Stiff-person syndrome coexisting with critical illness polyneuropathy: A case report.

RATIONALE: Stiff-person syndrome (SPS) is an uncommon neurological disorder with autoimmune features. Here, we report a 60-year-old man with SPS associated with critical illness polyneuropathy (CIP). CIP was diagnosed during an episode of acute respiratory failure secondary to muscular rigidity and spasms, which has rarely been reported in this condition. The overlapping of CIP and SPS complicated the case. PATIENT CONCERNS: A 60-year-old man presented with gradual onset of cramps, stiffness, and rigidity in his lower limbs 1 year before admission, which eventually led to inability to stand a

Clinical Spectrum of Stiff Person Syndrome: A Review of Recent Reports.

BACKGROUND: "Classic" stiff person syndrome (SPS) features stiffness, anti-glutamic acid decarboxylase (anti-GAD) antibodies, and other findings. Anti-GAD antibodies are also detected in some neurological syndromes (such as ataxia) in which stiffness is inconsistently present. Patients with otherwise "classic" SPS may either lack anti-GAD antibodies or be seropositive for others. Hence, SPS cases appear to fall within a clinical spectrum that includes conditions such as progressive encephalomyelitis with rigidity and myoclonus (PERM), which exhibits brainstem and autonomic features. We have co

Immunology of stiff person syndrome and other GAD-associated neurological disorders.

Antibodies against glutamic acid decarboxylase (GAD), the rate-limiting enzyme for the synthesis of GABA, are associated with an array of distinct, mostly autoimmune, neurological conditions. In all associated syndromes, namely stiff person syndrome, cerebellar ataxia, epilepsy, limbic encephalitis or abnormal eye movements, anti-GAD antibodies are detected at high titers and play a fundamental role in diagnosis, but do not correlate with disease severity, diversity of symptomatology or response to therapies. Despite considerable efforts, including in vitro (enzymatic assays) and in vivo (anim

[Anesthetic management of a patient with stiff-person syndrome undergoing thymectomy].

Stiff-person syndrome is an uncommon disease characterized by muscular rigidity and painful spasms in the axial and limb muscles. We report a 58-year-old woman with stiff-person syndrome undergoing thymectomy under general anesthesia. Before surgery, her medications were 25 mg of diazepam, 2 mg of clonazepam, and 15 mg of gabapentin per day. After epidural catheterization for the postoperative analgesia, general anesthesia was induced and maintained with continuous remifentanil infusion and propofol with target controlled infusion. With train-of-four ratio (TOFR) monitoring by stimulating the

[Anesthetic management for thymectomy in a patient with stiff-person syndrome].

Stiff-person syndrome is an autoimmune disease characterized by muscle rigidity accompanied by decreased respiratory function. We report a patient with this syndrome who underwent thymectomy under general anesthesia. A 79-year-old woman complaining of increasing muscle rigidity over the past four months was transferred to this hospital. Marked speech disturbance and dysphagia were observed on admission. The diagnosis of stiff-person syndrome was made based on an increase in serum anti-glutamic acid decarboxylase antibody level and thymoma in the anterior mediastinum. Following alleviation of m

A neuropsychological assessment of phobias in patients with stiff person syndrome.

A neuropsychological assessment was performed in 10 patients with stiff person syndrome (SPS) to determine whether their anxiety and phobic symptoms precede stiffness and spasms or represent a reaction to disability. No neurocognitive dysfunction was noted. Patients perceived fears and anxiety as realistic and caused by SPS rather than due to an inherent phobic neurosis.

Understanding stiff-person syndrome: Epidemiological trends, diagnostic challenges, and treatment advances.

Stiff-person syndrome spectrum disorders (SPSD) comprise a group of rare neuroimmunological conditions characterized by fluctuating muscle stiffness, painful spasms, gait disturbance, and hypersensitivity triggers. Emerging phenotypes, including posterior fossa dysfunction, highlight the heterogeneity of the spectrum. Although historically described as a "one-in-a-million" disorder, recent epidemiologic data suggest that SPSD is more common than previously recognized. Currently, consensus diagnostic criteria do not exist; however, proposed criteria emphasize key clinical findings, high titers

A Pediatric Case of Stiff-Person Syndrome: Presentation and Comparative Analysis.

INTRODUCTION: Stiff-person syndrome (SPS) is a rare autoimmune neurological disorder characterized by progressive muscle rigidity and spasms. While it predominantly affects adults, pediatric cases are exceptionally rare, often leading to delayed diagnosis due to non-specific early clinical manifestations. CASE REPORT: We report the case of an 11-year-old boy with progressive gait abnormalities and rigidity, initially misdiagnosed as an orthopedic disorder. The patient exhibited a forward-leaning posture, absent lumbar lordosis, and significant muscle tightness, leading to a primary surgical i

Stiff-person syndrome with concurrent Graves disease: a rare autoimmune overlap.

Stiff-person syndrome (SPS) is an autoimmune neurologic disorder characterized by progressive rigidity and spasms, often linked to antiglutamic acid decarboxylase 65 antibodies. While SPS has established associations with autoimmune diseases like type 1 diabetes and celiac disease, co-occurrence with hyperthyroidism, particularly Graves disease (GD), is rare. We present the case of a 34-year-old woman with paranoia, hallucinations, and altered mental status. Labs revealed thyrotoxicosis, with positive thyroid-stimulating immunoglobulin and thyrotropin receptor antibodies, confirming GD. Despit

Systematic Review of Immune and Symptomatic Treatments for Stiff-Person Syndrome.

BACKGROUND AND PURPOSE: This systematic review evaluated the effectiveness and safety of immune and symptomatic treatments in patients with stiff-person syndrome (SPS). METHODS: A systematic search of PubMed, Embase, and the Cochrane Library was conducted up to September 30, 2024 in accordance with the PRISMA guidelines. All studies that involve SPS patients treated with symptomatic pharmacotherapy or immunotherapy and where the outcome is reported were included in the initial screening. Case reports were excluded. Two independent reviewers carried out study selection, bias assessment with th

Oculomotor Abnormalities in Anti-Glutamic Acid Decarboxylase-Positive Stiff Person Syndrome.

Background: Antibodies to glutamic acid decarboxylase (anti-GAD) can give rise to stiff person syndrome (SPS), an infrequent autoimmune condition of the central nervous system marked by fluctuating muscular rigidity and stimulus-evoked spasms. Disturbances in eye-movement control are rarely identified yet may provide insight into underlying neural involvement. Methods: Two individuals with anti-GAD-related SPS showing distinctive ocular-motor abnormalities were examined with quantitative videonystagmography, supplemented by representative video documentation. Results: Recordings demonstrated v

Stiff Person Syndrome With Positive Anti-glutamic Acid Decarboxylase (GAD) Autoantibodies.

Stiff person syndrome (SPS) is a progressive autoimmune disorder characterized by muscle rigidity, frequent falls, and spasms, affecting primarily women. Recent advances have linked SPS to specific antibodies, such as anti-glutamic acid decarboxylase (GAD)-65, but effective treatments remain elusive. We report the case of a 53-year-old female who developed chronic lower back pain, tingling paresthesias, and progressive rigidity in the lower limbs. Electromyographic examination revealed muscle spasms and co-contractions, along with severe rigidity and reactive spasms upon touch. Imaging studies

Brachial monomelic amyotrophy as an initial manifestation of stiff person syndrome.

Stiff person syndrome (SPS) is characterized by rigidity of truncal and proximal muscles. The presence of abdominal and paraspinal rigidity is a defining clinical feature of SPS. It is rarely associated with the lower motor neuron (LMN) features. We report a patient with SPS whose initial clinical presentation was that of brachial monomelic amyotrophy (BMA). A 24-year-old gentleman presented with a history of the left upper limb wasting and weakness. In addition, he reported stiffness of the lower limbs and abdomen while walking. On examination, patient had left upper limb monomelic amyotrophy

Stiff-Person Syndrome: Seeing Past Comorbidities to Reach the Correct Diagnosis.

Stiff-person syndrome (SPS) is a rare disorder seen in approximately one in one million people. Although it is rare, the symptoms and findings of a typical case should paint a clear clinical picture for those who are familiar with the disease. The primary findings in SPS include progressive axial muscle rigidity as well as muscle spasms. These symptoms most commonly occur in the setting of antibodies against Glutamic Acid Decarboxylase (GAD), the rate-limiting enzyme in the production of Gamma-Aminobutyric Acid (GABA), which is the primary inhibitory enzyme in the central nervous system. Here,

Defining the Expanding Clinical Spectrum of Pediatric-Onset Stiff Person Syndrome.

BACKGROUND: We aimed to characterize the spectrum of clinical features and examination findings in pediatric-onset stiff person syndrome. METHODS: Medical records were reviewed for all patients treated for stiff person syndrome with symptom onset in childhood at a tertiary medical center between March 2001 and February 2019. RESULTS: Of the 15 individuals who met inclusion criteria, 11 (73%) were female and 13 (87%) were Caucasian. Median age at symptom onset was 14.8 years (range 8.4 to 16.9), and median latency from symptom onset to diagnosis was 6.2 years (range 0.4 to 15.0). Nine individ

[A case of stiff-person syndrome due to secondary adrenal insufficiency].

We report a case of flexion contractures in a patient's legs secondary to postpartum hypopituitarism. A 56-year-old woman presented with a 3-year history of worsening flexion contractures of the hips and knees. On admission, her hips and knees could not be extended, and she had muscle stiffness and tenderness to palpation of the lower extremities. We first suspected stiff-person syndrome or Isaacs' syndrome because of her muscle stiffness. However, multiple hormones did not respond to stimulation tests, and an MRI of the brain showed atrophy of the pituitary gland with an empty sella. A subseq

Stiff person syndrome and other immune-mediated movement disorders - new insights.

PURPOSE OF REVIEW: This review highlights the recent developments in immune-mediated movement disorders and how they reflect on clinical practice and our understanding of the underlying pathophysiological mechanisms. RECENT FINDINGS: The antibody spectrum associated with stiff person syndrome and related disorders (SPSD) has broadened and, apart from the classic glutamic acid decarboxylase (GAD)- and amphiphysin-antibodies, includes now also antibodies against dipeptidyl-peptidase-like protein-6 (DPPX), gamma-aminobutyric acid type A receptor (GABAAR), glycine receptor (GlyR) and glycine tran
