Two-stage skull base surgery for tumours extending to the sub- and epidural spaces.

The surgical management of extensive skull base neoplasms, which often extend to both the sub- and epidural spaces, is still a great challenge with considerable risk. The authors report 12 cases in which a two-stage operation was performed for such nonmalignant tumours. The series consisted of four cavernous sinus meningiomas, one sphenoid-ridge meningioma, one cerebello-pontine angle meningioma, three pituitary adenomas, two chordomas, and one fibroma. Our operative strategy involved removal of the epidural part of the tumour and extensive skull base reforming during the first stage. After ap

Staged Open Cranial Surgery for Primary Intra-axial Neoplasms: A Systematic Review.

BACKGROUND: Staged surgery for skull base lesions has been utilized to facilitate maximal safe resection and optimize outcomes while minimizing morbidity and complications. Conversely, staged surgery for primary intraparenchymal neoplasms is less commonly performed and has not been reported as extensively within the literature. As such, we performed a systematic review to examine the unique surgical indications for staging, timing between stages, specific surgical approaches utilized, and postoperative complications of staged surgery for primary intra-axial neoplasms. METHODS: A literature se

Meningioma: a case of transcranial recurrence managed by base-of-skull technique and a review of the tumor.

Meningiomas are the most common benign intracranial neoplasms, and although the otolaryngologist rarely sees these lesions, he or she may encounter them in surgery of the skull base. Surgical removal is the primary treatment modality for these meningiomas. To avoid recurrent disease, the best initial therapy is the most extensive procedure that can reasonably be effected. Chemotherapy and irradiation therapy have, at best, only limited roles in treatment.

Extended orbitozygomatic approach to the skull base to improve access to the cavernous sinus and optic chiasm.

BACKGROUND: The orbitozygomatic approach to the skull base has evolved over the past century, with a surge in development during the past 20 years. We describe an extension of this technique involving removal of the most inferior portion of the temporal bone to the level of the internal carotid artery as it enters the carotid canal, to further facilitate exposure of tumors in this region. METHODS AND MATERIALS: We performed the extended orbitozygomatic approach in a series of 17 patients with a variety of neoplastic lesions. Our case series was reviewed in a retrospective fashion, and our sur

Pathology and Differential Diagnosis of Anterior Skull Base Tumours.

The anterior skull base is a complex anatomic site which may be involved by a large number of biologically heterogenous neoplasms. They arise from the epithelium, both surface mucosa and glands, as well as soft tissues, bone, and cartilage. Many benign and malignant tumours in the anterior skull base are similar to their counterparts in other anatomic sites. Interestingly, unique tumours including teratocarcinosarcoma, olfactory neuroblastoma, and angiofibroma can also be found. Recognition of overlapping morphologic features of entities encountered in this anatomic site and the corresponding

Major Internal Carotid Artery Injury During Endoscopic Skull Base Surgery: Case Report.

Major vascular structures are always at risk during complex skull base surgery, particularly with use of the endoscopic endonasal approach, and intraoperative damage of the internal carotid artery (ICA) can be a devastating complication. Herein, we report a case of a young patient who had a major injury of the left ICA during endoscopic resection of a recurrent petrous bone chordoma. Massive bleeding was controlled by a Foley balloon inserted and kept in the resection area. Urgent angiography revealed a persistent leak from the petrous segment of the left ICA, and the vessel was sacrificed wit

Aneurysmal bone cyst of the sphenoid bone extending into the ethmoid sinus, nasal cavity and orbita in a child.

An aneurysmal bone cyst (ABC) typically involves the long bones of the extremities, thorax, pelvis, or vertebrae. Skull base involvement is rare. We describe the case of a 9-year-old girl with ABC of the skull base. The patient had presented with nasal obstruction and headache over a period of approximately 8 months. The patient had no history of trauma or surgery. Physical and neurological examination findings normal. Magnetic resonance imaging (MRI) showed a multicystic lesion arising from the sphenoid sinus and extending into ethmoid sinus, superior nasal cavity and medial walls of the orbi

Maffucci syndrome and intracranial chondrosarcomas: a case report featuring spontaneous resolution of sixth nerve palsy.

Maffucci syndrome is a rare disease process characterized by enchondromatosis with cutaneous hemangiomatosis. We report a 20-year-old woman with Maffucci syndrome with a 5-day history of diplopia. She was found to have a left sixth nerve palsy due to a parasellar chondrosarcoma. Three weeks later, the patient's diplopia spontaneously resolved. This unusual clinical course prompted us to review frequency of sixth nerve palsy with skull base neoplasms and the phenomenon of spontaneous resolution of diplopia.

Chordoma: the nonsarcoma primary bone tumor.

Chordomas are rare, slowly growing, locally aggressive neoplasms of bone that arise from embryonic remnants of the notochord. These tumors typically occur in the axial skeleton and have a proclivity for the spheno-occipital region of the skull base and sacral regions. In adults, 50% of chordomas involve the sacrococcygeal region, 35% occur at the base of the skull near the spheno-occipital area, and 15% are found in the vertebral column. Craniocervical chordomas most often involve the dorsum sella, clivus, and nasopharynx. Chordomas are divided into conventional, chondroid, and dedifferentiate
