Surgical technique: Iliosacral reconstruction with minimal spinal instrumentation.

BACKGROUND: Posterior pelvic ring reconstruction can be challenging and controversial. The choice regarding whether to reconstruct and how to reconstitute the pelvic ring is unclear. Many methods provide stability but often are technically difficult and require excessive dissection. DESCRIPTION OF SURGICAL TECHNIQUE: This unique reconstructive technique uses the anterior aspect of the iliac crest with its attached muscle pedicle to provide a biologic scaffold for healing. The construct is secured with pedicle screws into the posterior column and S1 vertebral body with a spinal rod locked in co

Surgical management of instrumentation-induced coronary artery dissection.

The mechanism of instrument-induced coronary artery dissections (IICDs) is briefly presented. Depending on the coronary anatomy, three different situations that may occur are distinguished: (1) major coronary branches between the site of dissection and planned anastomosis; (2) no important collaterals at the same location; and (3) perforation of the coronary artery. Surgical methods applicable to each for the correction of these situations is presented.

Domain-agnostic weakly supervised surgical instrument segmentation.

Recent advancements in visual foundation models open new avenues in the field of surgical instrument segmentation in medical images. Segmentation foundation models provide high segmentation accuracy for objects of interest that are selected via prompts in the form of points, bounding boxes, or text. However, the choice of suitable prompts either requires manual interaction or relies on two-stage pipelines based on supervised, typically domain-specific models. This limits their applicability for domain-agnostic surgical instrument segmentation. We propose a method for surgical instrument segmen

Context-Aware Weakly Supervised Surgical Instrument Detection.

In minimally invasive surgery, the detection of surgical instruments is the core of many operating room assistive systems. However, traditional methods mostly rely on fully supervised learning, which requires the use of large-scale annotated datasets, and high-quality annotation in the medical field is extremely costly. In recent years, weakly supervised learning has shown great potential in medical image analysis, as it only requires image-level annotation to analyze surgical images. This paper introduces weakly supervised object detection into the field of surgical instrument detection, trai

Surgical site infections following instrumented stabilization of the spine.

BACKGROUND: Implant-associated infections are still a feared complication in the field of orthopedics. Bacteria attach to the implant surface and form so-called biofilm colonies that are often difficult to diagnose and treat. Since the majority of studies focus on prosthetic joint infections (PJIs) of the hip and knee, current treatment options (eg, antibiotic prophylaxis) of implant-associated infections have mostly been adapted according to these results. OBJECTIVE: The aim of this study was to evaluate patients with surgical site infections following instrumented stabilization of the spine

Development of an intelligent surgical instrument for otitis media with effusion.

To treat a worldwide common ear disease (OME), a device allowing fast grommet tube insertion has been designed in our earlier works (Gao et al., 2015 [1] and Liang et al., 2013 [2]). However, the instrument has to be manually placed as close as to the Tympanic Membrane before the insertion procedures. To realize a fully automated surgical process, the instrument shall be automatically manipulated to align to the axial direction of ear canal and proceed to complete the surgery. A vision-based servomechanism is proposed to solve the path planning problem. A fuzzy-gain-scheduled controller is pro

Surgical treatment of Scheuermann's disease with segmental compression instrumentation.

Twenty-one patients with Scheuermann's kyphosis had surgery for progressive kyphotic deformity of 50 degrees or greater. There were six adolescents, with a mean age of 15.6 years (range, 13-17 years) and 15 young adults, with a mean age of 25.4 years (range, 18-40 years). All patients had posterior spine arthrodesis with segmental compression instrumentation. Seven patients with rigid kyphosis had combined anterior and posterior spine arthrodesis. One patient died of superior mesenteric artery syndrome. In the group of 13 patients with posterior arthrodesis only, followup was 4.5 years. The me

Characterization of Trimmed Nerve Morphology Using High-Resolution Imaging: Comparison of Three Surgical Instruments.

PURPOSE: Trimmed proximal and distal nerve stumps should be morphometrically matching and show evidence of healthy fascicular tissue to potentiate nerve regeneration. We hypothesized that differences exist between nerve trimming instruments, providing guidance for optimal nerve-trimming techniques. METHODS: Three common surgical instruments were used to trim nerves in three human upper-extremity specimens: a slotted neurotome, a no. 11 surgical blade with a tongue depressor, and super-cut serrated microscissors. Peripheral nerves in flexor tendon zones II and V of the hand were trimmed by sur

Can viable bacteria be present in the surface of ready-to-use surgical instruments?

INTRODUCTION: biofilm is considered a challenge regarding treatment of chronic diseases and, after a detailed observation of cleaning and sterilization processes, it is considered could be a threat to sterility of surgical instruments that are "ready to use". Colored plastic bands (color coding tapes for marking surgical instruments) are frequently used to assist in the assembly of surgical instrument boxes. These bands form a lifting, which makes cleaning the material difficult. Epidemiological data regarding the frequency of surgical site infection in Brazil (up to 24% in Center-West Region)

Unseen threats: Lumens 2.0 study reveals the hidden challenges of cleaning lumened surgical instruments.

BACKGROUND: Surgical site infections can cause significant morbidity requiring lengthy antimicrobial treatment. Infections have been linked to surgical instruments with retained tissue and foreign debris, as the presence of blood or soil interferes with sterilization effectiveness. This study aimed to determine the prevalence of visible soil or debris inside instruments and evaluate the impact of recleaning efforts. METHODS: Borescopes were used to inspect lumens of instruments used for orthopedic, neurologic, or ear-nose-throat procedures. Whenever visible soil or debris was observed, the in

Minimising carbon and financial costs of steam sterilisation and packaging of reusable surgical instruments.

BACKGROUND: The aim of this study was to estimate the carbon footprint and financial cost of decontaminating (steam sterilization) and packaging reusable surgical instruments, indicating how that burden might be reduced, enabling surgeons to drive action towards net-zero-carbon surgery. METHODS: Carbon footprints were estimated using activity data and prospective machine-loading audit data at a typical UK in-hospital sterilization unit, with instruments wrapped individually in flexible pouches, or prepared as sets housed in single-use tray wraps or reusable rigid containers. Transmission electron microscopy: evaluation of damage in human oviducts caused by different surgical instruments.

Different wave length lasers (CO2, Nd-YAG, KTP-532), electrocautery and radiofrequency instruments were used to assess the degree of tissue damage in human oviducts. Excision of the human fallopian tube for tuboplasty is achieved most often by electrocautery, scalpel and, more recently, the Carbon Dioxide (CO2) Laser. There is not enough evidence at the present time to scientifically justify using one particular mode of incision which would show minimal damage to surrounding healthy tissue, especially heat lateral damage. The present study compares the tissue damage produced by the microelectr

Surgical instrument-tissue characterization via multi-task self-supervised instrument segmentation and motion estimation.

Surgical instrument recognition and tissue motion analysis are two critical tasks for instrument-tissue characterization (ITC) in robot-assisted minimally invasive surgery, with broad applications in surgical scene understanding and 3D reconstruction. While precise instrument segmentation and detailed motion map estimation are the foundation of these tasks, existing methods commonly address these two tasks independently, and little attention has been paid to the fusion of these two tasks for exploring the intrinsic relationships between spatial segmentation and temporal motion. To address this

Face and content validity of analog surgical instruments on a novel physics-driven minimally invasive spinal fusion surgical simulator.

Medical simulators are a modern-day technology that allow clinicians to acquire the skills and knowledge needed to perform complex surgical procedures. Validating these simulators is crucial prior to their integration in surgical training programs. However, surgical simulators are typically validated as a whole, without emphasizing validation of the instruments themselves. The purpose of this study was to design and validate analog surgical instruments for a novel, minimally invasive spinal fusion simulator. The surgical procedure was performed on cadavers and on a surgical simulator by experi

Real-time instance segmentation of surgical instruments using attention and multi-scale feature fusion.

Precise instrument segmentation aids surgeons to navigate the body more easily and increases patient safety. While accurate tracking of surgical instruments in real-time plays a crucial role in minimally invasive computer-assisted surgeries, it is a challenging task to achieve, mainly due to: (1) a complex surgical environment, and (2) model design trade-off in terms of both optimal accuracy and speed. Deep learning gives us the opportunity to learn complex environment from large surgery scene environments and placements of these instruments in real world scenarios. The Robust Medical Instrume
