull caused by Aspergillus spp. and discuss the diagnosis, clinicopathological course, and management strategies.Late recurrence seen in this type of infection warrants long-term follow-up and a high index of suspicion for the clinical signs associated with recurrence.Such patients would benefit from their surgical debridement being planned and managed via a specialist craniofacial unit, so as to utilize the most aesthetically sensitive ap

[A woman with a growing skull].

In a 79-year-old female, a markedly elevated alkaline phosphatase level was found by chance. She had noticed her hats had become too small. Clinical examination revealed a large skull with frontal bossing. Bone scintigraphy showed increased activity of bone metabolism in the skull. These three findings combined are considered a classic example of Paget's disease.

Plasmocytoma of the skull vault.

AIM: Plasma cell tumours include solitary plasmocytoma, multiple plasmocytoma and multiple myeloma. Herein we report a case of plasmocytoma of the skull. CASE HISTORY: A 49 year-old-man suffered from amnesia, irritability and a rubbery swelling which was gradually increased approximately to 4-5 cm in diameter at the frontal vertex over a year. His neurological examination was normal. The skull direct X-ray revealed patchy destruction of the left frontal bone 10 cm in diameter. CT with bone windows showed relatively preserved but severely thinned inner table and irregularly destructed outer ta

[A girl with a lacunar skull].

Following trauma, an X-ray was made of the head of a 5-year-old mentally impaired girl. A lacunar skull ('Lückenschädel') was seen, which was a coincidental finding. A lacunar skull develops when the fetal skull is poorly ossificated, probably due to a lack of distention of the cerebral ventricles.

[Ontogeny and phylogeny of the skull].

Ontogeny of the skull is primarily determined by genetic factors and is modified during development by environmental factors. The shape of the skull and the direction of its development during intrauterine life are determined by the requirements of the growing brain. After birth, the most important role in modifying the shape of the skull is played by large forces from the stomatognathic system. The development of the cranium is fastest during fetal life; at birth, its size is 65% of its final value. After birth, changes in the shape of the skull take place at a slower pace and coincide with e

Median defect in the skull.

A median defect in the region of the root of the nose, in between the two orbits, was discovered in the dried skull of a 44-year-old female cadaver, during routine undergraduate teaching. The two small nasal bones articulated with each other and the cribriform plate of the ethmoid. The lacrimal bones and frontal processes of the maxillary bones were also deformed. We propose that the median defect was due to abnormality at the fonticulus frontalis, the prenasal space and the interorbitonasal part of the nasal capsule, as well as defects in the ossification of the maxilla, lacrimal and frontal

[Cementoma of the skull base].

Cementomas result from the odontotic epithelial-mesenchymal complex and are located mainly in the area of large and small molar teeth of the mandible. There was an extremely rare cement location in the analyzed clinical cases. In the first case, cementoma of the frontal sinus spread into the ethmoidal sinus and eye socket in a female patient aged 38 years. The tumor is manifested by right frontalorbital pain and exophthalmos. There were no problems in removing the tumor and repairing the defect of the skull base. The second case was an extensive tumor in a 11-year-old child, which involves the

Imaging of the skull base.

Skull-base imaging has been a key factor in the advancement of skull-base surgery. The analysis of MR imaging or CT of the skull base emphasizes important landmarks, which are key to surgical planning. Although the definitive diagnosis usually is done by biopsy, the radiologist can limit the list of possibilities of the identity of a skull base lesion. The apparent site of origin is a key factor. Separation of cystic abnormalities from more solid enhancing abnormalities also is critical.

Blastomycosis of the skull base.

Blastomycosis is usually considered a pulmonary or cutaneous disease. A patient with blastomycosis of the infratemporal fossa temporal bone and sinuses without pulmonary disease is presented. Extensive skull base involvement required surgical debridement for relief of pain, and long-term systemic antifungal therapy. The literature is reviewed and discussed.

[Pathogenesis of osteoma of the skull].

The sites of osteomas of the frontal and temporal bone are mostly the sutures and only rarely the flat bones. In a retrospective analysis of 12 osteomas, we found that the localisation influences the histological architecture: In osteomas of suture origin, the fibrovascular component is significantly reduced. The aetiopathogenesis of these benign osteogenic tumours is discussed.

Myxoma of the skull and orbit.

An unusual case of myxoma of the skull extending into the orbital cavity is reported. Myxoma is a benign tumour of the soft tissues and maxillary bones, which often presents infiltrating growth and recurrences. Histologically it is characterized by abundant myxoid stroma and is often confused with other tumours which can present mucoid degeneration. Although diagnosis is impossible before histological examination, the possibility of a myxoma must be considered in the presence of extradural osteolytic poorly vascularized lesions.

[The skull in renal osteodystrophy].

Skull X-rays of 60 patients with chronic renal failure were examined. Alterations included diminished or increased bone density, radiolucent areas, pepper pot skull and the disappearance of vascular grooves and sutures. It is suggested that the radiological aspect of the skull is of very little diagnostic use in the assessment of uremic osteopathy since specific alterations are rare and tardive and show no correlation with clinical and laboratory findings. Skull X-rays can be useful in assessing the effects of treatment (vitamin D derivatives, parathyroidectomy) and for the identification of f

Osteomyelitis of the base of the skull.

Infection in the marrow of the temporal, occipital, and sphenoid bones is an uncommon, but increasing occurrence. It is usually secondary to infections beginning in the external auditory canal and is caused almost uniformly by the gram negative Pseudomonas aeruginosa bacteria. Technetium and gallium scintigraphy help in the early detection of such infections while CT scans demonstrate dissolution of bone in well-developed cases. Headache is the predominant symptom. Dysphagia, hoarseness, and aspiration herald the inevitable march of cranial nerves. We have diagnosed and treated 17 cases of ost
