Fracture of popliteal artery stents.

In peripheral arterial obstructive disease, more than 50% of all lesions are localized in the femoropopliteal segment and surgical revascularization is the treatment of choice. Percutaneous transluminal angioplasty (PTA) is recommended for short lesions, with subsequent stent implantation if the result is sub-optimal or dissections occur after PTA or for restenosis. There are both acute and late complications with stent implantation. In the present patient, stents were placed in the left popliteal artery where the left knee joint flexes, and obstruction because of stent fracture occurred 6 mon

Late complications of ureteral stents.

OBJECTIVE: To review morbidity and late complications of ureteral stent insertion and to specifically evaluate hydronephrosis as a radiologic finding of obstruction in the presence of an indwelling ureteral stent. METHODS: In this prospective study, we evaluated 110 stented kidneys in a group of 90 patients. Of 110 stents, 52 were left in place for 3 months, 23 for 6 months, 11 for 9 months, and 24 for up to 12 months. With the stent in place, patients were followed by plain abdominal X-ray 1 and 30 days after stenting. Further follow-up was performed through ultrasound and plain film every 3

Colonic stents in colorectal obstruction.

Obstruction is the presenting symptom of colorectal cancer in up to 40 per cent of patients. Benign strictures and other neoplasms including lymphoma and gynecologic tumors occur as well. Emergent operative therapy is often suboptimal and associated with significant morbidity and mortality. Our objective was to review our experience with stent placement for colonic obstruction. Seven patients underwent stent placement for a total of eight procedures. There were three patients with unresectable colorectal cancer, two patients with metastatic gynecologic cancer, one patient with rectal lymphoma,

Stents in acute myocardial infarction.

It has been widely reported throughout studies comparing mechanical reperfusion by primary percutaneous transluminal coronary angioplasty (PTCA) with thrombolytic therapy in acute myocardial infarction (AMI) that PTCA results in reduced rates of in-hospital mortality, reinfarction, recurrent ischemia, and stroke, allowing earlier hospital discharge with similar total costs. The attraction of primary PTCA is its relative simplicity and predictability with operators who have a wide range of experience with PTCA. With these results, it is legitimate to wonder what, if any, possible advantages oth

Securing silicone stents in dacryocystorhinostomy.

Silicone stents are commonly used in dacryocystorhinostomy to help maintain a patent mucosal passageway into the nose. It is not unusual for a tube to become displaced and migrate laterally (stent prolapse), and, at times, it may be difficult to reposition it. We describe how stent prolapse may be prevented by using aneurysm clips to secure the silicone stents once they are in place.

Evaluation of stents following septoplasty.

This prospective study was designed to evaluate the immediate postoperative course and long-term efficacy of septal stents. In 20 patients who underwent primary septorhinoplasty, Doyle II silicone stents were inserted bilaterally parallel to the septum. The stents were removed after an average of 3.3 days. A hollow tube was designed into this particular stent in an attempt to minimize patients discomfort from nasal airway obstruction. Subjective ratings of airway improvement were very similar for patients whose septums were supported with nasal stents and for those patients from our 1989 study

Safety and efficacy of everolimus-eluting stents versus paclitaxel-eluting stents in a diabetic population.

OBJECTIVES: This study aimed to analyze the use of everolimus-eluting stents (EES) and paclitaxel-eluting stents (PES) in an unrestricted diabetic population and to compare the performance of these two drug-eluting stents. BACKGROUND: EES have demonstrated superiority in efficacy when compared to PES in a general population. However, it is controversial whether this superiority holds true in a diabetic population. METHODS: From March 2004 to May 2010, 968 patients with consecutive diabetes who underwent percutaneous coronary intervention and implantation of an EES (n = 388) or PES (n = 580)

Drug-eluting stents versus bare-metal stents for treatment of bare-metal in-stent restenosis.

OBJECTIVES: We compared the long-term outcomes of drug-eluting stents (DES) versus bare-metal stents (BMS) for treatment of bare-metal in-stent restenosis (ISR). BACKGROUND: There are no randomized trials or observational studies directly comparing the safety and efficacy of DES versus BMS for treatment of bare-metal ISR. METHODS: We examined data on all patients who underwent percutaneous coronary intervention (PCI) for ISR at Cleveland Clinic between 05/1999 and 06/2007. We compared the efficacy and safety of DES to BMS for treating bare-metal ISR. The primary end point was a composite of

Symptoms related to ureteral stents in renal transplants compared with stents placed for other indications.

OBJECTIVE: To use a validated questionnaire to compare indwelling ureteral stent symptoms in patients who had the ureteral stent placed during renal transplantation with patients who had retrograde insertion of the stent in a native kidney during endoscopic procedures. METHODS: Symptoms related to the presence of a ureteral stent were assessed prospectively in the two patient groups. Just before stent removal, patients completed a validated Ureteral Stent Symptom Questionnaire evaluating urinary symptoms, body pain, general health, work performance, and sexual function. Categorical scores and

Role of stents, drug-eluting stents, and stent-grafts in treatment of infrainguinal arterial disease.

Infrainguinal peripheral arterial disease is increasingly treatable by endovascular techniques. Bare-metal, drug-eluting, and covered stents (stent-grafts) are increasingly important adjuncts to percutaneous transluminal angioplasty in the treatment of occlusive and aneurysmal disease. Preliminary data suggest bioabsorbable stents may also have a significant clinical impact as well. In this article, we describe the available evidence supporting use of stents in the femoropopliteal and tibial arteries, as well as some of their limitations and complications.

Sirolimus-eluting stents versus standard stents in patients with stenosis in a native coronary artery.

BACKGROUND: Preliminary reports of studies involving simple coronary lesions indicate that a sirolimus-eluting stent significantly reduces the risk of restenosis after percutaneous coronary revascularization. METHODS: We conducted a randomized, double-blind trial comparing a sirolimus-eluting stent with a standard stent in 1058 patients at 53 centers in the United States who had a newly diagnosed lesion in a native coronary artery. The coronary disease in these patients was complex because of the frequent presence of diabetes (in 26 percent of patients), the high percentage of patients with l

Open-cell design stents in congenital heart disease: a comparison of IntraStent vs. Palmaz stents.

Slotted stainless Palmaz stents (PS) remain the most commonly used in congenital heart disease (CHD). Limitations of PS include rigidity, foreshortening, poor conformability on expansion, balloon rupture, and jailing of side branches. Recently, stents with open-cell design (IntraStent; IS) in appropriate sizes for CHD have been introduced. We reviewed our experience with the IS, comparing performance with the PS in a retrospective nonrandomized, uncontrolled observational study. Between May 1999 and March 2001, 34 IS (10-36 mm) and 34 PS (10-40 mm) were implanted in 57 patients (3 months to 25

Covered Stents for Coronary Applications.

Covered stents have been utilized in percutaneous coronary interventions (PCIs) for over 2 decades. They are an essential part of the PCI toolbox, especially for the management of complications. Over the last decade, covered stents have been additionally utilized for various other coronary indications with mixed results. The overall design of covered stents has continued to evolve improving the ease, the deliverability, and removing some of the challenges that were faced before in view of the bulky design. These advances can facilitate further use in the future with potentially better long-ter

Coronary Stents: All Those Trials.

In the nearly 40 years that have elapsed since the first coronary stent implantation in man, there has been a multitude of trials regarding this procedure. Stents were designed to combat two major weaknesses of balloon angioplasty: a high restenosis rate (>30%) and a 2 to 10% rate of abrupt closure. Early trials by some of the pioneers of the field suggested that stents, by scaffolding the artery, were indeed very successful in treating angioplasty-induced flow-limiting dissections. Findings of the BENESTENT-I (Belgian Netherlands stent trial) and STRESS (Stent Restenosis Study) demonstrated t

Angioscopic evaluation after venous stents.

BACKGROUND: Venous stenting is increasingly used to manage femoro-ilio-caval venous outflow obstruction/stenosis due to post-thrombotic syndrome. Although the safety, efficacy, and long-term patency of venous stents have been reported, re-interventions due to stent occlusion and in-stent restenosis (ISR) have also been reported. The mechanism of ISR and the in-stent neointimal growth after venous stenting remains unclear. We performed angioscopy to evaluate intraluminal details after venous stenting, allowing real-time direct visualization of the vessel lumen. METHODS: Ten angioscopic procedu

Nanoglass-based balloon expandable stents.

Here, a prototypical metallic nanoglass is proposed as a new alloy for balloon expandable stents. Traditionally, the stainless steel SS 316L alloy has been used as a preferred material for this application due to its proper combination of mechanical properties, corrosion resistance, and biocompatibility. Recently, metallic glasses (MGs) have been considered as promising materials for biodevice applications. MGs often display outstanding mechanical properties superior to those of conventional metallic alloys and overcome some of the weaknesses of SS 316L, such as radiopacity, stainless steel al
