Sternoclavicular Joint Infections: Improved Outcomes With Myocutaneous Flaps.

Sternoclavicular joint (SCJ) infection is a rare disease and its management remains controversial. Our institution has adopted an aggressive surgical approach of radical SCJ resection combined with myocutaneous flap (MCF) closure whenever possible. We reviewed our experience with this approach in the management of this condition. From July 2004 to June 2018, 50 consecutive patients were treated surgically for SCJ infections. Patient demographics, imaging studies, microbiology, and operative variables were analyzed. All patients underwent ipsilateral SCJ resection. Wound closure was performed w

Osteoradionecrosis of the sternoclavicular joint after laryngopharyngeal radiation.

OBJECTIVES: Adequate treatment of laryngopharyngeal malignancy often incorporates radiation therapy. Structures surrounding laryngopharynx exposed to traditional radiation doses are susceptible to posttreatment toxicity. Among poorly understood sequelae is the rare manifestation of sternoclavicular joint (SCJ) osteoradionecrosis (ORN). METHODS: Three institutional encounters prompted a comprehensive literature search, generating three published case reports. Systematic extraction and analysis (n = 6) of demographics, cancer history, comorbidities, ORN presentation, imaging, and management est

Sternoclavicular joint osteophytosis: a difficult diagnosis to swallow.

Unexplained dysphagia requires prompt investigation to rule out a possible underlying malignancy. We describe the case of a 60-year-old man who presented to his family practitioner with a 1-year history of increasing dysphagia with associated pain over the front of his chest. He was referred on to an ear, nose and throat specialist where no obvious laryngeal pathology was found at direct laryngoscopy, but an 'indentation' of the right anterior larynx, which increased with external pressure on the sternoclavicular joint (SCJ), was noted. A subsequent CT scan of his neck demonstrated osteoarthri

Posterior sternoclavicular joint injuries in skeletally immature patients.

The management of sternoclavicular injuries in skeletally immature patients has not been well described. The purpose of this study was to describe the authors' experience treating this rare and potentially life-threatening injury. All skeletally immature patients who underwent treatment for a medial clavicular physeal fracture or sternoclavicular dislocation between 2003 and 2011 were identified using ICD-9 diagnostic codes. Sternoclavicular injuries with posterior displacement were isolated from this cohort for a thorough chart review. Patients were contacted to complete brief phone surveys a

Sternoclavicular joint osteomyelitis following head and neck surgery.

OBJECTIVES/HYPOTHESIS: To review all reported cases of sternoclavicular joint (SCJ) osteomyelitis following head and neck surgery. STUDY DESIGN: Retrospective case review. METHODS: PubMed search and one additional case from our institution. RESULTS: Twelve cases were reviewed, and the following recommendations are suggested: risk factors for SCJ osteomyelitis should be identified; early and prompt diagnosis of SCJ osteomyelitis is warranted; tracheostoma care and careful examination of the skin should not be neglected; meticulous screening for cancer recurrence is imperative; if SCJ infecti

Biomechanical analysis of reconstructions for sternoclavicular joint instability.

BACKGROUND: A variety of reconstructive methods have been described for the treatment of sternoclavicular joint instability, yet none have been analyzed in the laboratory, to our knowledge. The purpose of the present study was to evaluate three different reconstruction techniques with use of a cadaveric model: (1) intramedullary ligament reconstruction, (2) subclavius tendon reconstruction, and (3) reconstruction with use of a semitendinosus graft placed in a figure-of-eight fashion through drill-holes in the clavicle and manubrium. METHODS: Thirty-six fresh cadaveric specimens were mounted s

Clinical features of septic arthritis of sternoclavicular joint.

We studied 21 patients with septic arthritis of the sternoclavicular joint at Chulalongkorn University Hospital between January 1987 and January 1997. There were 15 males (71.4%) and 6 females (28.6%). The mean age was 47.4 years with a range of 16 to 69. More than half of the patients (57.1%) were aged more than 50 years and most had associated diseases including diabetes mellitus and cirrhosis. Almost all of the younger age group had a history of intravenous drug abuse. All of the patients had fever and sternoclavicular joint pain. Most of the patients (66.7%) had monoarticular arthritis, wh

Synovial osteochondromatosis of the sternoclavicular joint in early adolescence.

Synovial osteochondromatosis is a rare benign disorder characterised by cartilaginous metaplasia of the synovium, most often affecting large joints. We describe an unusual case in the sternoclavicular joint of an adolescent, who presented with a localised swelling. Imaging demonstrated multiple calcified loose bodies, and the diagnosis was confirmed histologically following surgical excision. Management consisted of removal of the loose bodies with consideration of synovectomy to reduce recurrence. Early recognition is crucial to prevent progressive joint damage.

Sternoclavicular joint arthritis as the initial presentation of sarcoidosis.

Sarcoidosis is a multisystem granulomatous disease of unknown etiology. It can affect the musculoskeletal system in the form of acute and rarely, in the form of chronic peripheral arthritis involving bilateral small and large joints. Axial joint involvement with sarcoidosis is rare. Herein we report a case with sternoclavicular joint arthritis as the initial presentation of sarcoidosis. This report aims to demonstrate a rare initial presentation of sarcoidosis and highlight the importance of considering sarcoidosis as a differential diagnosis in patients presenting with sternoclavicular joint

Tuberculosis of the Sternoclavicular Joint: Report of Three cases.

Ebstein's anomaly accounts for 0.3% to 0.7% of all cases of congenital heart disease. The condition is characterized by abnormal tricuspid valve and right ventricle with apical displacement of tricuspid valve leading to atrialization of right ventricle. There have been case reports of patients surviving up to ninth decade. It is unusual for these patients to be asymptomatic in adulthood for long duration. We describe a patient with Ebstein's anomaly in the ninth decade with coronary artery disease.

The prevalence of osteoarthritis of the sternoclavicular joint on computed tomography.

BACKGROUND: Symptomatic disorders around the sternoclavicular joint (SCJ) are relatively uncommon. Previous cadaveric and radiographic studies have suggested that asymptomatic osteoarthritic changes are relatively common, progressively increasing with age. The purpose of this study was to determine the prevalence of SCJ osteoarthritis in the general population using computed tomography (CT) scans. METHODS: We assessed 464 SCJs in 232 patients undergoing a standardized axial CT scan of the thorax including both SCJs, across a range of ages from the second to tenth decade. The scans were undert

[Treatment Strategies for Septic Arthritis of the Sternoclavicular Joint].

Septic arthritis of the sternoclavicular joint (SCJ) is a relatively rare disease. Due to serious complications including mediastinitis and generalised sepsis early diagnosis and rapid onset of treatment are mandatory. The disease often affects immunocompromised patients, diabetics, or patients with other infectious diseases. The therapeutic options range from administration of antibiotics to extended surgery including reconstructive procedures. Apart from rare situations where conservative treatment with antibiotics is sufficient, joint resection followed by plastic surgical procedures are re

Tuberculosis of the sternoclavicular joint. Report of two cases.

The sternoclavicular joint accounts for only 1 to 2% of all cases of peripheral tuberculous arthritis and is more often infected by pyogenic organisms than by the tubercle bacillus. We report two cases of sternoclavicular joint tuberculosis, in a 38-year-old man and a 46-year-old woman without risk factors for immune deficiency. Swelling of the joint was the presenting manifestation. Laboratory tests indicated inflammation in only one of the patients. The intradermal tuberculin test was strongly positive in both patients, whereas smears and cultures of sputum and urine samples were negative fo

Biomechanics of the sternoclavicular joint: an experimental and computational study.

BACKGROUND: This study aims to thoroughly investigate the biomechanics of the sternoclavicular joint (SCJ), a key component of the shoulder complex. METHODS: Inertial sensors were used on live human subjects to measure joint movements during abduction-adduction, flexion-extension, and shrugging. These motion data were then applied to a musculoskeletal model (OpenSim) to analyze the SCJ's range of motion and joint forces. The model was validated using previously published data involving transcortical pins for motion tracking and results from instrumented shoulder prostheses for kinetic compari

[Treatment concepts for the medial clavicle and the sternoclavicular joint].

Medial clavicle fractures and injuries to the sternoclavicular joint are rare injuries but can have life-threatening consequences. There are no standardized treatment algorithms or guidelines for the diagnostics and treatment. This article provides an overview of the individual topographies as well as the conservative and surgical treatment strategies.Conservative treatment is preferred for medial clavicle fractures. The indications for surgical treatment are variable but this is frequently carried out if there is a fracture displacement > 1 cm or 1 shaft width and high functional demands. In
