A study was performed to analyse antibiotic usage in a surgical department, with the object of providing a basis for improving antibiotic policies. Patients admitted to the Department of Surgery (134 beds) of the Waid Community Hospital in Zürich (400 beds) after 1st Jan. 1979, and who received antibiotics during the period between 22 Jan. and 31 March were included in the study (n = 154). Daily chartreview provided all necessary information on clinical, diagnostic and therapeutic data of the patients included. 154 patients had 178 treatment courses which were reviewed according to guidelines

In the 10 months since the establishment of a 24-hour replantation service, the centre at the Klinikum rechts der Isar of the Technical University of Munich has treated 162 amputation injuries; 132 underwent replantation surgery and 117 of these healed successfully. Surgical technique, medication and postoperative treatment with physiotherapy are described. Special emphasis is placed on the difficulties associated with crush and avulsion injuries; in most of them the only solution is an intermediate vein graft to bridge the defect in the vessels.

Hans Rudolph Ranke (1849-1887) studied medicine in Halle, located in the eastern part of Germany, where he also trained as a surgeon under Richard von Volkmann (1830-1889), during which time he became familiar with the new antiseptic technique that had been introduced by Joseph Lister (1827-1912). A clinical statistic survey was made on the operations performed at the department of urology, Tatebayashi-Kosei Hospital between June 1990 and May 2000. The total number of operations was 2,680, consisting of 217 operations of the kidney, 126 operations of the ureter, 454 operations of the bladder, 1,354 operations of the prostate, 186 operations of the penis and ureter, 305 operations of the scrotum, 16 urologic operations performed by laparoscopy and 22 other operations.

BACKGROUND: Sales data indicate a major increase in the prescription of antifungal drugs in the last two decades. Many new agents for systemic use that only recently have become available are likely to be prescribed intensively in acute care hospitals. Sales data do not adequately describe the developments of drug use density. Given the concerns about the potential emergence of antifungal drug resistance, data on drug use density, however, may be valuable and are needed for analyses of the relationship between drug use and antifungal resistance.

This review aimed to synthesize the evidence on the likelihood of harm and mortality on medical and surgical inpatient wards for people with severe mental illness (SMI). The various procedures of regional anesthesia were widely used before the introduction of intratracheal anesthesia; thereafter, it was replaced by the today commonly used general anesthesia. Regional anesthesia nonetheless plays an important role, primarily in outpatient treatment, because a large number of surgical departments still have no anesthesiologist on the staff. Often the surgeon is responsible for both the operation and the anesthesia. Indications, risks, and complications including legal problems related to regional anesthesia are discussed.

A permanent evaluation of a department's activity and the quality of health care it provides is needed to avoid inappropriate use resulting from a wide range of causes. The activity of a general surgery department treating and average of 1,500 patients per year and performing 1,200 operations was analyzed over the period 1986 to 1992.
