Eight-year chronic wound caused by Tarlov's cyst: a case report.

BACKGROUND: Tarlov's cyst is often underdiagnosed since it is difficult to identify without imaging assistance. Herein, we report the case of a young girl who presented with an 8-year history of chronic osteomyelitis of bilateral proximal phalanges and metatarsal bones caused by a Tarlov's cyst that did not contain a nerve root. The chronic wound in the forefoot is an unusual presentation and resulted from the Tarlov's cyst accompanied with tethered conus syndrome. CASE PRESENTATION: A 10-year-old Asian girl presented with an 8-year history of chronic osteomyelitis of bilateral proximal phala

A case of symptomatic cervical perineural (Tarlov) cyst: clinical manifestation and management.

Perineural (Tarlov) cysts are most often found in the sacral region and are rare in the cervical spine. Although they are usually asymptomatic, a small number of those at the lumbosacral level have been known to produce localized or radicular pain. Few reports are available on symptomatic perineural cysts in the cervical spine and it has not been discussed how they should be managed. We present here a case of cervical perineural cysts with persistent radicular pain where the pain was adequately managed with repetitive transforaminal epidural steroid injection (TFESI). The patient had experienc

Sacral plexus catheter analgesia in a patient with traumatic sacral fractures and Tarlov cysts.

We report the use of a sacral plexus catheter for continuous local anaesthetic infusion in a patient with a unilateral sacral ala fracture following a fall from a horse. Although sacral plexus blockade has been well described for lower limb surgery, an ultrasound-guided continuous catheter technique for conservatively managed fractures has not been described. Despite appropriate oral analgesia, the patient reported severe pain, particularly in the right gluteal region, which meant she was unable to sit upright or take deep breaths. A sacral plexus catheter was inserted with ultrasound guidance

Sacral Tarlov Cyst as a Rare Cause of Lower Limb Radiculopathy in a Child.

Tarlov cysts (TC) are uncommon perineural cysts that may arise anywhere along the spine, especially in the sacral region. To date, symptomatic TCs in children are very rare. Most of these benign lesions tend to be incidental discoveries during neuroimaging for other reasons. Nonetheless, they have been occasionally reported to cause sensorimotor-related neuropathy in the lower back, pelvis, legs, and urogenital system due to direct compression of the adjacent nerve roots. Management of adult TCs is well-described. In contrast, there is a paucity of contemporary literature on symptomatic TCs th

Anaesthetic considerations for elective cesarean section in a patient with a giant Tarlov cyst.

Tarlov cysts are a pathological dilatation of the meninges. Their incidence is more frequent in women between 30 and 50 years of age. The imaging test of choice for diagnosis is MRI. Of unknown etiology, most cases are asymptomatic, but symptoms of radicular irritation, among others, may occur. The therapeutic possibilities are multiple, reserving surgical excision as the last option. We report a case of successful spinal anaesthesia for elective cesarean section in a patient with a giant Tarlov cyst but with potential airway compromise, in whom the risks of general anaesthesia would be increa

Incomplete cauda equina syndrome secondary to haemorrhage within a Tarlov cyst.

Sacral perineural (Tarlov) cysts are benign, cerebrospinal fluid containing lesions of the spinal nerve root sheath. They are usually asymptomatic; however, a small proportion have the potential to cause compression of nerve roots and/or the cauda equina. We report a case of a 61-year-old man who presented with acute onset back pain associated with bilateral radiculopathy. Between referral and consultation, the patient developed urinary dysfunction which resolved spontaneously.MRI revealed haemorrhage within a Tarlov cyst, resulting in compression of the cauda equina. Due to the considerable cl

Surgical management of symptomatic Tarlov cysts: cyst fenestration and nerve root imbrication-a single institutional experience.

BACKGROUND: Tarlov cyst disease is a collection of cerebrospinal fluid between the endoneurium and perineurium of spinal, usually sacral, nerve roots. These cysts can become symptomatic in 20% of patients, causing lower back pain, radiculopathy, bladder and bowel dysfunction necessitating medical or surgical intervention. Different surgical and non-surgical modalities have been described for the treatment of symptomatic Tarlov cysts. However, there has been no published study that examined types of surgical techniques side by side. Our study presents a preliminary experience in the surgical ma

Spinal Cord Stimulation for the Treatment of Refractory Pain From Tarlov Cysts: A Case Report.

Tarlov cysts are extradural meningeal cysts with a collection of cerebrospinal fluid that most often affects sacral nerve roots, causing chronic low back pain and radiculopathy. Still, there is no consensus regarding the best treatment for symptomatic cysts. We describe a patient who developed worsening lower back pain and radiculopathy after interventional drainage and surgical management. Medication and various procedures failed to relieve the pain. Subsequently, the patient received significant pain relief from spinal cord stimulation (SCS). This case provides evidence that SCS could be use

Successful Surgical Ligation of a Giant Tarlov Cyst at the Cyst Neck Including the Nerve Root: A Case Report.

Perineural cysts are meningeal protrusions at nerve roots that can be asymptomatic but may require surgical intervention. Tarlov cysts, specifically located at sacral nerve roots, lack a consensus on optimal surgical treatment. This case report describes surgical management of a giant Tarlov cyst. A 29-year-old woman with infertility, low back pain, and constipation was found to have an 11 cm giant pelvic cyst originating from the left S3 nerve root. Preoperative imaging and symptoms suggested a perineural cyst. Intraoperative electromyographic monitoring revealed that the S2 nerve root could

A Rare Cause of Urinary Incontinence in a Child: A Case of Tarlov Cyst and a Review of Literature.

Tarlov cysts are formed by ectasia of the perineural spaces around the spinal nerve roots in or distal to the dorsal root ganglion. The cerebrospinal fluid constitutes the cerebrospinal fluid content. Pathogenesis and clinical findings remain unclear. The majority of the defects are asymptomatic. However, in the case of nerve root or spinal cord compression, symptoms such as low back pain, radiculopathy, and bowel and bladder dysfunction occur. Tarlov cyst is rare, and there is limited data on its clinical findings and incidence. This study presents a 10-year-old child patient with urinary inc

Spontaneous Rupture of a Huge Presacral Tarlov Cyst Leading to Dramatic Neurologic Recovery.

BACKGROUND: This manuscript discusses the case of huge presacral Tarlov cysts (TCs) and the substantial neurologic recovery noted in the patient following spontaneous rupture of the most prominent cyst. Perineural or TCs are nerve root cysts, which are usually incidental findings on magnetic resonance imaging (MRI) and are most frequently observed in the sacral spine. Symptomatic lesions are rarely encountered. CASE DESCRIPTION: In this case, a 44-year-old woman presented with anal and vulva pain on the right side, and bladder and bowel dysfunction. MRI of the lumbosacral spine showed multipl

Isolated lumbar intradural extra medullary spinal cysticercosis simulating tarlov cyst.

Spinal cysticercosis is a very uncommon manifestation of neurocysticercosis, which is caused by the larvae of Taenia solium. Here, we present a rare case of isolated lumbar intradural extramedullary neurocysticercosis, initially thought to be Tarlov cyst. A 40-year-old man, presented with low backache for 1-year with radiation of pain to right leg for 3 months. The patient was treated successfully with the surgical removal of the cyst, followed by medical treatment. Spinal neurocysticercosis should be considered in the differential diagnosis in high-risk populations, with new symptoms suggesti

Microsurgical reinforced radiculoplasty for symptomatic sacral Tarlov cysts in children: a retrospective study assessing safety and efficacy.

Symptomatic sacral Tarlov cysts are rare in the pediatric population. Traditional surgical methods are associated with high recurrence rates and risks of neurological injury. Microsurgical reinforced radiculoplasty (MRR), which focuses on reconstructing the nerve root sleeve to obliterate the cerebrospinal fluid communication, may offer a superior alternative. This study aimed to evaluate the safety and efficacy of MRR in children. We conducted a retrospective analysis of 24 consecutive pediatric patients (<18 years) with symptomatic sacral Tarlov cysts who underwent MRR at our institution bet

Efficacy of neuroendoscopic-assisted surgery in the management of symptomatic sacral perineural (Tarlov) cysts: a technical report.

INTRODUCTION: The Tarlov cysts are pathological enlargements of the cerebrospinal fluid spaces between the endoneurium and perineurium, which can cause intolerable sciatic pain, motor impairment of lower limbs, and bladder/bowel dysfunction. Currently, the treatment results are unsatisfactory due to the low cure rates and extensive surgical trauma. Thus, there is an ongoing exploration of surgical techniques for Tarlov treatment. In the current study, we present a novel neuroendoscopic-assisted technique that combines the fenestration, leakage sealing, and tamponade of the Tarlov cyst. METHOD

Uncommon cause of radiculopathy: A case of symptomatic Tarlov cyst in an elderly female and literature review.

KEY CLINICAL MESSAGE: Tarlov cysts are uncommon causes of sacral radiculopathy, with particular predilection to second and third sacral roots, requiring timely diagnosis with lumbosacral MRI, and surgical management if symptomatic. ABSTRACT: Tarlov cysts or Type II meningeal cysts, are CSF-filled sacs located in the extradural space of the sacral spinal canal, commonly originating at the dorsal root ganglion. While they were first documented by Tarlov in 1938, their etiology remains uncertain, with theories suggesting trauma-induced bleeding or congenital abnormalities. These cysts, estimated

Presacral Tarlov Cyst as an Unusual Cause of Abdominal Pain: New Case and Literature Review.

BACKGROUND: A presacral Tarlov cyst is an unusual cause of abdominal pain. The unusual location of a presacral Tarlov cyst can lead to misdiagnosis and inappropriate treatment. To guide appropriate diagnosis and treatment, we reviewed causes, clinical presentations, radiologic features, and management options of this extremely rare lesion. METHODS: We performed an English-language literature search using PubMed to identify reports of patients with presacral Tarlov cysts. The diagnosis was based on classic radiologic signs. Seven patients described in 7 publications met the inclusion criteria

Beyond the Cyst: The Many Contributions of Isadore Tarlov (1905-1977) to Contemporary Neurosurgical Practice.

: Isadore Max Tarlov (1905-1977) is primarily remembered for his 1938 description of the eponymous perineural "Tarlov cyst." However, during his long career as a neurosurgeon and researcher, he was responsible for many other observations and inventions that influenced the development of neurosurgery in the 20th century. While studying at Johns Hopkins Medical School he was acquainted with Walter Dandy, and he became the first resident to study under Wilder Penfield at the newly formed Montreal Neurological Institute. He made many novel observations about peripheral and cranial nerve anatomy, p

Painless legs and moving toes syndrome associated with a sacral Tarlov cyst: a case report.

BACKGROUND: Painless legs and moving toes syndrome is a very rare syndrome characterized by continuous and involuntary movement of the toes. The etiology of the disease is not clear though it has been linked to a wide range of neuronal insults including proximal root compression and neuropathy. A previous study has reported bilateral painful legs and moving toes syndrome in a patient with a sacral Tarlov cyst. In this report we present a case of unilateral painless legs and moving toes syndrome in a woman with a sacral Tarlov cyst. CASE PRESENTATION: A 50-year-old Mediterranean woman presente
