An evaluation of speech audiometry by bone conduction in hearing-impaired adults.

Pure-tone thresholds, speech reception thresholds (SRT), and speech discrimination scores (DS) were obtained by ac and bc on 20 normal-hearing and 30 hearing-impaired adults. Correlations between the SRT and pure-tone thresholds (ave. of .5, 1, and 2 kc/s) were significant by both ac and bc. Correlations between DS by ac and bc were also highly significant, indicating the value of the bc DS in specific difficult cases. The clinical observation was confirmed statistically of decreasing DS with increasing sensorineural hearing loss.

Psychophysical suppression effects for tonal and speech signals.

This experiment assessed the benefits of suppression and the impact of reduced or absent suppression on speech recognition in noise. Psychophysical suppression was measured in forward masking using tonal maskers and suppressors and band limited noise maskers and suppressors. Subjects were 10 younger and 10 older adults with normal hearing, and 10 older adults with cochlear hearing loss. For younger subjects with normal hearing, suppression measured with noise maskers increased with masker level and was larger at 2.0 kHz than at 0.8 kHz. Less suppression was observed for older than younger subj

Masked speech recognition by 6-13-year-olds with early-childhood otitis media: effects of acoustic condition and otologic history.

OBJECTIVE: To investigate speech recognition in school-age children with early-childhood otitis media (OM) in conditions with noise or speech maskers with or without interaural differences. To also investigate the effects of three otologic history factors. DESIGN: Using headphone presentation, speech recognition thresholds (SRTs) were measured with simple sentences. As maskers, stationary speech-shaped noise (SSN) or two-talker running speech (TTS) were used. The stimuli were presented in a monaural and binaural condition (SSN) or a co-located and spatially separated condition (TTS). Based on

Predicting performance on the digits-in-noise test in noise and reverberation using the speech transmission index.

This study investigated the effects of noise and reverberation on recognition of Dutch digits-in-noise (DIN) test triplets. The findings demonstrate that the STIDIN, an adaptation to the standard speech transmission index (STI), provides a reliable, low‑error predictor of DIN test performance in conditions with noise and/or reverberation when only a single speech recognition threshold (SRT) measurement in noise is completed. To achieve this, twenty-four normal-hearing (NH) adults completed adaptive SRT measurements in noise-only, reverberation-only and combined noise and reverberation conditio

Challenging Times for Cochlear Implant Users - Effect of Face Masks on Audiovisual Speech Understanding during the COVID-19 Pandemic.

Unhindered auditory and visual signals are essential for a sufficient speech understanding of cochlear implant (CI) users. Face masks are an important hygiene measurement against the COVID-19 virus but disrupt these signals. This study determinates the extent and the mechanisms of speech intelligibility alteration in CI users caused by different face masks. The audiovisual German matrix sentence test was used to determine speech reception thresholds (SRT) in noise in different conditions (audiovisual, audio-only, speechreading and masked audiovisual using two different face masks). Thirty-seve

Effects of age and hearing loss on the relationship between discrimination of stochastic frequency modulation and speech perception.

OBJECTIVE: The frequency modulation (FM) of speech can convey linguistic information and also enhance speech-stream coherence and segmentation. The purpose of the present study was to use a clinically oriented approach to examine the effects of age and hearing loss on the ability to discriminate between stochastic patterns of low-rate FM and determine whether difficulties in speech perception experienced by older listeners relate to a deficit in this ability. DESIGN: Data were collected from 18 normal-hearing young adults, and 18 participants who were at least 60 years old, nine of whom had n

A comparative study on speech in noise understanding with a direct acoustic cochlear implant in subjects with severe to profound mixed hearing loss.

The aim of this study was to investigate the efficacy of a direct acoustic cochlear implant (DACI) for speech understanding in noise in patients suffering from severe to profound mixed hearing loss (MHL) due to various etiologies compared to the preoperative best-aided condition. The study was performed at five tertiary referral centers in Europe (Belgium, Germany, Poland and Spain). Nineteen adult subjects with severe to profound MHL due to (advanced) otosclerosis, ear canal fibrosis, chronic otitis media, tympanosclerosis or previous cholesteatoma were implanted with a DACI (Codacs™ Investig

Hearing performance in single-sided deaf cochlear implant users after upgrade to a single-unit speech processor.

INTRODUCTION: Single-sided deaf (SSD) patients report multiple benefits after cochlear implantation (CI), such as tinnitus suppression, speech perception, and sound localization. The first single-unit speech processor, the RONDO, was launched recently. Both the RONDO and the well-known behind-the-ear (BTE) speech processor work on the same audio processor platform. However, in contrast to the BTE, the microphone placement on the RONDO is different. The aim of this study was to evaluate the hearing performances using the BTE speech processor versus using the single-unit speech processor. Subjec

Use of a monosyllabic adaptive speech test (MAST) with young children.

Sixty children aged 3, 5, and 7 years were tested using a simple up-down adaptive speech threshold procedure. The test stimuli were familiar monosyllabic words presented as a closed set with a picture-pointing response. The results indicate that monosyllabic adaptive speech test (MAST) procedures can be used reliably with children as young as 3 years of age. Thirty of the children also received a different randomization of the same speech stimuli presented at a constant level, equal to their MAST threshold. The results confirmed the accuracy of the MAST estimate of the children's 50% speech th

Previous experience as a confounding factor in comparing cochlear-implant processing schemes.

It is of great importance to compare the relative merits of different cochlear-implant speech-processing strategies. Some groups have compared different strategies within single subjects, but usually the subject has prior experience with one strategy, and no allowance is made for this prior experience. We show in the present study that this is inappropriate. We tested one subject using the Melbourne (Cochlear Corp.) multichannel implant with the device set to process sounds in two different ways. In the first processing scheme, the device functioned normally, extracting information about voici

Decreased Speech-In-Noise Understanding in Young Adults with Tinnitus.

OBJECTIVES: Young people are often exposed to high music levels which make them more at risk to develop noise-induced symptoms such as hearing loss, hyperacusis, and tinnitus of which the latter is the symptom perceived the most by young adults. Although, subclinical neural damage was demonstrated in animal experiments, the human correlate remains under debate. Controversy exists on the underlying condition of young adults with normal hearing thresholds and noise-induced tinnitus (NIT) due to leisure noise. The present study aimed to assess differences in audiological characteristics between n

Estimates of basilar-membrane nonlinearity effects on masking of tones and speech.

OBJECTIVE: The aim of this experiment was to assess the contribution of cochlear nonlinearities to speech recognition in noise for individuals with normal hearing and a range of quiet thresholds. For signals close to the characteristic frequency (CF) of a place on the basilar membrane, the normal growth of response of the basilar membrane is linear at lower signal levels and compressed at medium to higher signal levels. In contrast, at moderate to high CFs, the basilar membrane responds more linearly to stimuli at frequencies well below the CF regardless of input level. Thus, for moderate-leve

Genetic and environmental influences on hearing in older women.

BACKGROUND: This study examined the relative contribution of genetic and environmental effects on the air-conducted hearing threshold level (0.5-4 kHz) and speech recognition threshold level of the better ear as well as self-reported hearing in older women. METHODS: Hearing was measured as a part of the Finnish Twin Study on Aging in 103 monozygotic (MZ) and 114 dizygotic (DZ) female twin pairs aged 63-76 years. Audiometric measured hearing was tested using standardized methods in soundproof conditions. Self-reported hearing was assessed by a structured question. Quantitative genetic modeling

Adaptation of the HINT (hearing in noise test) for adult Canadian Francophone populations.

The HINT provides an efficient and reliable method of assessing speech intelligibility in quiet and in noise by using an adaptive strategy to measure speech reception thresholds for sentences, thus avoiding ceiling and floor effects that plague traditional measures performed at fixed presentation levels A strong need for such a test within the Canadian Francophone population, led us to develop a French version of the HINT. Here we describe the development of this test. The Canadian French version is composed of 240-recorded sentences, equated for intelligibility, and cast into 12 phonemically

Evaluation of the speech perception in noise (SPIN) test.

Several investigations were perfomed with normal hearing subjects to determine the effects of presentation level and signal-to-babble ratio on the speech perception in noise (SPIN) test. The SPIN test contains sentences that simulate a range of contextual situations encountered in everyday speech communication. Findings from several representative patients with sensorineural hearing loss demonstrate the possible clinical utility of the test to measure the effects of context on speech discrimination.

Speech-in-noise intelligibility does not correlate with efferent olivocochlear reflex in humans with normal hearing.

CONCLUSION: According to the presented data, speech-in-noise intelligibility (SI) does not correlate with olivocochlear efferent activity - as measured by contralateral suppression (CS) of distortion product otoacoustic emissions (DPOAE) in humans with normal auditory threshold. OBJECTIVES: Literature data indicate a possible role of the medial olivocochlear efferents in speech intelligibility, especially in background noise. The objective of this study was to investigate this relationship. MATERIALS AND METHODS: SI was evaluated in three independent sessions by determining the ratio speech

The intelligibility of sentences in quiet and in noise in aged listeners.

The monaural speech-reception threshold (SRT) for sentences was investigated in quiet and under four noise conditions for 80 male subjects (age 60-90) and 30 female subjects (age 71-89). The noise levels used were 28, 43, 58, and 73 dBA. The noise had the long-term average spectrum of speech. It is shown that a model developed by Plomp [J. Acoust. Soc. Am. 63, 533-549 (1978)], which interprets any hearing loss for speech (SHL) as a combination of a loss of class A (attenuation of both speech and noise) and a loss of class D (distortion of the sound signals), enables one to make an accurate des

Effect of reverberation and noise on the intelligibility of sentences in cases of presbyacusis.

For 80 male subjects (age 60--90) and 30 female subjects (age 71--89) the monaural Speech-Reception Threshold (SRT) for sentences was investigated under five reverberation conditions at a constant noise level. The reverberation times used were between 0 and 2.3 s. The noise, with the long-term average speech spectrum, had a level of 52.5 dBA. Each subject was assigned to one of several subgroups formed on the basis of the maximum reverberation time at which the subject was still able to understand the sentences correctly The mean SRTs and the standard deviations are given, per subgroup, as a f

Polyarteritis nodosa and cochlear implantation.

Polyarteritis nodosa is a systemic disease which affects the small to medium-sized muscular arteries. Sudden or progressive, bilateral hearing loss is a presenting otologic manifestation. To date, no case of cochlear implantation in patients with polyarteritis nodosa has been reported. The authors present a case of polyarteritis nodosa (confirmed by biopsy) in a 71-year-old man with progressive, bilateral sensorineural hearing loss who underwent cochlear implantation. A successful full insertion of the Nucleus 3G electrode array was achieved without surgical or post-operative complications. Th

Elevation of internal auditory canal pressure by vestibular schwannomas.

OBJECTIVE: The exact mechanism of hearing loss, the most common presenting symptom in patients with vestibular schwannomas, remains unclear. To test whether increased pressure in the internal auditory canal from tumor growth is responsible for this clinical finding, the intracanalicular pressure in patients harboring these tumors was measured. STUDY DESIGN: Prospective study. SETTING: Tertiary referral hospital. PATIENTS: Fifteen consecutive patients undergoing a retrosigmoid approach for resection of vestibular schwannomas were included in the study. INTERVENTION: The intracanalicular pre
