The Journal of foot and ankle surgery : official publication of the American College of Foot and Ankle Surgeons2012eng
Many joint-depressive, neglected calcaneal fractures need distraction arthrodesis of the subtalar joint. Because the calcaneal tuberosity is usually malunited in the varus position, more distraction is required on the medial side to place the tuberosity in the everted position before insertion of a bone graft. Traditionally, a distractor is placed on the medial side to achieve this task. However, the medially placed distractor hinders with the positioning of the extremity and exposure of the main operative site, which is commonly on the lateral side. We introduce a technique to avoid this burd

Subtalar joint dislocation is a rare entity, accounting for only 1% of all traumatic dislocations. The authors report a case of an adolescent with gradual lateral subluxation of an anatomically abnormal subtalar joint with no history of trauma, an extremely rare presentation. The patient also had a congenitally short limb. The patient underwent llizarov leg lengthening along with multiple surgeries of the subtalar joint. The choice of the Ilizarov technique to lengthen the leg and realign the subtalar joint in this case is discussed.

The use of subtalar arthrodesis procedures has been widely implemented to relieve hindfoot issues after failure of conservative treatments; however, fusion failures persist in some patients with certain risk factors. Currently, surgeons utilize cannulated screws in these arthrodesis procedures to immobilize the subtalar joint. Recent clinical studies have demonstrated improved fusion outcomes in at-risk patients using sustained dynamic compression devices in the tibiotalocalcaneal complex. These devices utilize pseudoelastic nitinol which enables sustained dynamic compression when faced with p

BACKGROUND: Anatomical knowledge of the tarsal canal and sinus is still unclear owing to the complexity of the ligamentous structures within them, particularly the relationship with the capsules of the subtalar joints. The aim of this study was to examine the anatomical relationship between the fibrous tissues of the tarsal canal and sinus and the articular capsules of the subtalar joint.
METHODS: We conducted a descriptive anatomical study of 21 embalmed cadaveric ankles. For a macroscopic overview of the subtalar joint, we removed the talus in 18 ankles and separated the fibrous tissues fro

Subtalar joint arthrodesis is a procedure used in posttraumatic arthritis, osteoarthritis, tarsal coalition management, posterior tibial tendon dysfunction, and inflammatory arthropathies, and can be used in deformity correction. The goals of the procedure are to eliminate pain, improve function, restore stability, and realign the hind foot. The procedure has high patient satisfaction with low complications while preserving motion in adjacent tarsal joints. Joint preparation is important and time should be spent preparing the joint for successful arthrodesis and the fixation construct needs to

Osteochondritis dissecans can affect any joint, although the ankle is the third most common location after the knee and elbow. When the ankle is involved, the lesion usually affects the talar dome. The authors report a case of osteochondritis of the subtalar joint affecting the talus in a 14-year-old girl without any history of trauma. Plain radiographs did not show any bony pathology, and the diagnosis was made with computerized tomography. The patient was treated initially in a non-weight bearing plaster cast with antiinflammatory medication and subsequent physiotherapy. The conservative tre

STUDY DESIGN: An anatomic study.
OBJECTIVES: To evaluate the effect of displacement of the fractured posterior facet in tongue fracture of the calcaneus on the congruity of the subtalar joint.
METHODS: Eleven feet were used in this study. Seven females and four males with age range from 59 to 78. The specimens were dissected from both the lateral and the medial aspects of the calcaneus to expose these surfaces. A primary fracture line was created first, then a secondary line was engineered to simulate tongue fracture. Displacement of the superio-lateral fragment was done with 5-mm increment.

The articular facets of the sustentaculum tali have a variety of configurations that are generally viewed as nonmetric traits of little functional significance. Bruckner (1987), in contrast, has hypothesised that sustentaculum tali facet variations are functionally important because they influence subtalar joint stability. To test this hypothesis, 191 calcanei were analysed for correlations between sustentaculum tali facet morphology and osteoarthritis of the subtalar joint. Calcanei with 2 separate sustentaculum tali facets had a lower frequency of arthritic changes associated with joint inst

Routine radiographic analysis does not visualize the posterior aspect of the hindfoot (posterior facet of the talocalcaneal joint). To help evaluate patients with persistent hindfoot abnormalities, computerized tomographic coronal sections in 22 clubfeet were studied. Seventeen feet (77.3%) had a lateral subluxation of the calcaneus. Of 14 feet with acceptable radiographic analysis, 12 (85.7%) had a lateral subluxation of the calcaneus. This study is evidence that a lateral subluxation of the calcaneus can exist, through a horizontal rotation, in the presence of acceptable radiographic analysi

A series of 73 Grice extra-articular arthrodeses in 54 children with hind foot valgus is presented with an overall satisfactory result in 79% of these cases. The diagnostic categories in which we have used this procedure include: cerebral palsy, poliomyelitis, meningomyelocele, pes plano valgus, convex pes valgus, osteogenesis imperfecta, congenital hydrocephalus, partial adactylia, and congenital calcaneovalgus. The results with 11 meningomyelocele deformities are encouraging as only 2 of these children required a triple arthrodesis; 5 out of 6 patients in whom the opposite foot could serve a

Congenital talo-calcaneal coalition is one of the conditions responsible for spastic flat foot. The common site is across the middle talo-calcaneal joint, and although routine radiographic views of the foot do not show the coalition, important secondary signs may be present. The findings in 17 cases are reviewed, the following secondary signs being recognised: 1. A talar beak. 2. Narrowing of the posterior talo-calcaneal joint space. 3. Rounding of the lateral process of the talus. 4. Failure of visualisation of the middle subtalar joint in the lateral view, and 5. Asymmetry of the anterior su

OBJECTIVE: Various surgical techniques have been reported in treating calcaneal malunions over the decades, while the operations on single plane were accompanied by respective limitations. The purpose of this study is to evaluate the efficacy of a novel multiple reconstructive osteotomy for treating malunited calcaneal fractures without subtalar joint fusion.
METHODS: From March 12, 2010, to August 17, 2017, 10 patients (10 feet) with malunited calcaneal fractures were treated with multiple reconstructive osteotomy with subtalar joint-preserving operations. All patients were treated with a co

OBJECTIVE: The purpose of this study is to test the hypothesis that conarticular surfaces areas and curvatures are correlates of mobility at the hominoid talocalcaneal and talonavicular joints.
MATERIALS AND METHODS: Articular surface areas and curvatures of the talonavicular, anterior talocalcaneal, and posterior talocalcaneal joints were quantified using a total of 425 three-dimensional surface models of extant hominoid and fossil hominin tali, calcanei, and naviculars. Quadric surface fitting was used to calculate curvatures, pairwise comparisons were used to evaluate statistical differenc

Recent literature has proposed that restriction of joints in the rearfoot secondary to coalitions may lead to increased risk for severe ankle fracture after trauma. There is a paucity of literature regarding the rigidity of the ankle joint after arthrodesis of the subtalar and talonavicular joints. In this study, load-to-failure testing of cadaveric ankle joints with and without fusion of the subtalar and talonavicular joints was performed to determine if clinically relevant fracture patterns could be reproduced. Of the 3 fixation patterns studied, combined subtalar and talonavicular joint fus

BACKGROUND: Ankle osteoarthritis (OA) can significantly impact mobility and quality of life. Total ankle replacement (TAR) has emerged as an alternative to ankle arthrodesis due to its potential for preserving natural joint kinematics. However, it remains unclear if this maintained motion also limits progression of adjacent joint degeneration.
OBJECTIVE: This study aims to evaluate the influence of stemmed implants on the progression of subtalar (STJ) osteoarthritis and delayed secondary STJ fusion.
METHODS: A retrospective analysis was performed on 115 patients who underwent primary Stryker

BACKGROUND AND PURPOSE: Our primary aim was to compare the early complication rate (< 6 weeks postoperatively) after open or arthroscopic fusion of the subtalar joint. Secondary outcomes included late complications (> 6 weeks postoperatively), function, pain, and patient satisfaction.
METHODS: In this prospective randomized controlled trial, patients listed for subtalar joint fusion were included and randomized for open or arthroscopic fusion. Complications were assessed at scheduled visits at 2 and 6 weeks, 3, 6, and 12 months postoperatively. Functional scores, pain scores, and patient sati

INTRODUCTION: Malunited comminuted calcaneal fractures result in poor function due subtalar joint arthritis and altered biomechanics. We aimed to assess whether percutaneous subtalar joint screws after fracture reduction provide good outcomes for these difficult injuries.
METHODS: We retrospectively analysed 15 comminuted calcaneal fractures (in 14 patients) treated with percutaneous subtalar screw fixation. All patients had a minimum of 12 months' follow-up. Six patients had open injuries. On the preoperative and the latest postoperative radiograph, Bohlers angle, Gissane angle, calcaneal in

This cadaveric study assessed factors that could lead to nonunions in subtalar joint arthrodesis. The purpose of this study was to assist surgeons in achieving sufficient joint preparation in hopes to achieve more frequent arthrodesis of the subtalar joint. We evaluated the influence of experience in regard to cartilage preparation of the joint. We also assessed which quadrants of the subtalar joint were more likely to have unprepared cartilage. The subtalar joints in 17 cadaveric limbs were prepared by 17 participants with differing levels of experience. After the cartilage was denuded, the p

We present a unique case of an open talar neck fracture with medial subtalar joint dislocation. This rare and traumatic injury was treated with immediate open reduction of the subtalar joint and open reduction internal fixation of the talar neck fracture. After a follow-up of 2.2 years, highlighted by numerous complications including posttraumatic arthritis, soft tissue abscess, and fibrotic adhesions, the patient recovered sufficiently to return full activity.
