Acute cholecystitis complicating ventriculo-peritoneal shunting: report of a case and review of the literature.

CASE: A 3-year-old boy underwent emergency external ventricular drainage and excision of a fourth ventricle anaplastic ependymoma. A week later, the child was given a ventriculo-peritoneal shunt. Fourteen days after shunting, the child developed a subphrenic abscess and acute cholecystitis that required surgery. RESULTS: A Staphylococcus epidermidis was isolated both from the ventricular catheter and CSF and from the subphrenic abscess and the gallbladder. To our knowledge, this is the first report of cholecystitis evolving as a descending shunt infection. The current literature related with

Case report of parapneumonic effusion and mesothelial subdiaphragmatic cyst: relationship or coincidence?

Parapneumonic pleural effusions are common in patients with pneumonia. When colonized by pathogenic bacteria or other microorganisms, these effusions can progress to empyema. Additionally, empyema formation may result in extension of the infection into the infradiaphragmatic region, further complicating the clinical scenario. Many subphrenic collections are found to be mesothelial cysts, which are congenital in origin. However, data regarding the potential association between mesothelial diaphragmatic cysts and parapneumonic effusions are limited. We herein describe a toddler with pneumonia co

[Percutaneous drainage of abdominal abscesses].

Percutaneous drainage has been widely accepted as the preferred treatment for abdominal abscess. Indications have been clarified recently and the absence of a secure route is the only absolute contraindication. We performed this procedure in 65 patients with abdominal abscess at different locations: liver 27, subphrenic 33, lesser cavity 3 and perirenal 3. Overall success rate was 85%, with 89% for liver and 88% for the subphrenic location. Six patients died of multisystemic failure even though the abscess was properly drained. Four patients were operated on for persistent abscess. A pancreati

Liver abscess caused by Clostridium bifermentans following blunt abdominal trauma.

A case of hepatic abscess and subsequent septicemia caused by Clostridium bifermentans is described. The abscess manifested itself on the third day after blunt trauma to the torso. The patient had nausea, vomiting, fever, evidence of hepatic dysfunction, and subphrenic gas. This case illustrates the association of hepatic abscess and blunt trauma to the torso.

Video-assisted Thoracoscopic Surgery (VATS) drainage of salmonella enteritidis empyema and needle-localization for retrieval of a dropped gallstone.

INTRODUCTION: Spillage of gallstones into the peritoneal cavity (dropped gallstones) is more common in laparoscopic compared with open cholecystectomy. Subsequent infectious complications are often delayed, occurring in 0.1% to 2.9% of cases. We present the first reported case of Salmonella enterica subphrenic abscess and empyema complicating dropped gallstones, treated with video-assisted thoracoscopic surgery (VATS), and surgical retrieval. CASE REPORT: A 67-year-old male with symptomatic cholelithiasis underwent an uncomplicated laparoscopic cholecystectomy. Four years later, he presented

A Novel Method of Intra-Abdominal Cavity Obliteration: A Case Report of a Colonic Seromuscular Flap.

Obliteration of an organized subphrenic abscess with an enteric fistula is a great challenge, especially after hepatectomy, as most of the conventional flaps used to fill the abscess cavity are not feasible. A seromuscular flap is an innovative option for this purpose because of its proximity to the pathology, flexibility in the size and volume of the flap harvested, antibacterial ability of the muscle flap, and preservation of trunk musculature. We illustrate the use of a colonic seromuscular flap for filling such an abscess and show its long-term change. A 66-year-old man developed a right s

The new retained foreign body! Case report and review of the literature on retained foreign bodies in laparoscopic bariatric surgery.

The advent of laparoscopic surgery has created a set of peculiar morbidities. As the laparoscopic devices, also the type of retained foreign bodies has changed. We present a case of unusual, apparently isolated and recurrent lung abscess, pleural effusion and poorly evident subphrenic abscess after laparoscopic gastric bypass, due to a retained Endo-Catch bag. A 27-year-old obese female underwent an uneventful laparoscopic Roux-en-Y gastric bypass. After surgery she developed a left basal lung abscess, that resolved in two weeks with heavy antibiotic therapy, while radiological abdominal imagi

[Echinococcal disease of the liver].

Hydatid disease is rare and sporadically seen in our region. With the use of modern diagnostic methods the diagnose of echinococcus cyst in the liver is easily and exactly made; most frequently we performed cystectomy paying attention not to disseminate the parasite fluid, and treating the cavum of the pericyst as described by PAPADIMITROU. Using the drainage of subphrenic space we prevent postoperative complications. When the subphrenic abscess appears the hospitalisation period is prolonged, the patient's life is threatened and the treatment is getting expensive.

[Septic complications following splenectomy after abdominal trauma].

We reviewed retrospectively the septic postsplenectomy complications after splenic rupture from abdominal trauma in 53 patients treated in the last five years (1984-1988). We recorded a higher incidence of complications (pneumonia, subphrenic abscess) in these patients than in those who underwent clean surgery during the same period, but we did not demonstrate the same difference in other infectious processes (infection of the surgical wound). Infections had a higher incidence in patients with splenic rupture with associated lesions (54.10%) than in those with only a splenic lesion (37.50%) (S

An Unexpected Case of Perforated Acalculous Cholecystitis Caused by a Fish Bone.

Gallbladder inflammation is most often determined by the presence of gallstones. Acalculous cholecystitis usually occurs in patients with multiple comorbidities or with an immunosuppressed status, and therefore its evolution is faster and more severe compared to acute calculous cholecystitis. The presence of a fish bone into the peritoneal cavity, through a gastrointestinal fistula is not very rare, but acute cholecystitis caused by a fish bone is unexpected. Here, we present the case of a 75-year old woman who had eaten fish two months before and presented at the Emergency Room with perforate

[Spontaneous cholecystocutaneous fistula presenting in the right breast].

We report a case of an 89 year-old woman, with an abscess in the right breast. On incision the abscess produced bile. Ultrasound scan, computed tomography, magnetic resonance imaging and endoscopic retrograde cholangiopancreatography showed a fistula from a subphrenic abscess to the breast. Conservative treatment was insufficient and the condition only resolved after open cholecystectomy. During the latest 50 years, less than 20 cases of cholecystocutaneous fistula have been reported. Perforation to the breast has not previously been described.

Nephrobronchial fistula. Complication of perinephric abscess secondary to ureteral obstruction and pyonephrosis.

A case of nephrolithiasis complicated by obstruction leading to pyonephrosis, perinephric abscess, and nephrobronchial fistula is presented. The patient was treated successfully by nephrectomy and drainage of the subphrenic space and the pleural cavity. Nephrobronchial fistula, while a rare complication of perinephric abscess, should be considered when a patient presents with perirenal suppurative process and chest x-ray findings consistent with pleural effusion or pulmonary infiltrates.

From Localization to Navigation: Expanding the Role of Intraoperative Ultrasound in Robotic Surgery.

Recurrent subphrenic abscess from retained radiolucent gallstones is a difficult problem: the stones are often difficult to localize due to its radiolucency, the operative field is confined and distorted due to its inflammation, and dissection lies close to the heart, pleura, and kidney. We report the case of a 77-year-old man with recurrent subphrenic abscesses, in whom these challenges were resolved with a robotic approach using a miniature drop-in ultrasound probe (FUJIFILM L51 K). The probe confirmed that a trans-diaphragmatic abscess and its contained stone lay deeper than the dissection
