Practitioner review: early developmental language delay: what, if anything, should the clinician do about it?

Early developmental language delay is characterized by slow development of language in preschoolers. The condition is frequent among two- and three-year-olds, causes concern among parents, and generates differences of opinion as to significance among informed professionals. Poorer long-term outcomes are much more likely if language delay persists until the later preschool years, and if the delay is not specific to language and/or includes problems in understanding. Specific language delay in the preschool period is better characterized as a risk factor than a disorder; most children with speci

Language XX: What shall it be called and why does it matter?

Terms suggested for unexpected language learning difficulty include specific language impairment, primary language disorder and language learning disability; however, none of them represents the critical element of literacy. Some experts have suggested that debating what to call language disorders is parallel to debating what to call reading disorders and dyslexia. In contrast, it is argued here that reading disorders are language disorders and not separate or parallel. Evidence is presented for quadrant models that explain language disorders and dyslexia along two dimensions-sound/word level

Screening and analysis of specific language impairment in young children by analyzing the textures of speech signal.

A child having a delayed development in language skills without any reason is known to be suffering from specific language impairment (SLI). Unfortunately, almost 7% kindergarten children are reported with SLI in their childhood. The SLI could be treated if identified at an early stage, but diagnosing SLI at early stage is challenging. In this article, we propose a machine learning based system to screen the SLI speech by analyzing the texture of the speech utterances. The texture of speech signals is extracted from the popular time-frequency representation called spectrograms. These spectrogr

Psychiatric implications of language disorders and learning disabilities: risks and management.

This article reviews the relationship between different learning disabilities, language disorders, and the psychiatric disorders that are commonly associated with learning disabilities and language disorder: attention-deficit hyperactivity disorder (ADHD), anxiety disorders, depression, and conduct or antisocial personality disorder. The complex associations between language disorders and specific learning disabilities--dyslexia, nonverbal learning disorder, dyscalculia--and the various psychiatric disorders are discussed. Clinical vignettes are presented to highlight the impact of these disor

Grammaticality of Tag Questions as a Longitudinal Morphosyntactic Marker of Children With Specific Language Impairment Compared to Peers Ages 5-18 Years.

PURPOSE: Previous studies documenting longitudinal linguistic outcomes of children with specific language impairment (SLI) compared to their age peers focus on the property of obligatory finiteness marking in sentences across the age span of 5-18 years. This study evaluates tag questions as syntactically complex sentences that extend the demands of finiteness marking across clauses, requiring coordination of negation in the base sentence and tag question. METHOD: Five hundred eleven children (240 unaffected, 271 SLI affected), between 5 and 18 years of age participated, following a rolling re

Diagnostic assessment of communicative and interactive behaviours in children with autism and receptive language disorder.

Children with autism and children with a severe specific receptive language disorder both show clear deficits in communicative language skills and in social relationships. In this study the usefulness of the Autism Diagnostic Observation Schedule (ADOS) in the differential diagnosis between these two groups of developmentally impaired children is assessed. 11 children with early infantile autism and 20 children with a specific receptive language disorder participated in the study. 18 children with an expressive language disorder were used as a control group. The ADOS was individually administe

Psychosocial outcomes at 15 years of children with a preschool history of speech-language impairment.

BACKGROUND: Evidence suggests there is a heightened risk of psychiatric disorder in children with speech-language impairments. However, not all forms of language impairment are strongly associated with psychosocial difficulty, and some psychiatric disorders (e.g., attention deficit/hyperactivity disorder (ADHD)) are more prevalent than others in language-impaired populations. The present study assessed the psychosocial adjustment in adolescence of young people with history of speech-language impairment, and investigated specific relationships between language deficits and psychiatric disorders

Retrieval Practice and Word Learning in Children With Specific Language Impairment and Their Typically Developing Peers.

Purpose In this article, we review the role of retrieval practice on the word learning and retention of children with specific language impairment (SLI). Method Following a brief review of earlier findings on word learning in children with SLI and the assumptions behind retrieval practice, four experiments are described that compared novel words learned in a repeated spaced retrieval condition and those learned in either a repeated study condition or a repeated immediate retrieval condition. Preschool-age children with SLI and their same-age peers with typical language development were the par

[Value of the Heidelberg Language Development Test in diagnosis of children with language development disorders].

Diagnosis of developmental speech and language disorders in accordance with the ICD-10 is to be based on the psychometric assessment of speech and language performance. In the present study the clinical validity of the Heidelberger Sprachentwicklungstest (HSET) for diagnosing expressive language disorders was assessed by comparing HSET subtest results for 92 children aged 4 to 10 years who had various speech and language disorders with the children's scores on another language test, a cognitive test and clinical findings. The HSET scores differed considerably from the clinical ratings of the s

The prevalence of specific learning disorder in mathematics and comorbidity with other developmental disorders in primary school-age children.

Mathematics difficulties are common in both children and adults, and they can have a great impact on people's lives. A specific learning disorder in mathematics (SLDM or developmental dyscalculia) is a special case of persistent mathematics difficulties, where the problems with maths cannot be attributed to environmental factors, intellectual disability, or mental, neurological or physical disorders. The aim of the current study was to estimate the prevalence rate of SLDM, any gender differences in SLDM, and the most common comorbid conditions. The DSM-5 provides details regarding these only f

Language and communication rehabilitation in patients with schizophrenia: A narrative review.

Language impairments often appear in patients with schizophrenia and are potential targets for rehabilitation. Clinical practice and research should be intimately connected. The aim was to perform a narrative review of the assessment and intervention tools that have been used for the rehabilitation of schizophrenia patients with language and communication impairments. Two types of tools, general and specific, were developed for both purposes. General tools include the Positive and Negative Syndrome Scale for assessment, and the Integrated Psychological Therapy for intervention. The specific to

The Relationship Between Language and Planning in Children With Language Impairment.

Purpose This study examined the relationship between language and planning, a higher order executive function skill, in children with specific language impairment (SLI) and typically developing (TD) children. We hypothesized differences between groups in planning performance and in the role of verbal mediation during planning. Method Thirty-one children with SLI and 50 TD age-matched peers (8-12 years) participated in the study. We assessed language ability via a standardized language measure and planning via a dual-task Tower of London paradigm with 3 conditions: no secondary task (baseline),

The Finite Verb Morphology Composite: Values From a Community Sample.

Purpose We investigated the finite verb morphology composite (FVMC), a measure associated with developmental language disorder (DLD), in a large community sample to provide evidence on its distribution, its association with other variables, and its sensitivity and specificity. Method We coded percent occurrence of FVMC morphemes in obligatory contexts in archival language samples from 676 six-year-olds conversing with an adult family member. Using multiple regression, we quantified the variance in FVMC scores accounted for by language sample characteristics (number of utterances and obligatory

The clinical examination of non-word repetition tasks in identifying Persian-speaking children with primary language impairment.

OBJECTIVE: This study aimed to examine the diagnostic accuracy of a non-word repetition (NWR) test in identifying Persian-speaking preschool children with specific/primary language impairment (PLI). Children with PLI show consistently poorer performance in non-word repetition tasks than their typically developing language (TDL) counterparts. It is assumed that the ability to repeat non-words triggers language skills and that the absence of this ability may be responsible for language impairment in PLI children. METHODS: Twenty preschool children with PLI participated in this study and were co

The relationship between cerebral hemisphere volume and receptive language functioning in dyslexia and attention-deficit hyperactivity disorder (ADHD).

Because poor comprehension has been associated with small cerebral volume and there is a high comorbidity between developmental dyslexia, attention-deficit hyperactivity disorder (ADHD), and specific language impairment, the goal of this study was to determine whether cerebral volume is reduced in dyslexia and attention-deficit hyperactivity disorder in general, as some suggest, or whether the reduction in volume corresponds to poor receptive language functioning, regardless of the diagnosis. Participants included 46 children with and without dyslexia and attention-deficit hyperactivity disord

Grammatical correction and morphological productivity tasks as potential identifiers of developmental language disorder in Spanish-Catalan bilingual children: a pilot study.

A critical problem for Catalan-Spanish bilinguals is the lack of assessment tools to conduct valid and accurate oral language evaluations. The purpose of this preliminary study was to provide pilot data for a new potential assessment tool in Catalan. We examined the possibility that two novel tasks, a grammatical correction (GramCorr) and morphological productivity (MP; nonword and word subtasks), could differentiate between Catalan-speaking children with and without developmental language disorder (DLD) with good diagnostic accuracy. Twenty-seven school-aged children with DLD (Mage = 9;1 year

Parent information and direct observation in the diagnosis of pervasive and specific developmental disorders.

Children with autism and children with a severe specific receptive language disorder show clear deficits in communicative language skills and social relationships. In this study the usefulness of a standardized parent interview (ADI-R) and a standardized observation schedule (ADOS-G) for the differential diagnosis of these two groups was assessed. Eleven children with early infantile autism and 16 children with a specific receptive language disorder participated. The parent interview was conducted with all parents and the observation schedule was administered to all children. Ten out of 11 chi

Deficits in auditory temporal and spectral resolution in language-impaired children.

Between 3 and 6 per cent of children who are otherwise unimpaired have extreme difficulties producing and understanding spoken language. This disorder is typically labelled specific language impairment. Children diagnosed with specific language impairment often have accompanying reading difficulties (dyslexia), but not all children with reading difficulties have specific language impairment. Some researchers claim that language impairment arises from failures specific to language or cognitive processing. Others hold that language impairment results from a more elemental problem that makes affe

Brain basis of childhood speech and language disorders: are we closer to clinically meaningful MRI markers?

PURPOSE OF REVIEW: Developmental speech and language disorders are common, seen in one in 20 preschool children, in the absence of frank neurological deficits or intellectual impairment. They are a key reason parents seek help from paediatricians. Complex neurogenetic and environmental contributions underpin the disorders, yet few specific causes are known. With the advent of quantitative brain imaging, a growing number of studies have investigated neural contributions. Here, we discuss current MRI approaches and recent findings (January 2014-June 2016) in the field. RECENT FINDINGS: Five rel
