Complications associated with bacitracin powder in surgical wounds.

OBJECT: There has been renewed interest in the application of concentrated antibiotic powder to surgical wounds as a method to decrease infection rates. While there is substantial medical literature describing the effectiveness and complications associated with vancomycin and gentamycin powders, very little has been reported regarding the safety and effectiveness of bacitracin powder in surgical wounds. In this paper the authors report their detailed analysis of potential bacitracin powder-related complications in a population of pediatric patients who underwent shunt surgery. METHODS: A deta

Microcirculation in the healing of surgical wounds in the oral cavity.

The aim of this research is to evaluate in vivo the characteristics of microcirculation after taking a biopsy sample from the oral mucosa. 20 patients were recruited to the study and all underwent an oral mucosa biopsy for the excision of benign neoformations. The modifications in the oral microcirculation were evaluated in vivo in correspondence to the surgical site through videocapillaroscopy at three different times: 30 min before the biopsy; 48 h after the biopsy; and 7 days after the biopsy. The statistical significance was checked with the Mann-Whitney U-test (P<0.05). The analysis of vi

Assessment and management of surgical wounds in clinical practice.

Successful nursing care of surgical wounds is dependent on the nurse's knowledge and understanding of normal wound healing physiology, the type of surgery performed, the method of closure and the optimal treatment of the resultant wound. Using this knowledge, nurses can provide a systematic and holistic patient assessment, and consider any potential wound-related complications.

Pediatric clean surgical wounds: is dressing necessary?

BACKGROUND/PURPOSE: The covering of the sutured surgical wound with a sterile dressing is usually considered a routine conclusion to an aseptic operation. The wound is usually left dressed for a minimum of 3 to 5 days. The main purpose of dressing is protection of the wound against bacterial contamination that remains a significant source of postoperative morbidity. The aim of this study was to compare the infectious local risk when the clean pediatric surgical wounds were dressed or left exposed without dressing after the completion of wound closure. METHODS: Four hundred fifty-one patients

[Problems in the diagnosis and surgical care of thoracoabdominal wounds].

The article presents an 11 years experience in work of a specialized department of thoracoabdominal trauma with an analysis of treatment of 292 wounded. The main causes of the thoracoabdominal wounds (TAW) were knife stabs (96.2% of the patients). The diagnosis of injuries to vital organs of the chest and abdomen is known to be very difficult. The X-ray analysis is one of major diagnostic methods for TAW. Emergency thoracoscopy is of great significance in diagnosis of intrathoracic injuries and laparocentesis and laparoscopy are of great value for intraperitoneal wounds. The sequence of measur

[Streptococcus bovis in a surgical wound and a colonic neoplasm].

The association of colorectal carcinoma and septicemia or endocarditis by Streptococcus bovis is well known. Nonetheless, other localizations of infection by Streptococcus bovis have not been associated with colorectal carcinoma. The case of association of colon neoplasm with infection by Streptococcus bovis localized in the surgical wound of resection of a prostate adenoma by the transvesical route carried out four months previously is presented. Possible intraoperative bacteremia colonizing the surgical wound due to colic compression during surgery may have been the cause. This localization

Selective surgical management of renal stab wounds.

Patients with stab wounds and haematuria were selected for surgical exploration if they had signs of severe blood loss, an associated intra-abdominal laceration or major abnormality on the intravenous urogram (IVU). Patients without these signs were selected for non-operative management, consisting of bed rest, an intravenous antibiotic for 24 h and regular observation. Of 95 patients, 60 (63%) were selected for non-operative management (Group 1) and 35 (37%) were selected for primary surgical exploration (Group 2). At surgery in Group 2, a major renal injury and/or associated intra-abdominal

Surgical priorities of abdominal wounded in a combat situation.

It is well established that abdominal wounded should be operated on as soon as possible. Combat conditions or mass casualty situations may dictate a delay in surgery because of higher priorities or lack of surgical facilities. A review of 178 abdominal wounded treated in an evacuation hospital during the 1973 and 1982 Israeli wars evaluates the effects of delay on mortality and complications. Surgery on 141 of the 178 abdominal casualties was delayed from 8 to 17 hours from the time of their injury. They were well supported hemodynamically along the chain of evacuation and none died en route.

Spontaneous exit of a gnathostome through a surgical wound.

A case of subcutaneous gnathostomiasis in which subsequently a gnathostome larva crept out through a surgical wound in a 22-year-old Thai male is described. This case disclosed that although a worm may creep out of the body through certain natural passages as well as the skin, spontaneous exit via a surgical wound is a possibility. Based on the morphology and number of the head-bulb hooklets and cuticular spines by light and electron microscopy and gnathostome was confirmed as a third stage larva of gnathos toma spinigerum.

Measured Wound Outcome Feedback Improves Surgical Site Infection Rates.

Feedback supports learning. No matter how effective medical professionals think they are, evidence of measured patient wound outcomes helps inform their wound care practice, empowering them to improve patient outcomes. Prospective randomized clinical trials (RCTs) proved this principle in relation to healing chronic wounds. Measuring wound healing outcomes and providing caregivers with feedback about the 4-week healing progress increased the percentage of diabetic foot ulcers healed within 20 weeks and venous leg ulcers healed within 24 weeks. Longitudinal research suggests this principle hold

[Modern aspects of wound sepsis in war surgical trauma].

The results of the treatment of 1020 wounded with symptoms of wound infection have been studied. The wounded were admitted with developed complications from the hospitals in Afghanistan. The syndrome of systemic inflammatory reaction detected in 239 (23.4%) wounded was the indication for application of the whole complex of antiseptic treatment. Sepsis in patients with complicated gunshot wounds was revealed only in 54 wounded, i.e. in 5.4%. Early diagnosis and multicomponent therapy resulted in a decrease of mortality rate in wound sepsis up to 9.3%.
