Botulinum Toxin for Treatment of Synkinesis: Effects on Anxiety and Depression.

BACKGROUND: Facial synkinesis is associated with increased anxiety/depression and decreased quality of life. OBJECTIVES: To evaluate changes in anxiety and depression among patients with synkinesis before and after chemodenervation as measured by validated scales. METHODS: A retrospective review of botulinum toxin naïve adults treated with chemodenervation for synkinesis was performed. Validated outcome metrics were compared before the first treatment and >3 months after the final treatment. Clinician-graded assessment used the Sunnybrook Facial Grading System (SB). Patient-reported assessme

Clinical Perspectives on the Pathophysiology of Facial Synkinesis: A Narrative Review.

IMPORTANCE: Facial synkinesis refers to pathologic cocontraction and baseline hypertonicity of muscles innervated by the facial nerve, commonly attributed to the aberrant regeneration of nerve fibers following injury. The pathomechanism and optimal treatment of facial synkinesis remain unclear. The goal of this review is to highlight current understanding of the epidemiology, pathophysiology, clinical presentation, assessment, and treatment of facial synkinesis. OBSERVATIONS: Research into the epidemiology and risk factors of facial synkinesis is limited due to a lack of large databases track

First case of presumed trigemino-oculomotor synkinesis in a dog.

An 11-year old, intact male Border Collie was referred with a history of subacute and progressive left eye exophthalmos and mydriasis associated with reduced pupillary light reflex, ventrolateral strabismus, and absence of physiologic nystagmus in the left eye. Neuroanatomical localization was consistent with a left oculomotor neuropathy, involving the general somatic and visceral parasympathetic efferent components. Computed tomography and magnetic resonance imaging of the head were performed. Imaging findings were consistent with an infectious-inflammatory process involving the left retrobul

Crumley's Classification of Laryngeal Synkinesis: A Comparison of Laryngoscopy and Electromyography.

OBJECTIVES/HYPOTHESIS: Applying the principles of misdirected nerve regeneration to the larynx, Roger Crumley in 1989 coined the term laryngeal synkinesis (LS) which he later (2000) classified into 4 types (type I - good voice, type II - involuntary twitches and poor voice, type III - adduction during inspiration, type IV - abduction during phonation). Neurophysiological data were not available for all LS patients at that time. The current study was undertaken to utilize and test the Crumley classification for a clinical interrater comparison and, secondly, compare predicted with actual laryng

Functional assessments of the rodent facial nerve: a synkinesis model.

OBJECTIVES/HYPOTHESIS: Rodent whisker movement has been used as a tool, after facial nerve manipulation, to quantify functional recovery. We have recently established a method to study functional correlates of aberrant regeneration of the facial nerve. Our objective was to establish normative parameters for both spontaneous and induced whisking and blinking behavior in a large group of normal rats. STUDY DESIGN: Prospective animal study. METHODS: Eighty animals underwent quantitative facial movement testing to measure simultaneous vibrissal movement and ocular closure for each side independe

On the origin of synkinesis in hemifacial spasm: results of intracranial recordings.

Recordings were made from facial muscles and the facial nerve near its entrance into the brain stem in patients with hemifacial spasm (HFS). The purpose of this study was to determine if the synkinesis commonly seen in patients with HFS could be linked to ephaptic transmission at the presumed site of the lesion (at the root entry zone (REZ) of the facial nerve). When the mandibular branch of the facial nerve was electrically stimulated, a response could be recorded from the orbicularis oculi muscles during the operation. The latency of the earliest response was 11.03 +/- 0.66 msec (mean respon

Facial-Stapedial Synkinesis Following Ramsay Hunt Syndrome: A Case Report.

We report the first documented case of facial-stapedial synkinesis (FSS) secondary to Ramsay Hunt syndrome (RHS) by describing the clinical manifestations and definitive surgical treatment outcome of this rare condition. A 35-year-old female presented with initial symptoms of severe right-sided facial weakness, otalgia, hyperacusis, ear fullness, and erythematous vesicles in the external auditory canal and tympanic membrane, suggestive of RHS. She later developed an intermittent "quivering" or "buzzing" sensation in her right ear initiated by facial movements such as closing her eyes and smili

Surgical Treatment for Postparalytic Facial Synkinesis: A 35-Year Experience.

BACKGROUND: Surgical intervention with combined myectomy and neurectomy followed by functioning free muscle transplantation has been proposed to effectively resolve the problem of postparalytic facial synkinesis since 1985, and it continues to be the authors' standard of care. The authors aim to provide evidence that this surgical strategy is effective for treatment of synkinesis and smile quality. METHODS: One hundred three patients with postparalytic facial synkinesis were investigated (1985 to 2020). They all underwent extensive removal of the synkinetic muscles and triggered facial nerve

Oculomotor synkinesis: an uncommon sequela of paediatric cavernous sinus thrombosis.

We report an unusual case of acquired oculomotor synkinesis as a sequela of cavernous sinus thrombosis in a child. A 4-year-old male child presented to our emergency services with sudden onset periorbital swelling with complete ptosis of the left upper eyelid. This was preceded by a febrile episode and a furuncle at the tip of the nose. Computerised axial tomography of the orbit revealed orbital cellulitis in the left eye. The child was started on systemic antibiotics followed by a short course of systemic steroids. MRI of the brain with contrast revealed left cavernous sinus and superior opht

Clinical correlates of ocular dominance in post-facial paralysis synkinesis.

PURPOSE: We aimed to explore the relationship between ocular dominance and synkinesis severity. METHODS: Through phone interviews and a web-based survey, patients were asked to test their ocular dominance via the Miles test. Additionally, patients were asked to complete the standardized synkinesis questionnaire to assess the severity of their symptoms, where higher scores correlate with more severe symptoms. A retrospective chart review and survey were conducted on eligible patients. RESULTS: Sixty-six participants with a mean age of 66.24 years (standard deviation 11.80 years) completed the

Modified Selective Neurectomy for Post Facial Paralysis Synkinesis Smile Dysfunction.

Following incomplete facial nerve injury, patients may develop aberrant facial nerve reinnervation, which can result in facial synkinesis. The treatment goals for patients with postfacial paralysis synkinesis are to improve resting oral commissure position, oral competence, facial and cervical tightness, and smile symmetry and spontaneity. Modified selective neurectomy of the facial nerve as described by Azizzadeh and colleagues is a targeted surgical method that allows the surgeon to eliminate the antagonist movements of the face and allow the favorable movements of the face to predominate, r
