Nano-size tantalum particles suppress the osteogenic activity of osteoblasts and stimulate osteoclasts.

BACKGROUND: Tantalum (Ta) and its derivatives are inert, possess mechanical qualities such as corrosion resistance, and are biocompatible. They also offer structural support during surgical correction, such as bone grafts during surgery, in instances of dental or skeletal disabilities. However, various sizes of Ta particles could be expelled from the implant's surface due to mechanical stress and load-induced wear caused by micromotion between loose implant surfaces during usage. Therefore, the study examined the effects of nano (25 nm) and micro-sized Ta particles (10 μm and 40-50 μm) on oste

Histological Analysis of a Retrieved Porous Tantalum Total Ankle Replacement: A Case Report.

CASE: We present a case report documenting the retrieval and histological analysis of a porous tantalum (P-Ta) total ankle replacement (TAR) from a 50-year-old woman after a below-knee transtibial amputation. This rare opportunity to examine an intact TAR may help to better understand the implant-bone relationship because it would be in situ. CONCLUSION: In this case study, we demonstrate bone ingrowth to the first layer of the P-Ta and organized trabecular orientation, suggesting that equal bone load was achieved on the base and the rails in both components using a transfibular surgical appr

[Blow out fracture of the medial orbital wall--plastic repair with tantalum mesh].

Two cases of blow out fracture of the medial orbital wall were reported. The one was a 14 year-old boy and the other was a 32 year-old man. After hitting the eye against another man or a hard object, the patients felt diplopia, especially on lateral gaze toward the injured side. The blow out fracture was able to be diagnosed by plain roentgenography or tomography. The fractured bone as well as the entrapped soft tissue were clearly detected by CT scan. The fractures occurred at the thinnest part of the Lamina orbitalis ossis ethmoidalis (Lamina papyracea) in both cases. Objects with diameter l

Cationic Tantalum Oxide Nanoparticle and Neutral Iodixanol Contrast Agents Alter Cartilage Mechanical Properties.

Early detection of cartilage degeneration remains a critical unmet clinical need. While contrast-enhanced computed tomography shows promise in detecting early structural and compositional changes in articular cartilage, full adoption into clinical use requires a comprehensive understanding of how contrast agents affect cartilage function, including potential biomechanical alterations. This study investigates how exposure to two contrast agents, pre-clinical ultrasmall cationic tantalum oxide nanoparticles (Ta2O5-cNPs) and neutral iodixanol, approved for clinical use, affect the mechanical and

Total Knee Replacement with an Uncemented Porous Tantalum Tibia Component: A Failure Analysis.

Porous tantalum has been extensively used in orthopaedic surgery, including uncemented total knee arthroplasty (TKA). Favourable results were reported with earlier monobloc tibial components and the design evolved to modular implants. We aimed to analyse possible causes for extensive medial tibia bone loss, resulting in modular porous tantalum tibia baseplate fracture after primary TKA. Retrieved tissue samples were scanned with 3 MeV focused proton beam for Proton-Induced X-ray Emission (micro-PIXE) elemental analysis. Fractographic and microstructural analysis were performed by stereomicrosc

Early failures of porous tantalum osteonecrosis implants: a case series with retrieval analysis.

BACKGROUNDS: Porous tantalum osteonecrosis implants have been used in femoral head necrosis for several years, while the clinical outcomes were mixed. As a joint-preserving surgery, early necrosis deterioration and conversion to total hip arthroplasy failed our expectation. We hence investigate an observational study with retrieval analysis to find out the underlying reasons. METHODS: Thirteen patients were treated with core decompression and implantation of a tantalum rod. The cases were evaluated both functionally and radiologically. We retrieved and analyzed the micro-structural changes an

A biomechanical analysis of initial fixation options for porous-tantalum-backed glenoid components.

BACKGROUND: Porous-tantalum (PT)-backed glenoid components have recently been developed to improve fixation and minimize the incidence of glenoid component loosening, which remains a key limiting factor in long-term survival in total shoulder arthroplasty. PT-backed glenoids promote bony ingrowth as a method of preventing glenoid loosening at the prosthesis-glenoid interface. The use of polymethyl-methacrylate (PMMA) cement for initial fixation may prevent osteointegration due to mechanical occlusion of the porous surface and the nonosteoconductive properties of PMMA. This study aims to invest

A monoblock porous tantalum acetabular cup has no osteolysis on CT at 10 years.

BACKGROUND: Aseptic osteolysis has been the single most important factor limiting the longevity of a THA. A great deal of attention has been focused on the development of implants and materials that minimize the development of osteolysis. The monoblock porous tantalum acetabular cup was designed to minimize osteolysis, but whether it does so is unclear. QUESTIONS/PURPOSES: We evaluated the incidence of osteolytic lesions after THA using a monoblock porous tantalum acetabular component. METHODS: We retrospectively reviewed 51 patients who had a THA using a monoblock porous tantalum acetabular

Can porous tantalum be used to achieve ankle and subtalar arthrodesis?: a pilot study.

UNLABELLED: A structural graft often is needed to fill gaps during reconstructive procedures of the ankle and hindfoot. Autograft, the current gold standard, is limited in availability and configuration and is associated with donor-site morbidity in as much as 48%, whereas the alternative allograft carries risks of disease transmission and collapse. Trabecular metal (tantalum), with a healing rate similar to that of autograft, high stability, and no donor-site morbidity, has been used in surgery of the hip, knee, and spine. However, its use has not been documented in foot and ankle surgery. We

[The use of tantalum cones for reconstruction of bone defects in revision total knee arthroplasty].

OBJECTIVE: Revision arthroplasty of the knee is often associated with substantial femoral and/or tibial bone loss. Tantalum cones are used to reconstruct these defects and to improve initial stability. This requires an implantation in the "press-fit" technique with maximum contact to the host bone. INDICATIONS: Tantalum cones may be used in grade 2-3 femoral and/or tibial defects according to the AORI (Anderson Orthopedic Research Institute) classification system. CONTRAINDICATIONS: There are no contraindications described. SURGICAL TECHNIQUE: After removal of the implant and cement remnant

[Tantalum in diagnosing tumors of the ureter and renal pelvis. A preliminary report (author's transl)].

Differential diagnosis of small tumors of the ureter and renal pelvis is often difficult. Retrograde pyelogramm and tri-iodied contrast material is not effective in diagnosing small tumors and their surface condition. A retrograde pyelogram with a tantalum powder solution offers the following advantages: 1. Clearer representation of the tumor surface. 2. X-ray films show sharper contrast. 3. Small tumors could be seen that are not visible with routine contrast material. Preparation of tantalum solution and the technique of retrograde pyelogram are described.
