Doxepin is more effective than zolpidem in improving executive function in patients with insomnia disorder.

UNLABELLED: BACKGROUND : Insomnia disorder is associated with an impairment in cognitive performance. Doxepin and zolpidem have been found to be effective in improving sleep. In this study, we aimed to compare the effects of doxepin and zolpidem on sleep structure and executive function in patients with insomnia disorder. METHODS: Patients with primary insomnia were randomly assigned to receive doxepin 6 mg/day orally or zolpidem 5-10 mg/day orally. Polysomnography (PSG) and the Pittsburgh Sleep Quality Index (PSQI) were used at baseline and after the 8-week treatment to compare clinical effi

Cardiovascular Complications of Sleep Disorders: A Better Night's Sleep for a Healthier Heart / From Bench to Bedside.

Sleep is essential to and an integral part of life and when lacking or disrupted, a multitude of mental and physical pathologies ensue, including cardiovascular (CV) disease, which increases health care costs. Several prospective studies and meta-analyses show that insomnia, short (<7h) or long (>9h) sleep and other sleep disorders are associated with an increased risk of hypertension, metabolic syndrome, myocardial infarction, heart failure, arrhythmias, CV disease risk and/or mortality. The mechanisms by which insomnia and other sleep disorders lead to increased CV risk may encompass inflamm

Complex mixtures, complex responses: Assessing pharmaceutical mixtures using field and laboratory approaches.

Pharmaceuticals are present in low concentrations (<100 ng/L) in most municipal wastewater effluents but may be elevated locally because of factors such as input from pharmaceutical formulation facilities. Using existing concentration data, the authors assessed pharmaceuticals in laboratory exposures of fathead minnows (Pimephales promelas) and added environmental complexity through effluent exposures. In the laboratory, larval and mature minnows were exposed to a simple opioid mixture (hydrocodone, methadone, and oxycodone), an opioid agonist (tramadol), a muscle relaxant (methocarbamol), a s

Cigarette Smoking as Sleep Aid: Self-Medication Practices in Adolescents.

BACKGROUND: While smoking has been associated with alertness, it has also been reported as a stress relief and sleep aid. However, evidence on tobacco self-medication to improve sleep remains limited, particularly among adolescents. Therefore, the aim of this study was to explore the prevalence of tobacco self-medication in adolescents and analyze the association with sleep quality. METHODS: A cross-sectional descriptive study was conducted with 1,121 Spanish adolescents (mean age = 16.2 ± 0.7; 57.1% female) from public secondary schools. Participants completed assessments on their sleep qual

Evaluating the Safety of Prescription Sleep Medications in Patients Undergoing Total Knee Arthroplasty: A Propensity Matched Analysis.

BACKGROUND: Impaired sleep is reported in over 50% of patients after a total knee arthroplasty (TKA). Clinicians often treat postoperative sleep disturbances with medications; however, the risks associated with the use of sleep medications after TKA are not well understood. We assessed the association between sleep medication use after TKA and postoperative complications. METHODS: Using a national administrative claims database, we identified 101,806 patients undergoing primary TKA from 2011 to 2022 who were taking sleep medications within 90 days after surgery and 509,030 patients who were n

Psychotropic effects of L-theanine and its clinical properties: From the management of anxiety and stress to a potential use in schizophrenia.

Anxiety disorders are highly prevalent in modern societies, and are ranked the sixth most important contributor of non-fatal negative health outcomes. L-theanine is an amino acid naturally found in green tea (Camellia sinensis) and some other plant extracts, and recent clinical studies have proposed promising adjuvant effects of L-theanine for the negative impact of anxiety and psychological stress on health. In this integrative narrative review, we aimed to appraise and further discuss the effects of L-theanine administration on anxiety disorders and psychological stress. Published data sugge

Subclinical brain manifestations of repeated mild traumatic brain injury are changed by chronic exposure to sleep loss, caffeine, and sleep aids.

INTRODUCTION: After mild traumatic brain injury (mTBI), the brain is labile for weeks and months and vulnerable to repeated concussions. During this time, patients are exposed to everyday circumstances that, in themselves, affect brain metabolism and blood flow and neural processing. How commonplace activities interact with the injured brain is unknown. The present study in an animal model investigated the extent to which three commonly experienced exposures-daily caffeine usage, chronic sleep loss, and chronic sleep aid medication-affect the injured brain in the chronic phase. METHODS: Subcl
