Patient specific finite element model of the face soft tissues for computer-assisted maxillofacial surgery.

This paper addresses the prediction of face soft tissue deformations resulting from bone repositioning in maxillofacial surgery. A generic 3D Finite Element model of the face soft tissues was developed. Face muscles are defined in the mesh as embedded structures, with different mechanical properties (transverse isotropy, stiffness depending on muscle contraction). Simulations of face deformations under muscle actions can thus be performed. In the context of maxillofacial surgery, this generic soft-tissue model is automatically conformed to patient morphology by elastic registration, using skin

[Computer assisted localization in endoscopic sinus surgery--state of the art? The Insta Trak system].

BACKGROUND: Image-guided surgery is increasingly acknowledged as useful technology also for endoscopic sinus surgery. For many years our department has been involved in the development and testing of devices for image-guided endoscopic sinus surgery. Since January 1997 we have been testing the "Insta Trak" System, which is based on electromagnetic tracking and was especially designed for use in endoscopic sinus surgery. METHODS: We tested practicability and accuracy of the system in a wide range of cases from standard sphenoethmoidectomies to tricky frontal sinus surgery, from endoscopic tumo

OPTIMISING ACCURACY OF COMPUTER-ASSISTED IMPLANT SURGERY USING NOVEL DRILLING KEY DESIGN: SIMULATION-BASED STUDY.

BACKGROUND: Accurate implant positioning remains the goal of guided surgery for improving surgical outcomes and minimizing complications. The aim is to compare the accuracy of standard versus novel drilling key in static-dynamic computer assisted implant surgery across D2 and D3 bone densities relative to a zero-deviation benchmark. METHODS: Sixty-four bone-level tapered implants were placed in 32 models across four groups (n = 16), stratified by drilling key designs (standard vs novel) and bone density (D2 vs D3). Preoperative cone-beam computed tomography (CBCT) was imported into the naviga

Repairing Facial Fractures with Interrupted Maxillary-mandibular Arches by Computer-assisted Reverse Planning Model Surgery.

BACKGROUND: Management of comminuted facial fractures with maxillary-mandibular arch interruption is difficult, resulting in inadequate bone reduction and malocclusion. Traditionally, a good quality dental splint is helpful, but difficult to obtain in acute trauma. We apply a computer-assisted design and three-dimensional printing technology to improve splint fabrication and utilization, thus facilitating restoration of dental occlusion and facial fracture. METHODS: We retrospectively reviewed patients who suffered from facial fractures with interruption of the maxillary-mandibular arches. We

Computer-assisted surgery in total knee replacement: advantages, surgical procedure and review of the literature.

INTRODUCTION: Total knee replacement (TKR) is one of the most frequent orthopaedic procedures performed every year. At the same time 20% of patients who underwent TKR are not satisfied with the outcome. The reasons are unknown; we think that a mechanical alignment beyond 3° of varus-valgus can represent the most important cause of failure of TKR and consequently patient dissatisfaction. MATERIALS AND METHODS: Neutral mechanical alignment is the main goal in every TKR: this can be achieved through different tools, such as extramedullary and intramedullary guides, patient-specific instrumentati

Computer assisted system for precise lung surgery based on medical image computing and mixed reality.

In patients with idiopathic pulmonary fibrosis (IPF), the role of 5‑methylcytosine (m5C)‑associated genes in the pathogenesis and development of the disease remains unclear. The present study aimed to identify reliable diagnostic markers based on the expression of m5C‑associated genes for the early detection of IPF. Count data were obtained by screening the IPF genome‑wide assay in the Gene Expression Omnibus database, followed by a comparison of m5C gene expression in patients with IPF and controls. The GSE150910 and GSE173355 datasets yielded a total of 23 differentially expressed m5C‑associ

Computer hexapod-assisted orthopaedic surgery provides a predictable and safe method of femoral deformity correction.

AIMS: Computer hexapod assisted orthopaedic surgery (CHAOS), is a method to achieve the intra-operative correction of long bone deformities using a hexapod external fixator before definitive internal fixation with minimally invasive stabilisation techniques. The aims of this study were to determine the reliability of this method in a consecutive case series of patients undergoing femoral deformity correction, with a minimum six-month follow-up, to assess the complications and to define the ideal group of patients for whom this treatment is appropriate. PATIENTS AND METHODS: The medical record

Correction of orbital wall fracture close to the optic canal using computer-assisted navigation surgery.

BACKGROUND: Repairing orbital wall fractures can result in serious complications, including enophthalmos, diplopia, or even blindness. Especially, surgeons worry about damaging the optic nerve while dissecting the optic canal area. We avoid these complications by using a navigation system that was adapted to stereotactic concept based on three-dimensional imaging of the patient's anatomy. Here, we report 5 cases of orbital wall fracture that were repaired using a navigation system. METHODS: The Navigation System II (Stryker, Freiburg, Germany) and the iNtellect Cranial Navigation (version 1.1

No difference in accuracy between pinless and conventional computer-assisted surgery in total knee arthroplasty.

PURPOSE: Many studies have demonstrated higher precision and better radiological results in Total knee arthroplasty (TKA) with computer-assisted surgery (CAS). On the other hand, studies revealed a lengthening of operation time up to 20 min for this technique and demonstrated rare additional complications as fractures and neurovascular injuries caused by the array pins and any intraoperative array dislocation leads to abortion of CAS. To combine the advantages and eliminate the disadvantages of standard CAS, we evaluated the accuracy of a so-called pinless CT-free version of knee navigation (p

Computer-assisted surgery patterns of ligamentous deformity of the knee: a clinical and cadaveric study.

Knee malalignment during total knee arthroplasty (TKA) is commonly classified as either varus or valgus on the basis of a standing anteroposterior radiograph. Computer-assisted surgery (CAS) navigation TKA provides precise dynamic evaluation of knee alignment throughout the full range of motion (FROM). The goal of this study was to classify patterns of CAS-generated knee deformity curves that match specific soft tissue contracture combinations. This can then be applied as an algorithm for soft tissue balancing on the basis of the preoperative knee deformity curve. Computer navigation-generated

No difference between tibia-first and femur-first techniques in TKA using computer-assisted surgery.

PURPOSE: The measured resection technique and the gap-balancing technique are two philosophies used in total knee surgery. It is still unknown whether one or the other technique provides superior results when computer-assisted surgery is performed. We hypothesized that the gap-balancing technique improves joint stability because the technique relies primarily on the soft tissue. METHODS: A prospective controlled study was performed in 116 patients using the tibia-first or femur-first technique. Advanced imaging findings and computer-assisted surgery of suspected synovial chondromatosis in the temporomandibular joint.

Synovial chondromatosis of the joint occurs mainly in teenagers and young adults. Only 3% of these neoplasms are located in the head and neck region. Synovial chondromatosis of the temporomandibular joint is therefore a very rare disorder. Therefore, developing a working, histological confirmation is required for differential diagnosis. In this case series, the outcome of histological investigation and imaging techniques are compared. Based on clinical symptoms, five cases of suspected synovial chondromatosis of the temporomandibular joint are presented. In each of the subjects, the diagnosis

Advances in computer-assisted single-photon emission computed tomography (SPECT) for epilepsy surgery in children.

Epilepsy surgery has emerged as an important option in the treatment of children with epilepsy that is refractory to antiepileptic drug management. The cornerstone of successful surgery is accurate localization of the brain region of seizure onset. Traditional techniques of seizure onset localization, e.g. surface electroencephalography (EEG) recording and magnetic resonance imaging (MRI), allow accurate localization in a significant number of patients. When the focus of seizure onset is not apparent from these non-invasive techniques, other methods of localization, e.g. intracranial EEG recor

Barriers to Clinical Integration of Artificial Intelligence in Computer-Assisted Dental Implant Surgery: A Critical Review.

Translating artificial intelligence (AI) from virtual implant planning to dental implant surgical execution is obstructed by technical, clinical, and socioethical barriers. This critical review appraises these impediments, focusing on tomographic data heterogeneity, closed hardware ecosystems that block kinematic data extraction, and algorithmic opacity that erodes clinician trust. Current literature over-relies on static retrospective validations with high bias risk. This gap should be bridged by a roadmap prioritizing federated learning, open-source interoperability, explainable AI, and pros

The role of computer-assisted navigation in pelvic tumor surgery: A systematic review and meta-analysis.

BACKGROUND: Pelvic bone tumors frequently pose significant challenges due to their proximity to noble structures, including blood vessels, nerves, and organs. Computer-assisted navigation (CAN) for pelvic bone oncology surgery was introduced in the early 2000s to improve accuracy and precision of pelvic tumor resections. The objective of this systematic review and meta-analysis was to evaluate the differences in surgical accuracy, clinical outcomes, recurrence rates, and complications in pelvic bone tumor resections performed with or without CAN. METHODS: The literature search was independent
