[Isotopic kidney investigations in staghorn lithiasis in the adult (author's transl)].

The authors report a study of 37 kidneys in 30 patients with staghorn calculi undergoing isotopic investigation of renal function by sequential scintigraphy and isotopic nephrogram by gamma-camera. 7 of the patients had bilateral staghorn calculi. Tracers used: Hippuran labelled with I131 and DPTA labelled with Technetium 99 m. 62% of kidneys poorly tolerated their staghorn calculus as shown by severe impairment of the isotopic nephrogram involving the uptake phase. Amongst the 20 cases of unilateral lithiasis, a nephrectomy was performed whenever the functional value was less than 20% normal

[Extracorporeal lithotripsy in children].

14 patients of pediatric age with renal and ureteral calculi were treated with extracorporeal shock wave lithotripsy (Piezolith 2300). Three of the 14 patients had bilateral renal calculi. There were nine male and five female patients with an average age of 9.7 years. Most of the calculi were the renal pelvis. four calculi were smaller than 10 mm, 12 were between 10 and 20 mm and one was a partial staghorn calculi. A double "J" stent way placed in two patients, one with an ureterostomy and another with a partial staghorn calculi. The average number of shock waves received for patient was 4275

[Extracorporeal shock wave treatment of urinary calculi. Results after treatment of the first 207 patients at the Copenhagen City Stone Center with a second generation lithotriptor].

Early experience of extracorporeal shock wave lithotripsy (ESWL) using a second generation lithotriptor (Siemens Lithostar) is reported. Two hundred and seven patients underwent 272 treatments for 291 stones. There were 259 renal calculi, including three staghorn calculi and 32 ureteric calculi. Treatments were performed under local analgesia (78%) or epidural or general anaesthesia (22%) when invasive procedures had to be done in connection with the treatment. Stone fragmentation was achieved with 2,487 +/- 1,262 shocks. The first month stone clearance rate was 43%, 24% had fragments less tha

Sarcomatoid Squamous Cell Carcinoma of the Renal Pelvis Masquerading as Emphysematous Pyelonephritis with Staghorn Calculus.

BACKGROUND: Staghorn calculi are well-established risk factors for recurrent urinary tract infections (UTIs) and subsequent renal deterioration. Less commonly, long-term urothelial irritation from a calculus may also pose a risk of malignant transformation. CASE PRESENTATION: A 77-year-old male with multiple medical comorbidities presented with a chronic right renal pelvic staghorn calculus and findings concerning for emphysematous pyelonephritis. He was subsequently taken to the operating room for a planned laparoscopic right nephrectomy. Final pathology analysis revealed sarcomatoid squamou

Squamous Cell Carcinoma of the Renal Pelvis, A Rare Site for a Commonly Known Malignancy.

Chronic nephrolithiasis predisposes to squamous metaplasia and subsequently SCC which is a rare malignancy of the upper urinary tract. A 60-year-old woman with a long standing history of renal calculi presented with flank pain and fever. Investigations revealed a mass in the superior pole of a non functioning left kidney while the right kidney was sub optimally functioning, hydronephrotic and there was presence of bilateral staghorn calculi. Patient underwent decompression of right kidney by double j stenting and left radical nephrectomy that revealed well differentiated squamous cell carcinom

The fragmented staghorn calculus: a radiological sign of pyonephrosis.

Staghorn calculi may cause several complications, particularly important being the development of pyonephrosis due to long-standing infection. We describe four patients who presented with either a marked systemic illness or a loin mass due to pyonephrosis. In each patient, the plain abdominal radiograph showed a fragmented staghorn calculus, which is suggested as a radiological sign of pyonephrosis and indicative of the need for surgical attention.

[Extracorporeal lithotripsy in the treatment of renal lithiasis. 5 years' experience].

The historical background to extracorporeal lithotripsy using shock waves is described and indications for use of this treatment discussed in relation to other types of therapy for reno-ureteral lithiasis: percutaneous or trans-ureteral endoscopy. The reduction in invasive surgical procedures is emphasized. The first human use of extracorporeal lithotripsy by shock waves dates back to 1980. Since then, more than 30,000 calculi have been treated in this way, either exclusively or in combination with other therapy. For simple small calculi (less than 1 cm in diameter and situated in the pelvis o

Percutaneous nephroscopic with holmium laser and ultrasound lithotripsy for complicated renal calculi.

The aim of this work is to validate the clinical efficacy of the high-power holmium:YAG laser with percutaneous nephrolithotripsy (PCNL) in combination with ultrasound lithotripsy for complicated renal calculi. From November 2006 to December 2007, 60 patients with complicated renal calculi were treated with PCNL, where an F24 standard renal access tract was established by percutaneous renal puncture under the guidance of B-mode ultrasound, and stones were fragmented and cleared by high-power holmium laser in combination with ultrasound under an F20.8 nephroscope. Of the 60 patients with compli

[Two cases of replacement lipomatosis of the kidney].

Two cases of replacement lipomatosis of the kidney are reported. One patient was a 64-year-old woman who presented with pain caused by a subcutaneous abscess in the back as the chief complaint. Replacement lipomatosis of the kidney associated with staghorn calculi was diagnosed by computerized tomography (CT) and magnetic resonance imaging (MRI), and nephrectomy was performed. The other patient was a 71-year-old woman whose chief complaint was repeated episodes of pyelonephritis. Staghorn calculi and severe renal dysfunction were diagnosed, and nephrectomy was performed. In both patients, path

Adenosquamous carcinoma of the renal pelvis.

A case of adenosquamous carcinoma of the renal pelvis is reported, associated with multiple staghorn calculi, occurring in a 35-year-old woman. A review of the literature revealed only two previously reported cases. It is postulated that chronic irritation from the renal calculi had induced squamous and glandular metaplasia with subsequent adenosquamous neoplasia.

Ultrasonic differentiation of renal pelvic filling defects.

We herein describe 9 patients with non-opaque calculi and 3 patients with soft tissue filling defects in the renal pelvis studied by ultrasound. The calculi varied in size from 1.5 cm. to staghorn calculi. All stones were imaged satisfactorily by ultrasound, allowing the diagnosis of stone to be made with confidence. Ultrasonic differentiation of renal pelvic filling defects proved to be a simple, reliable, non-invasive method to differentiate these clinically perplexing lesions.

What Factors Drive Staghorn vs Nonstaghorn Pattern Growth in Patients with Metabolic Stones?

Purpose: There has been an increase in the number of staghorn calculi that form in the absence of infection (metabolic staghorns). It is unknown why some large metabolic calculi form as solitary or multiple separate, nonbranching caliceal stones, whereas others develop as staghorn stones forming a cast of the collecting system. We sought to compare these two groups of metabolic stone formers (SFs) in an attempt to shed light on these disparate stone-forming phenomena. Materials and Methods: From January 2017 to September 2018, 190 patients underwent percutaneous nephrolithotomy for stones >2 c

Percutaneous nephrostomy in conjunction with ESWL in treatment of nephrolithiasis.

The use of percutaneous nephrostomies (PCNs) in 1456 patients (1660 kidneys) treated with extracorporeal shock-wave lithotripsy (ESWL) was evaluated. In this group, 138 PCNs (130 patients) were performed in 133 kidneys. Forty-seven percent of PCNs were placed in patients with staghorn calculi; 24% were for stones in the renal pelvis, and 20% for ureteral stones. The most common indication for PCN was fever and obstruction (57%). In 15%, the indications were failure to decompress an obstructed system from a retrograde direction, clogged double-J ureteral stents, and perforation of the ureter. P
