Mandarin Electrolaryngeal Speech Recognition Based on WaveNet-CTC.

Purpose The application of Chinese Mandarin electrolaryngeal (EL) speech for laryngectomees has been limited by its drawbacks such as single fundamental frequency, mechanical sound, and large radiation noise. To improve the intelligibility of Chinese Mandarin EL speech, a new perspective using the automatic speech recognition (ASR) system was proposed, which can convert EL speech into healthy speech, if combined with text-to-speech. Method An ASR system was designed to recognize EL speech based on a deep learning model WaveNet and the connectionist temporal classification (WaveNet-CTC). This s

Intelligibility of oesophageal and tracheo-oesophageal speech: preliminary observations.

The purpose of this study was to compare the intelligibility of two types of alaryngeal speech commonly used after total laryngectomy. Four male oesophageal speakers and four male tracheo-oesophageal speakers read a series of monosyllabic words, multisyllabic words and sentences. The monosyllabic word list consisted of several minimal pairs for each of eight phonetic contrasts; multisyllabic words and sentences were not selected on specific phonetic grounds. Audio recordings of all subjects' readings were presented to eight naïve adult listeners who completed both an item identification task a

High-speed digital imaging and electroglottography of tracheoesophageal phonation by Amatsu's method.

BACKGROUND: Our previous findings have indicated that the thyropharyngeal muscles form a retropharyngeal prominence during alaryngeal phonation via the TE fistula. This prominence forms a so-called 'neoglottis', which is thought to function as the vibratory source. To better understand the mechanism of TE phonation, we analyzed the vibration of the neoglottis using electroglottography (EGG) and a high-speed digital imaging system. PATIENTS AND METHODS: Two volunteers who use TE phonation for their daily speech communication were subjected to this study. The vibrations of the neoglottis were r

Computerised measurement of fundamental frequency in Scottish neoglottal patients.

An electrolaryngograph has been used to investigate the fundamental frequency of neoglottal speech produced by patients who have undergone near-total (parsimonious) laryngectomy. The resulting estimates of fundamental frequency were compared with the fundamental frequency of normal and oesophageal speakers of a similar age and Scottish dialect. It was found that the average fundamental frequencies of neoglottal and oesophageal male speakers were higher than those in the normal group, though many fall into the normal range. The fundamental frequency of the single neoglottal female speaker was h

The ultra-low resistance Groningen voice prosthesis: aerodynamic properties.

Post-laryngectomy voice rehabilitation using the low resistance (LR) Groningen voice prosthesis has over the past years provided good voice and speech results. The valve part of the prosthesis is largely responsible for the airflow resistance of the prosthesis. This study was performed to evaluate if by modifying the valve part of the LR Groningen prosthesis a lower airflow resistance in vitro could be achieved. Several prototypes with modified valves were tested. Based on the aerodynamic measurements the prosthesis with a single slit of 200 degrees in the 'hat' of the esophageal flange was se

Pitfalls after laryngectomy and neoglottis formation.

Intrraluminal manometric investigations in the hypopharynx and sphincter area post laryngectomy revealed pressure recordings of 54 mm Hg and 33 mm Hg, respectively, during the act of swallowing. Aspiration of saliva or food after a laryngectomy with a hypopharyngeal-tracheal shunt can be explained on these grounds.

Novel esophageal speech therapy method in total laryngectomized patients: biofeedback by intraesophageal impedance.

The loss of the best communication port after total laryngectomy surgery makes speech rehabilitation an important goal. Our aim was to improve the quality of esophageal speech (ES) using online esophageal multichannel intra-luminal impedance (MII) as a new biofeedback method. Twenty-six total laryngectomized patients were included. Before ES therapy, an esophageal motility test was carried out. MII catheters were placed in all subjects who were then randomized into two groups. Group 1 included 13 cases, who were retrained according to the classical method. Group 2 included 13 cases, who were r

[Restoration of the voice function after subtotal laryngectomy and formation of tracheopharyngeal shunt].

We introduce our newly designed voice restoration technique for patients who have undergone subtotal laryngectomy for laryngeal and pharyngolaryngeal cancer. To study the efficacy of this technique, we analyzed the voice rehabilitation results in 14 patients with laryngeal and pharyngolaryngeal cancer. Voice function was restored in 13 (92.9%) patients 6-12 days after starting speech therapy. The middle voice, voice of medium timbre, long speech (phrase of 4-5 words) and speech sufficient for communication in everyday life and on the phone were formed. An acoustic analysis of spectral componen

The Nijdam voice prosthesis: a self-retaining valveless voice prosthesis for vocal rehabilitation after total laryngectomy.

Voice prosthesis-assisted speech rehabilitation after total laryngectomy has proven to be successful in the majority of patients and exceeds the results of traditional oesophageal speech. Nevertheless 10-30% failure rates are still reported. In part this is due to prosthesis-related problems, in particular ingrowth of Candida species in the valve-bearing parts of the devices. A new indwelling, low-resistance and valveless voice prosthesis is described; the Nijdam voice prosthesis. The device has an average device lifetime of 19 weeks which appears to be superior to other indwelling voice prost

[Secondary surgical voice rehabilitation].

Surgical voice restoration is possible years after laryngectomy by using hypopharyngeal puncture and cricopharyngeal myotomy if necessary. We have developed a technique of insertion of the ESKA-Herrmann tracheostoma valve for fingerfree speech. The patient moistens and cleans the tracheostoma valve every day. The quality of fingerfree speech is the best of all known techniques.

[A new method of surgical voice rehabilitation: microsurgical jejunum transplantation].

A new method of surgical rehabilitation of the voice is being presented. A shunt is introduced between the trachea and the hypopharynx or the entrance to the oesophagus, by the use of a jejunal graft which is sutured isoperistaltically. By microsurgical anastomoses the jejunum is revascularized by the cervical vessels. Seven patients, having undergone total laryngectomy because of carcinoma of the larynx, have been treated by this procedure. In one case a venous thrombosis developed and the graft had to be resected. The other patients regained a voice, able to meet all requirements of daily li

[A new procedure for reconstruction of the voice--neoglottis of small intestine].

The fundamental concept in vocal rehabilitation after laryngectomy is the tracheo-hypopharyngeal shunt. The usual contemporary methods possess numerous disadvantages. In order to improve the results a syphon-fashioned shunt with free transplanted jejunum between the tracheal stump and the hypopharynx is created (stimulated by K. Ehrenberger). Aspiration can be avoided by means of the autonomic peristalsis of the implant. The procedure has been performed in 7 cases. One developed necrosis of the loop. The other patients possess a clear voice.

Jejunal grafts for reconstructing a phonatory neoglottis in laryngectomized patients.

After laryngopharyngectomy and reconstruction of the upper digestive tract, a free jejunal graft can be anastomosed in an isoperistaltic direction end-to-end to the trachea and the hypopharynx in order to form a shunt for vocal rehabilitation. Following placement of the shunt, no special care is required nor does any patient show any aspiration. Because of a low-flow resistance of the shunt, patients so treated possess long-lasting phonation with loud and modulated voices.

Vocal tract area functions from magnetic resonance imaging.

There have been considerable research efforts in the area of vocal tract modeling but there is still a small body of information regarding direct 3-D measurements of the vocal tract shape. The purpose of this study was to acquire, using magnetic resonance imaging (MRI), an inventory of speaker-specific, three-dimensional, vocal tract air space shapes that correspond to a particular set of vowels and consonants. A set of 18 shapes was obtained for one male subject who vocalized while being scanned for 12 vowels, 3 nasals, and 3 plosives. The 3-D shapes were analyzed to find the cross-sectional

In vitro experiments with the triangular flap cutter applied to functional tracheoesophageal shunt for voice rehabilitation after total laryngectomy.

BACKGROUNDS: Since the introduction and practice of functional tracheoesophageal shunt (FTES) for voice rehabilitation after total laryngectomy, patients have often suffered from fistula-related complications such as aspiration, tract stenosis and infection partly as a result of nonstandardized or incorrectly made flap operation. In striving to solve these problems as far as possible, a triangular flap cutter (TFC) was developed to perform the flap operation. METHODS: Flap operations were performed using a TFC on 10 tracheoesophageal models (pigs) and 10 fresh esophageal models (esophageal ca

[Phoniatric aspects of reconstructive laryngectomy].

Reconstructive laryngectomy has been performed at the E.N.T. Clinic in Ferrara for some time. After surgery the patients undergo speech therapy and phoniatric treatment for a period of time varying from 2 to 6 months. In order to better evaluate the vocal quality obtained after such reeducation the vocal emissions of 25 patients were examined and recorded. The recorded material, made up of prolonged vowels and 6 phonetically balanced sentences, was then evaluated by a panel of 7 "trained" listeners. The evaluation score-card proposed by Woiers in 1977 was used in taking the data. This not only
