The rarity of isolated sphenoid sinus disease (ISSD) and the specificity of pediatric populations meant that a separate analysis was required in this study. This study aimed to present and discuss the results of an analysis of clinical manifestations, radiological findings and surgical methods based on a large series of exclusively pediatric patients. The study group covered 28 surgically treated children (aged 5.5-17.5 years). The medical data were retrospectively analyzed and meticulously discussed with regards to presenting signs and symptoms, radiographic findings, surgical approaches, com

We report 2 cases of a 29- and a 37-year-old male patient both having sphenoid sinus barotrauma associated with free diving at about 12-m depth. A unilateral occupation of the sphenoid sinus was revealed in both cases by computed tomography and magnetic resonance imaging examination of the paranasal sinuses.

Mucoceles of the sphenoid sinus are rare and may frequently remain undiagnosed until symptoms due to compression of surrounding structures set in. In such cases, the optic nerve, because of its close proximity to the sphenoid sinus, is at risk, and pressure exerted by the mucocele may result in visual impairment. A case of a sphenoid sinus mucocele with sudden-onset unilateral blindness as the only presenting symptom has been described. Early diagnosis and prompt surgical intervention enabled the vision to be restored completely.

OBJECTIVE: To improve the correct diagnosis for isolated sphenoid sinus diseases.
METHODS: Twelve patients with isolated sphenoid disease from 1991 to 1998 were reviewed. The factors of misdiagnosis were discussed.
RESULTS: In this group of 12 patients with isolated sphenoid diseases, 8 failed to be diagnosed at the first time of medical consultation. The misdiagnosis included optic neuritis, neuralgia, ophthalmitis, pulmonary tuberculosis, epistaxis and epilepsy.
CONCLUSIONS: The main causes of misdiagnosis were: low incidence of the disease, nonspecific clinical symptoms, and unawareness

The clinical presentation of sphenoid sinus mucocele is usually dependent on the degree of involvement of adjacent structures. Since extension can be in any direction, there is a wide spectrum of symptomatology and clinical diagnosis can be difficult. Five cases of sphenoid sinus mucocele, four of which were demonstrated by computed tomography (CT), are presented. CT with its multiplanar display capability, is the diagnostic procedure of choice. It not only demonstrates the extent of the lesions non-invasively, but its ability to demonstrate bony expansion and destruction as well as density an

Olfactory neuroblastoma (ONB) is a rare tumour of the skull base, typically originating from the nasal cavity and around the cribriform plate. We present the rare case of ONB originating from and limited to the sphenoid sinus in a 42-year old lady. Pre-operatively the lesion was thought to be a sinonasal polyp and underwent functional endoscopic sinus surgery (FESS) and total excision of the polypoid lesion. Review of histology unexpectedly revealed ONB. She underwent further surgery to ensure wide local excision was achieved with negative margins on histology, followed by radiotherapy. This i

We report the case of an 18-year-old male patient operated on for sphenoid sinus barotrauma after scuba diving. The patient attended our emergency department because of intractable headache but did not improve with conservative treatment. After computed tomography and magnetic resonance imaging examination, he was diagnosed with sphenoid sinusitis that extended to the nasal septum. He therefore underwent surgery for sinus ventilation and abscess drainage.

OBJECTIVES: To study the growth and size of the sphenoid sinuses in choanal atresia patients.
STUDY DESIGN: Case-control single blinded retrospective study.
METHODS: Fifteen non-syndromic patients (ages 0-26 years old) were selected because they had at least one suitable CT scan (nine bilateral and 11 unilateral=total 20 scans). Twenty age-matched CT scans were used as controls. The following linear measurements were obtained: (1) fronto-zygomatic width, (2) zygomatic width, (3) width of the maxillary sinuses, (4) width of the sphenoid sinuses. Statistical analysis was performed with the exa

OBJECT: The aim of the study was to perform endoscopic anatomical studies of skull base structures through the sphenoid sinus in order to better understand endoscopic approaches to skull base lesions.
METHODS: Anatomical studies were performed on six cadavers using sinus endoscopes. The sphenoid sinus was entered via a sublabial nasoseptal approach. Stepwise removal of bone surrounding the sphenoid sinus was done to expose the suprasellar area, the retrosphenoid area and the cavernous sinus. The suprasellar dissection exposed the anterior communicating artery (ACom) complex, optic apparatus,

Paranasal sinus agenesis is an unusual clinical condition. The frontal sinus agenesis is more frequently seen, whereas agenesis of the sphenoid sinus is extremely rare. This report presents three new cases of sphenoid sinus agenesis. The absence of the sphenoid sinus was demonstrated by computed tomography (CT) in all cases.

The aetiological spectrum of sphenoid sinus mucocele includes congenital anomaly, trauma, infection, allergy and surgery of the sphenoid sinus. Enlargement of the mucocele, even with a short history, can result in progressive expansion of the sinus and extension of the lesion into the pituitary fossa, the suprasellar region, nasopharynx, orbits, clivus or ethmoid air cells. It is a benign cystic lesion with an excellent prognosis when treated appropriately. Generally, these lesions are managed by an ear, nose and throat surgeon, but when there is extension into the sellar and parasellar (espec

A case of an ectopic prolactin-producing tumor located within the sphenoid sinus is reported. It was discovered in a girl who experienced secondary amenorrhea without galactorrhea. We discuss the different diagnoses proposed before immunocytochemical staining results were available. This case presented unusual radiological, surgical, and histopathological aspects.

Two patients with sphenoethmoid mucoceles developed visual field defects consistent with optic chiasm dysfunction. The presence of chiasmal field defects associated with the clinical syndrome of compressive optic neuropathy should not rule out the diagnosis of sphenoethmoid mucocele since definitive treatment can be carried out via transnasal drainage.

BACKGROUND: Simultaneous appearance of sphenoid sinus aspergilloma and pituitary adenoma is a very rare finding.
METHODS: Retrospective analysis of patients with sellar and sphenoid sinus mass lesions who underwent trans-sphenoidal surgery was performed. Demographic data, medical history, predisposing factors, clinical picture, neurological status and radiological findings were reviewed. All patients underwent a trans-sphenoidal microsurgical treatment, and acquired specimens underwent both histopathological and microbiological analysis.
RESULTS: Sphenoid sinus aspergilloma was encountered i

Alongside the past development of endoscopic sinus surgery, knowledge about the anatomy of the sinuses has become crucial for surgeons. The sphenoid sinus is one of the most variable of all sinuses. Its relations to vital vascular and nervous elements make its approach a challenge for endoscopic surgeons. These relations include the internal carotid artery, the optic nerve, the pituitary gland, the Vidian nerve, and other vascular and nervous elements depending on the size of the sinus. This paper will make a review of these anatomical relations and their importance in endoscopic sphenoid surg

We have analyzed the clinical data of 27 cases of sphenoid inflammatory neoplasms and sphenoid sinus tumors including inflammations (12 cases), benign tumors (10 cases) and malignant tumors (5 cases). The characteristics of sphenoid inflammatory neoplasms were compared with those of sphenoid sinus tumors. On CT scanning and magnetic resonance imaging (MRI) we found that sinusitis resulted in much less bony erosion than tumors on CT scanning and that the T2-signal intensity on MRI increased much more in sinusitis than in tumors. We have discussed the indications of various operative approaches

Mucoceles involving the sphenoidal sinus are not as common as those affecting the frontal and ethmoidal sinuses. The subtle expansile change produced by a sphenoidal sinus mucocele often requires conventional tomography or computed tomography examination to be detected. Similarly, involvement of surrounding structures by a sphenoidal mucocele may require special study, which this section illustrates. The sphenoidal enlargement and density change produced by fibrous dysplasia may mimic sinus disease. Focal or widespread bone destruction resulting from neoplastic involvement of the sphenoidal si
