BACKGROUND: Primary bone tumours of the talus are a rare entity and seldom described in the literature. Due to this, often the diagnosis and management of such cases are delayed. This study aims to describe a spectrum of lesions identified within our department and provide a pictorial review with the goal to aid radiologists in the evaluation and diagnosis of such lesions.
MATERIALS & METHODS: A retrospective study was conducted within our department, identifying all talar lesions, confirmed on histopathological and radiological diagnosis.
RESULTS: Results revealed 75 cases of talar lesions.

A 24-year-old woman fell while ice climbing and landed on her left foot. Radiographs were taken and read as noncontributory. Eight days after the injury, the patient was still unable to bear weight due to significant pain, so she self-referred to physical therapy. Following evaluation, she was referred to primary care for further imaging to rule out fracture. The physician ordered magnetic resonance imaging, which revealed a left mildly displaced fracture of the intra-articular lateral process of the talus. J Orthop Sports Phys Ther 2017;47(8):579. doi:10.2519/jospt.2017.7424.

Avascular necrosis of the talus is one the most challenging problems encountered in posttraumatic reconstruction of the hindfoot. Since the first description of the talus injury in 1608 by Fabricius of Hilden, our knowledge of the talar anatomy, injuries, sequelae, and management has increased significantly. Adequate knowledge of the etiology, the extent of the disease, and the degree of patient symptoms are required to determine optimal treatment.

A 22-year-old woman suffered an anterolateral dislocation of the talus in a horse-riding accident. Immediately after admission, closed reduction was performed, and then a low knee cast was used for 12 weeks of immobilisation with partial weight bearing (10 kg). Nine months later, the patient had achieved nearly free range of motion of the ankle without pain (dorsal-plantar 10-0-30 degrees ). There were no radiological signs of osteoarthritis or talus bone necrosis. Immediate closed reduction led to an excellent outcome.

This review article provides a current concepts overview of osteochondral defects of the talus, with special emphasis on treatment options, their indications and future developments. Osteochondral defects of the talar dome are mostly caused by a traumatic event. They may lead to deep ankle pain on weight-bearing, prolonged swelling, diminished range of motion and synovitis. Plain radiographs may disclose the lesion. For further diagnostic evaluation, computed tomography (CT) and magnetic resonance imaging (MRI) have demonstrated similar accuracy. Computed tomography is preferred for preoperati

The reported incidence of osteosarcoma of the foot is very low. Osteosarcoma of the talus is extremely rare and few cases have been reported in the literature. The clinical findings are not typical, and osteosarcoma of the talus can be easily misdiagnosed, resulting in a delay in proper treatment. We report the case of a patient with osteosarcoma of the talus, from a series of 120 osteosarcomas treated at our hospital between 1966 and 2002.

Fractures of the talus in children are rare. Repeated clinical examination and imaging techniques such as magnetic resonance imaging are often needed to establish a diagnosis. In case of the late recognition of talus fractures, catastrophic results may occur for the hind-foot. This case report presents a 5 year old boy with a non-displaced talus neck fracture with good outcome after minimally invasive osteosynthesis.

A 7-year-old boy was referred to us with a 3-month history of swelling and pain on his left ankle. On laboratory evaluation, he only had an elevated erythrocyte sedimentation rate. X-ray showed an extensive lytic lesion of the talus bone. Chest roentgenogram was normal. The Mantuex skin test was negative. Histological examination of the biopsy tissue showed granuloma and caseating necrosis of tuberculosis. Talectomy with 6 months of antituberculous chemotheraphy resulted in a good clinical outcome.

Fracture of posterior process of talus is quite rare, but is associated with significant morbidity as it involves two articular surfaces. We report two such cases in which open reduction and internal fixation were done with good functional results. One of the cases had a concomitant medial malleolus fracture which to the best of our knowledge has not been reported so far.

Osteochondritis dissecans (OCD) of the talus is rare in children. The lesion is encountered on either the medial or lateral surface of the talar dome. The etiology is unknown, although trauma has been implicated, particularly in lateral lesions. Since 1983, 24 children have been treated for OCD of the talus at a major Canadian pediatric referral center. Two children had bilateral involvement, for a total of 26 lesions. There were 10 boys and 14 girls. The average age at initial presentation was 13 years 4 months (range 6 years 7 months to 17 years 1 month). The lesion involved the medial aspec

A review of the literature reveals that all studies have been performed retrospectively. These studies have included limited numbers of low-grade lesions and no prospective, randomized, comparison studies have been performed to suggest the superiority of CT scanning over MR imaging. The following conclusions, however, can be made. Plain radiographs are useful in the initial evaluation of patients with acute or chronic complaints of ankle pain and swelling. These initial studies, however, may not identify all osteochondral lesions of the talus, particularly lower grade lesions. CT scanning can

Seventeen patients with 18 open talar fracture-dislocations or total dislocations of the talus were reviewed to determine the functional outcome and incidence of infection. Seven of 18 feet (38%) developed infection. Infection was associated with extrusion of the talar body through the open wound (p < 0.025). Final follow-up was achieved in 13 of 17 patients (14 of 18 feet) at an average of 7 years and 4 months after injury. According to the Boston Children's Hospital ankle grading system, the overall results were considered excellent in one, good in five, fair in two, and failures in six feet

A 20-year-old man complained of increasing pain and swelling in the right ankle joint. Radiographs of the ankle demonstrated an ill-defined osteolytic lesion in the talus associated with a small, round bone formation just proximal to the neck of that bone. Histologic examination of the lesion showed osteoblastic osteosarcoma originating in the talus. Below-knee amputation was performed because of the peripheral location of the tumor and the close anatomic confines of the foot. A literature review demonstrated that the calcaneus and metatarsals are favorite sites of this tumor in the foot, with

A case of 35 years old patient with open, total, fixed dislocation of the talus reduced with heel traction is presented. The Sudeck syndrome subsided after one year. Weight bearing was not allowed for 7 months. An examination after 2.5 years revealed minor osteoarthritis, full range of movement in the foot but limited walking capability.

Solitary myeloma can present as either extramedullary plasmacytoma or solitary plasmacytoma of bone (SPB). More than half of reported cases of SPB are localized to the spine. The rest include appendage long bones of the skeleton, pelvis, and skull. The first case of SPB (to the authors' knowledge) is reported involving the talus bone of the foot. Although approximately 50% of SPB patients progress to multiple myeloma in the first 3 years after diagnosis, aggressive local therapy is indicated because the median survival of patients with SPB is better than that for multiple myeloma.

Severe infections of the talus are often associated with complete septic collapse of the talus. In this connection, open fractures with defects or significant comminution have a bad prognosis is as far as reconstruction of the talus is concerned. In the Department of Traumatology, Braunschweig, in 1995 three patients (all male, average age 35.3 +/- 10.2 years) were treated with cancellous bone grafting after talectomy performed because of infection and complete septic collapse of the talus. In two of these cases third-degree open total dislocation of the talus had been sustained. The third pat
