Electrocardiographic differentiation of common type atrioventricular nodal reentrant tachycardia from atrioventricular reciprocating tachycardia via a concealed accessory pathway.

OBJECTIVE: The present study aimed to evaluate the diagnostic value of specific ECG markers in the differentiation of common type atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT) via a concealed accessory pathway. METHODS: One hundred and ten ECGs with paroxysmal narrow QRS complex, short RP tachycardia were evaluated. Subjects with overt ventricular pre-excitation during sinus rhythm were excluded from the study. The mechanism of arrhythmia was established during the electrophysiological study and confirmed by the efficacy of radiofrequen

Catheter ablation of orthodromic reciprocating tachycardia and atrioventricular nodal reentrant tachycardia in children with hypoplastic left heart syndrome.

INTRODUCTION: Experience with catheter ablation of orthodromic reciprocating tachycardia (ORT) and atrioventricular nodal reentrant tachycardia (AVNRT) in young children with hypoplastic left heart syndrome (HLHS) is limited. We report the feasibility, safety, and outcomes of catheter ablation of ORT and AVNRT in children with HLHS. METHODS AND RESULTS: This was a retrospective review of patients with HLHS who underwent catheter ablation for reentrant supraventricular tachycardias (excluding atrial tachycardias) between 2005 and 2017 at a single center. Descriptive data including demographics

Radiofrequency catheter ablation of coexistent atrioventricular reciprocating tachycardia and left ventricular tachycardia originating in the left anterior fascicle.

Coexistence of supraventricular tachycardia and ventricular tachycardia is rare. A patient with no structural heart disease and wide QRS complex tachycardia with a right bundle block configuration and right-axis deviation underwent electrophysiological examination. A concealed left atrioventricular pathway (AP) was found, and atrioventricular reciprocating tachycardia (AVRT) and left ventricular tachycardia (VT) originating in or close to the anterior fascicle of the left ventricle were both induced. Radiofrequency (RF) catheter ablation of the concealed left AP was successfully performed. Ten

Antidromic Nodoventricular Reciprocating Tachycardia With Left-Ventricular Insertion: Insights From High-Density Mapping.

We describe the case of a 38-year-old male with a long history of paroxysmal supraventricular tachycardia (PSVT) who was found to have typical slow-fast atrioventricular nodal reentrant tachycardia (AVNRT) and nodoventricular reentrant tachycardia (NVRT). During an electrophysiology study, a wide complex tachycardia with left bundle branch block (LBBB) morphology with V2 transition was induced, exhibiting ventriculo-atrial (VA) dissociation and characteristics suggesting an AV node-ventricle reentrant circuit. Careful mapping and pacing maneuvers supported the diagnosis of a nodoventricular re

Rare Case of Fetal Permanent Junctional Reciprocating Tachycardia Refractory to Prenatal Antiarrhythmic Therapy.

Permanent junctional reciprocating tachycardia (PJRT) is a rare form of atrioventricular reentrant tachycardia that is commonly resistant to most antiarrhythmic medication therapy and over an extended duration can result in tachycardia-induced cardiomyopathy. The prenatal presentation of PJRT is typically similar to that of other types of fetal supraventricular tachycardia (SVT), making it difficult to distinguish from other forms of SVT in utero by fetal echocardiography. Surface electrocardiography after delivery is typically required to make a definitive diagnosis of PJRT. We report a case

Antidromic Atrioventricular Reciprocating Tachycardia Using a Concealed Retrograde Conducting Left Lateral Accessory Pathway.

Atrioventricular reciprocating tachycardia is a common cause of undifferentiated supraventricular tachycardia. In patients with manifest or concealed accessory pathways, it is imperative to assess for the presence of other accessory pathways. Multiple accessory pathways are present in 4% to 10% of patients and are more common in patients with structural heart disease. In rare cases, multiple accessory pathways can act as the anterograde and retrograde limbs of the tachycardia.

Successful radiofrequency ablation in an infant with drug-resistant permanent junctional reciprocating tachycardia.

Over the past decade, the technique of radiofrequency ablation has evolved substantially. Currently, most forms of cardiac arrhythmias seen in children can be treated with good long-term results and low risk of adverse outcome. Curative arrhythmia treatment with this technique, however, is still uncommon in neonates and infants. Reported here is our experience in the management of an 8-week-old with drug-resistant permanent junctional reciprocating tachycardia.

[Complete atrioventricular block, a possible complication of radiofrequency ablation of reciprocating nodal tachycardia].

Radiofrequency ablation of the slow pathway of the reentry circuit is the usual radical treatment of nodal tachycardia. It is, however, possible to create atrioventricular conduction defects, the significance of which is not known. The aim on this study was to report the history of these conduction defects created during ablation of the slow pathway of the intranodal reentry circuit. Four cases were observed in a series of 27 patients. In one female patient, complete atrioventricular block was observed for 5 minutes before conduction returned to normal followed by recurrence of the tachycardia

Cycle length change during reciprocating tachycardia in patients with Wolff-Parkinson-White syndrome.

UNLABELLED: Cycle Length (CL) changes during reciprocating tachycardia (RT) were examined in 82 consecutive patients with Wolff-Parkinson-White syndrome (WPW) during electrophysiological studies. The significant CL changes (sudden and greater than 30 msec.) were found in 21 of 82 patients (26%). Thirteen patients had a manifest WPW and eight had a concealed WPW. An accessory pathway (AP) was located in the left side in 14 patients, the right side in four patients and the septum in two patients. One patient had multiple AP's. The development of ipsilateral bundle branch block during RT was resp

[Clinical experience with propafenone in paroxysmal supraventricular reciprocating tachycardias].

We studied the long term antiarrhythmic efficacy of Propafenon, (300-900 mg/day) in 25 patients (18 females, 7 males; mean age 37,6; follow-up 10 +/- 9,3 months) affected by episodes of disabling paroxysmal supraventricular reciprocating tachycardia. We performed an electrophysiologic study using cardiac catheterization in all 25 patients. Fifteen patients had a Wolff-Parkinson-White syndrome; in 8/25 pts the reentrant circuit was in the atrio-ventricular node and in 2/25 pts it was into the atrium. Ten of fifteen patients with Wolff-Parkinson-White syndrome and 5/8 patients with the reentrant

Variability of the VA interval at tachycardia induction: a simple method to differentiate orthodromic reciprocating tachycardia from atypical atrioventricular nodal reentrant tachycardia.

BACKGROUND: The differential diagnosis between orthodromic atrioventricular reentry tachycardia (AVRT) and atypical AV nodal reentrant tachycardia (aAVNRT) is sometimes challenging. We hypothesize that aAVNRTs have more variability in the retrograde conduction time at tachycardia onset than AVRTs. METHODS: We aimed to assess the variability in retrograde conduction time at tachycardia onset in AVRT and aAVNRT and to propose a new diagnostic tool to differentiate these two arrhythmia mechanisms. We measured the VA interval of the first beats after tachycardia induction until it stabilized. The

Reciprocated tachycardias in cardiac laminopathy: a clinical case report.

BACKGROUND: Cardiac laminopathies, associated with mutations in the LMNA gene, are a rare inherited disorder characterized by a broad range of clinical manifestations. There are currently no data on the association between supraventricular re-entrant tachycardias and LMNA-related cardiomyopathy. CASE SUMMARY: A 26-year-old male presented with either wide-QRS tachycardia with a left bundle branch block (LBBB) pattern or narrow QRS tachycardia, as well as a history of palpitations since age 15. Echocardiography showed no overt structural heart disease. Electrophysiological studies confirmed the

[Reciprocal supraventricular paroxysmal tachycardia in pregnancy. Apropos of a case].

The authors report a case of paroxysmal supraventricular reciprocal tachycardia, associated with transient Wolff-Parkinson-White syndrome in a 25-year-old woman, without heart disease, at 31 weeks gestation. The arrhythmia was successfully converted to sinus rhythm by intravenous flecainide. After delivery the arrhythmia and pre-excitation syndrome spontaneously disappeared and were not observed yet during the 6 months follow-up. Pregnancy may predispose patients with Wolff-Parkinson-White syndrome toward paroxismal supraventricular reciprocal tachycardia. The management of a similar case is d
