OBJECTIVES: To update the French guidelines on the management of trauma patients with spinal cord injury or suspected spinal cord injury.
DESIGN: A consensus committee of 27 experts was formed. A formal conflict-of-interest (COI) policy was developed at the outset of the process and enforced throughout. The entire guidelines process was conducted independently of any industrial funding (i.e. pharmaceutical, medical devices). The authors were advised to follow the rules of the Grading of Recommendations Assessment, Development and Evaluation (GRADE®) system to guide assessment of quality of ev

A series of 511 patients after vertebral column and spinal trauma admitted to Department of Neurosurgery of University of Medical Sciences, Poznań from 1979 to 1992 has been analyzed. Isolated vertebral and spinal injuries in 49 per cent. In 51 per cent they were associated with multiple injuries. Traffic accidents constituted the main cause (50 per cent of multiple injuries). The severity of the injury was assessed on the base of AIS, ISS rating systems and spinal symptoms. The concomitant multiple injuries were no cause of delay to surgical intervention within vertebral column. The short-ter

Neonatal spinal cord injury is rare, and diagnosis is often delayed. The causes of spinal cord injury are quite broad including mechanical trauma, ischemia in the prenatal or perinatal period, iatrogenic causes, or other etiologies such as spinal abnormalities or neoplasms. The perinatal history and specific physical examination findings can suggest a diagnosis of neonatal spinal cord injury, which warrants confirmation by magnetic resonance imaging. Management involves supportive care and physical and occupational therapy. Early recognition is paramount to determine the specific cause, manage

Traumatic spinal cord injury (SCI) has devastating consequences for the physical, social and vocational well-being of patients. The demographic of SCIs is shifting such that an increasing proportion of older individuals are being affected. Pathophysiologically, the initial mechanical trauma (the primary injury) permeabilizes neurons and glia and initiates a secondary injury cascade that leads to progressive cell death and spinal cord damage over the subsequent weeks. Over time, the lesion remodels and is composed of cystic cavitations and a glial scar, both of which potently inhibit regenerati

The drastic changes in body composition following spinal cord injury (SCI) have been shown to play a significant role in cardiovascular and metabolic health. The pattern of storage and distribution of different types of adipose tissue may impact metabolic health variables similar to carbohydrate, lipid and bone metabolism. The use of magnetic resonance imaging provides insights on the interplay among different regional adipose tissue compartments and their role in developing chronic diseases. Regional adipose tissue can be either distributed centrally or peripherally into subcutaneous and ecto

Acute spinal cord injury (SCI) is a devastating event with high mortality and, among survivors, a high degree of morbidity due to both motor and sensory deficits. The damage that occurs with a SCI is recognized to be pathophysiologically biphasic: the initial insult to neural tissue can be followed by a secondary process of progressive ischemia that may worsen the severity of dysfunction. The extent of this secondary insult is potentially modifiable, and a variety of interventions have been studied in an attempt to improve motor and sensory outcomes. What follows is a brief review of some such

A spinal cord injury leads to disturbances of sensory and motor signals due to the damage to white matter and myelinated fiber tracts. Moreover, the damage to gray matter causes segmental loss of interneurons of dorsal horn and motoneurons and restricts the therapeutic options. Neuroprotective strategies have the potential to improve the neurological outcome of patients. To achieve this, concerns to anesthetics or analgesics as neuroprotective interventions have been accumulating to explore neuroprotection during perioperative period. This review includes consideration of: 1) basic concepts of

Acute spinal cord injury is a devastating disease with enormous repercussions, not only for the victims and their families but for society as a whole. Despite the advent of novel medical therapies for the treatment of these injuries, many patients with spinal cord injury remain severely incapacitated and dependent on their families and/or specialized nursing care. Much of the controversy in the treatment of these injuries stems from insufficient knowledge about the pathophysiology of the disease as well as the timing of certain treatments such as surgery. We discuss the diagnosis and managemen

The years after SCI may be associated with acceleration of the aging process because of diminished physiologic reserves and increased demands on functioning body systems. Clinicians with expertise in the treatment and prevention of SCI-specific secondary complications need to collaborate with gerontologists and primary care specialists and need to invest in the training of future physicians to ensure a continuum of accessible, cost-effective, and high-quality care that meets the changing needs of the SCI population. Managed care payers often do not adequately cover long-term disability needs t

108 pigs were shot at the level of lumbar 1 to investigate the changes of spinal cord in relation to the speed of shooting and the trajectory through which the bullet passed. The cord lesions of the specimens could be divided, according to its severity, into 4 grades: (1) transection in 41; (2) complete destruction in 43; both lesion 1&2 resulted in permanent paraplegia; (3) incomplete destruction in 13, those resulted in incomplete paraplegia eventually recovered; (4) mild damage, that was concussion in 11, resumed perfectly. A survey of 200 women, admitted with spinal cord injuries over a 20-year-period, revealed that such injuries were much less common in females than males, with a ratio of 1 female to 4.4 males. Women had a higher ratio of cervical injuries, mainly due to elderly ladies falling downstairs at home. Most women (60 per cent) achieved full independence, pressure sores were seldom a problem, and some women were able to return to work in offices and teaching. Women were very rarely at risk of renal failure. Most complete tetraplegics required permanent catheter drainage, with a 30 per cent incidence o

In this review of the prognosis of patients having neonatal spinal cord injury, findings in 3 patients and 11 case histories from the literature are evaluated as to obstetric complications, clinical and pathologic examinations, and follow-up data from 2 to 12 years, or until death of the child. Eight of the 14 patients died: 4 at less than 3 months of age, 3 between 3 months and 1 year of age, and 1 at 3 1/2 years of age. Six children survived for more than 2 years for follow-up examination. Survival is dependent on the level of the lesion, with cervical lesions being almost incompatible with

11 cases of children with birth injury to the spinal cord born between 1960 and 1970 are presented in review and compared to about 200 previously published cases. 8 children presented at delivery with one or both feet and 2 with breech. 9 of these infants were born by difficult extraction and needed resuscitation due to primary asphyxia. One child had an easy vertex delivery without evident risk. Diagnosis was established within the first days of life, based on flaccid motor and sensory paralysis below a defined level, mostly in the cervical or upper thoracic spine, with bladder and bowel para

The literature dealing with radiation myelopathy is reviewed. The following points are to be noticed:radiation myelopathy is a rare complication in the radiation therapy of extraspinal tumors, nevertheless the number of case reports is increasing during the last years; probably this is due to the increasing use of high energy therapy. Already a cord dose of 1000 rad may be dangerous; with an increasing dose the risk of radiation myelopathy is increasing too. Besides the total dose the incidence of radiation myelopathy depends on the rate of delivery, the over-all time of administration, the si

The spinal injured child has speical needs owing to the processes of physical, mental and social growth. Goals of physical treatment programs include prevention of: genitourinary complications; contractures; pressure sores; long bone fractures, hip subluxation and dislocation; spinal deformity. Nonoperative treatment of spinal deformity employing external support should be initiated when the potential for spinal deformity exists. External support delays the development of spinal deformity, improves sitting balance and allows free upper extremity use. The overall treatment programs must conside

The pathomechanism of injuries of the spinal cord due to injuries of the cervical spine in 158 patients treated during the last ten years has been presented. It has been determined that total and irreversible injuries of the spine occurred most often after fractures of the spine with displacement from hyperflexion (29.7 per cent) crushed fractures (24.7 per cent), and dislocation of the spine (24.7 per cent). In the majority of the patients, the spinal cord was irreversibly damaged already at injury, and in 29.2 per cent, the symptoms of deformity of the lumen of the vertebral canal and compre
