Comparison of neuropathic pain and neuronal apoptosis following nerve root or spinal nerve compression.

Altered dorsal root ganglion (DRG) function is associated with neuropathic pain following spinal nerve injury. However, compression of the cauda equina and dorsal rhizotomy proximal to the DRG do not induce significant pain, whereas in the spinal nerve and peripheral nerve, injury distal to the DRG does induce neuropathic pain. Caspase signaling induces apoptosis, and caspase inhibitors prevent pain-related behavior. The degree of DRG neuronal apoptosis is thought to play a role in pain behavior. We suggest that differences in pain behavior according to the injury sites within the DRG may be r

Diffusion-tensor imaging of small nerve bundles: cranial nerves, peripheral nerves, distal spinal cord, and lumbar nerve roots--clinical applications.

OBJECTIVE: The purpose of this article is to review recent advances in diffusion-tensor imaging (DTI) and tractography of the cranial and peripheral nerves. CONCLUSION: Advances in MR data acquisition and postprocessing methods are permitting high-resolution DTI of the cranial and peripheral nerves in the clinical setting. DTI offers information beyond routine clinical MRI, and DTI findings have implications for the diagnosis and treatment of nerve disease.

Distinct functional types of macrophage in dorsal root ganglia and spinal nerves proximal to sciatic and spinal nerve transections in the rat.

Inflammation proximal to a peripheral nerve injury may be responsible for ectopic discharge and/or death of sensory neurones, factors thought to contribute to the development and/or maintenance of neuropathic pain. Here, ED1+, ED2+ and major histocompatibility complex class II (MHC II)+ macrophages in dorsal root ganglia (DRGs) and spinal nerve roots have been compared quantitatively in adult rats following transection of one sciatic or one spinal nerve, using double labelling immunohistochemistry. In control DRGs, all ED2+ cells expressed ED1 and some also MHC II. One week after either lesion

MRI parameters predict central lumbar spinal stenosis combined with redundant nerve roots: a prospective MRI study.

BACKGROUND: To observe changes in the cauda equina nerve on lumbar MRI in patients with central lumbar spinal stenosis (LSS). METHODS: 878 patients diagnosed with LSS by clinical and MRI were divided into the redundant group (204 patients) and the nonredundant group (674 patients) according to the presence or absence of redundant nerve roots (RNRs). The anteroposterior diameter of the spinal canal (APDS) and the presence of multiple level stenosis, disc herniation, thickening of ligamentum flavum (LF) and increased epidural fat were assessed on MRI. Univariate and multivariate logistic regres

A Meta-Analysis on the Clinical Significance of Redundant Nerve Roots in Symptomatic Lumbar Spinal Stenosis.

OBJECTIVE: Symptomatic lumbar spinal stenosis (LSS), which leads to severe socioeconomic consequences and places a heavy burden on global healthcare system, is a relatively frequent spine disorder. Redundant nerve roots (RNRs) are a relatively common finding in which slender, serpiginous, or tortuous nerve roots are found in the subarachnoid space of the lumbar spine. Previous studies that evaluated the prognostic assessment of RNRs in patients with symptomatic LSS are composed of doubtful results. Therefore, the clinical significance of RNRs in symptomatic LSS is still uncertain. The aim of t

An autopsy case of meningeal carcinomatosis with parenchymal invasion through the cranial and spinal nerve roots.

Meningeal carcinomatosis is a well-known complication of malignant neoplasms. We report a case of meningeal carcinomatosis of 2 months' duration in a 22-year-old man, in whom the initial symptom was gradually worsening headache. Postmortem examination revealed infiltrating adenocarcinoma of the stomach. Carcinoma cells showed diffuse spread to the subarachnoid space of the brain and spinal cord. In many places, subarachnoid tumor cells had infiltrated to the cranial and spinal nerves. Moreover, carcinoma cells in the nerve roots extended to the parenchyma of the brain and spinal cord beyond th

Cauda equina redundant nerve roots are associated to the degree of spinal stenosis and to spondylolisthesis.

UNLABELLED: To evaluate the association of redundant nerve roots of cauda equina (RNRCE) with the degree of lumbar spinal stenosis (LSS) and with spondylolisthesis. METHOD: After Institutional Board approval, 171 consecutive patients were retrospectively enrolled, 105 LSS patients and 66 patients without stenosis. The dural sac cross-sectional area (CSA) was measured on T2w axial MRI at the level of L2-3, L3-4 and L4-5 intervertebral discs. Two blinded radiologists classified cases as exhibiting or not RNRCE in MRI. Intra- and inter-observer reproducibility was assessed. RESULTS: RNRCE were

Control of leg-powered paraplegic cycling using stimulation of the lumbo-sacral anterior spinal nerve roots.

We investigated leg-powered cycling in a recumbent tricycle for a paraplegic using functional electrical stimulation (FES) with the lumbo-sacral anterior root stimulator implant (LARSI). A female complete T9 paraplegic had a stimulator for the anterior L2 to S2 spinal roots (bilaterally) implanted in 1994. She was provided with equipment for daily FES cycling exercise at home. The cycling controller applies a pattern of stimulation in each of 16 crank angle phases. A 7-bit shaft encoder measures the crank angle with adequate precision. Each pattern was originally chosen to give the greatest pr

Anatomical bases of the posterior approach to the brachial plexus for repairing avulsed spinal nerve roots.

Avulsion of nerve roots from the cervical spinal cord has always been considered as an untreatable injury, even by surgeons with expertise in this area. However, numerous experimental studies in animals, as well as a human case report, showed that if continuity is restored between the spinal cord and nerve roots, axons from spinal motor neurons can regrow into the peripheral nerve graft with a subsequent recovery of motor function. The posterior subscapular approach, based on the evolution of the posterolateral approach for removal of the first rib, is the only way to expose the entire brachia

Stimulation of the Spinal Cord and Dorsal Nerve Roots for Chronic Groin, Pelvic, and Abdominal Pain.

BACKGROUND: Chronic neuropathic groin pain is a common problem. It can arise following surgery or trauma, or spontaneously as part of various pelvic pain syndromes. A number of different stimulation techniques have been reported in the literature to treat this area, but due to the complex anatomy of the region, it can be difficult to target effectively with paresthesias. OBJECTIVES: In this study we report our results treating patients with chronic neuropathic groin, pelvic, and abdominal pain, using spinal cord stimulation and dorsal nerve root stimulation. STUDY DESIGN: Open label, prospec

Similar myelographic patterns of different origins (spinal angioma and redundant nerve roots of the cauda equina).

A case of redundant nerve roots of the cauda equina is presented. Its radiological findings are compared to a case of spinal angioma of the thoraco-lumbar region. The changing myelographic patter in later pictures of redundant nerve roots helps to differentiate this condition from spinal angioma.

Nerve root enhancement on spinal MRI in pediatric Guillain-Barré syndrome.

Guillain-Barré syndrome diagnosis is based on clinical presentation and supportive diagnostic testing. In its early stage, no single, reliable diagnostic test is available. However, a finding of nerve root enhancement on spinal magnetic resonance imaging may be useful. We evaluated the frequency of nerve root enhancement on spinal magnetic resonance imaging in children with Guillain-Barré syndrome. At a single tertiary pediatric center, we conducted a retrospective chart review of children with Guillain-Barré syndrome who had complete spinal or lumbosacral spinal magnetic resonance imaging wit

Spinal cord tethering by aberrant nerve root in myelomeningocele: case report.

INTRODUCTION: A 2,796-gram male baby was born at 40 weeks gestation by vaginal delivery. Soon after, he was admitted at our department for treatment of a dorsal appendage. RESULTS AND DISCUSSION: Magnetic resonance (MR) imaging showed a low-position spinal cord, skin-covered myelomeningocele, syringomyelia, and a tethered cord. Resection of the caudal appendage and tethering tissue was performed 22 days after birth under electrophysiological monitoring. Histopathological examination of the caudal appendage that was covered with skin revealed the presence of spinal cord tissue and peripheral n

The role of diagnostic selective nerve root blocks in the management of spinal pain.

Biochemical mechanisms to explain pain generation began relatively recently. Evaluating pain originating from the spine can be challenging because no historical or physical examination findings are sufficiently sensitive or specific for identifying each of the myriad of potential pain generators. These discrete anatomic structures include the nerve root, disc annulus, posterior longitudinal ligament, sacroiliac joint, and facet joint. The diagnostic selective nerve root block is a useful test to determine the etiology of pain when other testing is inconclusive. If a patient with radicular symp

Spinal cord and nerve root monitoring in spine surgery and related procedures.

Intensive research in the field of intraoperative neurophysiologic monitoring has been performed directed at finding reliable stimulating and recording techniques and adequate anesthetic regimes applicable to spinal procedures. The aim is a comprehensive monitoring not only of afferent and efferent spinal cord pathways but also of sensory and motor nerve roots and cauda equina fibers. Conventional somatosensory evoked potentials (SEPs) are complemented by motor evoked potentials, dermatomal sensory evoked potentials, spinal cord evoked potentials, evoked electromyography, sensory and motor fib
