Minimally invasive surgical versus catheter ablation for the long-lasting persistent atrial fibrillation.

OBJECTIVE: To assess the efficacy of video-assisted minimally invasive surgical vs. catheter ablation for the long-standing persistent AF. METHODS: We performed a retrospective comparative analysis in a series of 166 long-standing persistent AF patients treated between 2006 and 2009 with either video-assisted minimally invasive ablation (83 patients), or catheter ablations (83 patients). The catheter group was screened using a 'pair-matched case-control' methodology in order to select appropriate statistical comparison candidates out of 169 long-standing persistent AF patients which were pote

Immersion Phase Separation 3-Dimensional Printing for Strain-Stiffening Hydrogel Scaffolds.

Strain-stiffening hydrogels, which mimic the nonlinear mechanical behavior of biological tissues such as skin, arteries, and cartilage, hold transformative potential for biomedical applications. This study introduces immersion phase separation (IPS) 3-dimensional (3D) printing, an innovative technique that enables the one-step fabrication of strain-stiffening hydrogel scaffolds with intricate, hierarchical architectures. This technique addresses the long-standing challenge of balancing structural complexity and intrinsic mechanoresponsive behavior in traditional hydrogel fabrication methods. B

Intervention to Increase Uptake of Recommended Vaccines in an Inner-City HIV Care Clinic.

This study investigates the effectiveness of a targeted intervention aimed at improving vaccination rates among People Living with HIV (PLWH) at the University of Miami/Jackson Memorial Hospital's Adult HIV Outpatient Clinic, a key provider for a predominantly minority population in Miami-Dade County, Florida. Despite the established efficacy of vaccines and their endorsement by various health agencies, vaccination rates among PLWH remain suboptimal, particularly in minority groups. Our intervention, structured around the CERPS framework (Champion, Education, Reminder, Performance feedback, an

Ultraprecise Antigen 10-in-1 Pool Testing by Multiantibodies Transistor Assay.

Effective screening of infectious diseases requires a fast, cheap, and population-scale testing. Antigen pool testing can increase the test rate and shorten the screening time, thus being a valuable approach for epidemic prevention and control. However, the overall percent agreement (OPA) with polymerase chain reaction (PCR) is one-half to three-quarters, hampering it from being a comprehensive method, especially pool testing, beyond the gold-standard PCR. Here, a multiantibodies transistor assay is developed for sensitive and highly precise antigen pool testing. The multiantibodies capture SA

ACOG Committee Opinion No. 772: Immunization Implementation Strategies for Obstetrician-Gynecologists.

Immunization against vaccine-preventable diseases is an essential component of women's primary and preventive health care. Many studies have shown that a recommendation from an obstetrician-gynecologist or other health care provider for a vaccine is one of the strongest influences on patient acceptance. Obstetrician-gynecologists and other health care providers should develop a standard process for assessing and documenting the vaccination status of patients and for recommending and administering vaccines. If allowed by state law, obstetrician-gynecologists and other health care providers are

Understanding the Reactive Adsorption of H2 S and CO2 in Sodium-Exchanged Zeolites.

Purifying sour natural gas streams containing hydrogen sulfide and carbon dioxide has been a long-standing environmental and economic challenge. In the presence of cation-exchanged zeolites, these two acid gases can react to form carbonyl sulfide and water (H2 S+CO2 ⇌H2 O+COS), but this reaction is rarely accounted for. In this work, we carry out reactive first-principles Monte Carlo (RxFPMC) simulations for mixtures of H2 S and CO2 in all-silica and Na-exchanged forms of zeolite beta to understand the governing principles driving the enhanced conversion. The RxFPMC simulations show that the p

Pelvic floor muscle function in the standing position in women with pelvic floor dysfunction.

INTRODUCTION AND HYPOTHESIS: Clinical assessment of the pelvic floor muscles (PFMs) in a standing position may provide a more valid representation of PFM function experienced by patients in daily life than assessment in the lying position. The primary aim of this study was to examine PFM function in a standing versus a lying position in parous women with any type of urinary incontinence and/or pelvic organ prolapse. METHODS: In this exploratory cross-sectional study, participant symptom status was determined using the Australian Pelvic Floor Questionnaire. Pelvic floor muscle function was ass

Feasibility study: Towards Estimation of Fatigue Level in Robot-Assisted Exercise for Cardiac Rehabilitation.

Socially Assistive Robotics (SAR) has shown to be an important tool to assist patients in physical rehabilitation. SAR is used to provide feedback about patient's state and performance to users and health professionals, therefore, patients are monitored by means of sensor interfaces. In this context, aiming to avoid over-training conditions, one of the most important parameter to monitor is the fatigue level. However, it is usually measured by subjective scales such as Borg scale, thus, there is a need to develop systems that are able to estimate fatigue with greater accuracy. It has been demo

Electrically induced and voluntary activation of physiologic muscle pump: a comparison between spinal cord-injured and able-bodied individuals.

OBJECTIVE: To evaluate the central haemodynamic responses during position changes from supine to sitting and during 30 min of standing between able-bodied and spinal cord-injured subjects. Also to assess the effects of the physiologic muscle pump in both groups during 30 min of standing. DESIGN: A repeated measure design. Both groups were tested on two different days under two conditions of 30 min of stationary standing and 30 min of dynamic standing (voluntary activation of the lower leg muscles in able-bodied and FES-induced activation of these muscles in spinal cord injured). The order of

A practical guide to active stand testing and analysis using continuous beat-to-beat non-invasive blood pressure monitoring.

PURPOSE: The average adult stands approximately 50-60 times per day. Cardiovascular responses evoked during the first 3 min of active standing provide a simple means to clinically assess short-term neural and cardiovascular function across the lifespan. Clinically, this response is used to identify the haemodynamic correlates of patient symptoms and attributable causes of (pre-)syncope, and to detect autonomic dysfunction, variants of orthostatic hypotension, postural orthostatic tachycardia syndrome and orthostatic hypertension. METHODS: This paper provides a set of experience/expertise-base

The World Health Organization and the Shifting US and Global Political Orders.

The withdrawal of the United States from the World Health Organization (WHO) raises crucial questions about its future as the governing international organization for health. The executive order on withdrawal was one of President Donald Trump's first acts in his second term. Because the United States is WHO's biggest funder and most powerful state backer, withdrawal could indicate an existential threat. However, almost simultaneously member states passed a new international Pandemic Agreement expanding WHO's authority. How should these conflicting signals be understood? Analyzing WHO's decline

Evanescent-wave interference lithography enabled by guided-mode-resonance field transfer.

We propose a guided-mode-resonance (GMR) approach to evanescent-wave interference lithography that overcomes the severe depth of field and proximity limitations of conventional near-field methods. The scheme consists of a resonant dielectric grating and a photoresist layer separated by a low-index buffer medium, where phase-matched evanescent coupling transfers resonantly enhanced standing-wave fields from the grating to the photoresist. Using rigorous coupled-wave analysis, we design a UV-compatible structure comprising HfO2 grating ridges, a commercial photoresist layer, and a transparent im

Incorporating π-Conjugated Amino Acids Into Multi-Functional Amorphous Materials.

The controlled fabrication of amorphous materials from small organic molecules and their translation into high-performance functional materials remain long-standing challenges. Here, we introduce a multicomponent assembly strategy to developed amorphous materials from π-conjugated amino acids, offering a versatile platform for high-performance adhesive and optical applications. The incorporation of proline-based building blocks into multicomponent networks bypasses the crystallization-induced self-assembly that otherwise dominates in solution, thereby driving liquid-liquid phase separation-lik

Update on Routine Immunizations for Children and Adolescents.

Childhood and adolescent immunizations compose a valuable public health tool to prevent infection, morbidity, and mortality. The American Academy of Family Physicians recommends that patients receive all recommended immunizations from their usual source of primary care and does not support nonmedical immunization exemptions. Maintaining high vaccination coverage is crucial for preventing outbreaks of vaccine-preventable diseases. Less than 70% of US children have received the full series of recommended vaccinations at 24 months of age. Using electronic health record reminders and creating stan

One-Dimensional Channel with Side Pockets for Rapid Diffusion and Efficient C2H2 Purification.

Efficiently separating acetylene (C2H2) from carbon dioxide (CO2) has been a long-standing challenge due to their similar molecular dimensions and physical properties. Herein, we report a series of isostructural pillar-layered metal-organic frameworks (MOFs), named JNU-13, JNU-13-CH3, and JNU-13-(CH3)2. These MOFs feature one-dimensional (1D) channels integrated with orthogonally arrayed side pockets, enabling dense packing of C2H2 molecules while maintaining rapid diffusion pathways. Specifically, JNU-13-CH3 achieves a high C2H2 packing density of 360 g L-1, a low adsorption enthalpy of 31.61

Effects of dual tasking on postural and gait performances in children with cerebral palsy and healthy children.

Simultaneous execution of motor and cognitive tasks is embedded in the daily life of children. 53 children of 7-12 years and 22 adults (study 1), 20 healthy children and 20 children of 7-12 years with cerebral palsy (study 2) performed a Stroop-animal task simultaneously with a standing or a walking task in order to determine the attentional demand of postural control and locomotion. Dual-task cost decreased with advancing age in healthy children during balance. CP and healthy children were similarly affected by dual-task constraints during standing and walking. Children with diplegia were mor

Underestimation of varus angulation in knees with flexion deformity.

In order to investigate the change in limb alignment in cases of knee flexion deformity, 17 male volunteers aged from 25 to 35 years with no complaints and no knee disorders were selected. The femorotibial angle (FTA), which is the lateral angle at the intersection between the femoral axis and the tibial axis in an anteroposterior radiograph, was employed as an index of limb alignment. FTA in standing on one leg with and without knee flexion was measured. The mean standing FTA was 179.4+/-19 degrees in full extension, which decreased to 176.9+/-2.5 degrees in a mean flexion position of 18+/-3.

Avoiding the Personhood Issue: Abortion, Identity, and Marquis's 'Future-Like-Ours' Argument.

One reason for the persistent appeal of Don Marquis' 'future like ours' argument (FLO) is that it seems to offer a way to approach the debate about the morality of abortion while sidestepping the difficult task of establishing whether the fetus is a person. This essay argues that in order to satisfactorily address both of the chief objections to FLO - the 'identity objection' and the 'contraception objection' - Marquis must take a controversial stand on what is most essential to being the kind of entity that an adult human being is. Such a stand amounts to a controversial account of personhood
