Get real: clinical testing of patients' reading abilities.

Education of cancer patients is complicated by a number of factors including timing, understanding of medical terms, and anxiety-induced inattention. The concern about patient education has led to the common practice of providing brochures about cancer, responses to cancer, treatment, and management of side effects. This material is often written at reading levels that do not match the reading ability of the patient. Research has indicated that the stated educational level is not equivalent to reading level. Realistic testing of a patient's reading ability thus becomes important in choosing th

Diabetic foot ulcers: prevention, diagnosis and classification.

Diabetic ulcers are the most common foot injuries leading to lower extremity amputation. Family physicians have a pivotal role in the prevention or early diagnosis of diabetic foot complications. Management of the diabetic foot requires a thorough knowledge of the major risk factors for amputation, frequent routine evaluation and meticulous preventive maintenance. The most common risk factors for ulcer formation include diabetic neuropathy, structural foot deformity and peripheral arterial occlusive disease. A careful physical examination, buttressed by monofilament testing for neuropathy and

[Model for the functional instruction of swallowing].

It is difficult to teach students about the mechanism of swallowing. There are three phases of swallowing; oral phase, pharyngeal phase and esophageal phase. The bolus of food is propelled to back of mouth by the tongue and the swallowing reflex happens. After nasopharynx and mouth closure, the glottal closure occurs, then hyoid and larynx are lifted by the contractions of suprahyoid and thyrohyoid muscles. As for the epiglottis, it is compressed by the tongue and inclines downward. As the larynx is lifted upward and anteriorly, slight vacuum is caused in the lower pharynx and upper esophagus

[Educational effects on radiological technology students of programs using a simulated aged person and hemiplegia patient].

A practical training program that enabled students to experience an aged person and hemiplegia patient was developed as part of the "Practice Program in Medical Imaging Technology," to help the students understand how to support patients in the X-ray examination setting. Eighty-three third-year radiological technology students participated in the program. During the hour-long course, they experienced a simulated program that contained various checkpoints. The students pointed out the difficulty patients had in going up and down stairs (51.8%) and their feelings of uneasiness and fear of blindn

Polycystic ovary syndrome: it's not just infertility.

Recent diagnostic and pharmacologic developments have focused renewed attention on polycystic ovary syndrome. Clinical features of the syndrome include anovulation, hyperandrogenism and menstrual dysfunction, but several other abnormalities, including hyperinsulinemia, luteinizing hormone hypersecretion, elevated testosterone levels and acyclic estrogen production, have been documented. Accompanying obesity and lipid abnormalities compound the risk of developing diabetes mellitus or cardiovascular disease, and chronic anovulation increases the risk for endometrial cancer. A careful history and

Clinical evaluation and treatment options for herniated lumbar disc.

Degeneration of the intervertebral disc from a combination of factors can result in herniation, particularly at the L4-5 and L5-S1 levels. The presence of pain, radiculopathy and other symptoms depends on the site and degree of herniation. A detailed history and careful physical examination, supplemented if necessary by magnetic resonance imaging, can differentiate a herniated lumbar disc from low back strain and other possible causes of similar symptoms. Most patients recover within four weeks of symptom onset. Many treatment modalities have been suggested for lumbar disc herniation, but stud

Chronic bronchitis: primary care management.

Chronic bronchitis is a clinical diagnosis characterized by a cough productive of sputum for over three months' duration during two consecutive years and the presence of airflow obstruction. Pulmonary function testing aids in the diagnosis of chronic bronchitis by documenting the extent of reversibility of airflow obstruction. A better understanding of the role of inflammatory mediators in chronic bronchitis has led to greater emphasis on management of airway inflammation and relief of bronchospasm. Inhaled ipratropium bromide and sympathomimetic agents are the current mainstays of management.

Diagnosis and treatment of leptospirosis in the primary care setting.

Leptospirosis is an infectious illness that usually develops 1 to 2 weeks after indirect or direct exposure to the urine of infected animals. Infection occurs through open cuts, absorption through intact mucous membranes, and drinking contaminated water. The typical presentation is a flulike syndrome of sudden onset that tends to resolve within 2 weeks. A mild immunologic phase usually presents as an aseptic meningitis. A more severe immunologic phase (Well's syndrome) can result in jaundice, renal failure, adult respiratory distress syndrome, disseminated intravascular coagulation, and death.

A cognitive strategy instruction to improve math calculation for children with ADHD and LD: a randomized controlled study.

The authors examined the effectiveness of cognitive strategy instruction based on PASS (Planning, Attention, Simultaneous, Successive) given by special education teachers to students with ADHD randomly assigned by classroom. Students in the experimental group were exposed to a brief cognitive strategy instruction for 10 days, which was designed to encourage development and application of effective planning for mathematical computation, whereas the comparison group received-standard math instruction. Standardized tests of cognitive processes and math achievement were given at pretest. All stude

Improving Representation and Knowledge of Dermatological Conditions in Skin of Colour in Undergraduate Dermatology Teaching: An Educational Intervention.

BACKGROUND: Nottingham University medical students undergo a two-week rotation in dermatology which incorporates teaching sessions, including didactic lecture presentations, tutorial seminars on skin cancers and acute dermatological conditions and patient simulations. Many studies highlight the need to improve skin of colour teaching in medical school curricula globally, as low representation can negatively affect physician knowledge throughout their career. METHODS: Using a pre-post intervention study design, Nottingham University Hospitals NHS Trust (NUH) teaching materials were adapted by

Assessment of forceps use in obstetrics during a simulated childbirth.

BACKGROUND: In obstetrics, manipulations are mainly learned during real deliveries. To minimize the risks linked to such training, we propose a childbirth simulator as a teaching tool in hospitals. More specifically, we focus on training with forceps during obstetric manipulation. METHODS: The training method can be divided into two steps: the teaching of forceps placement, and the extraction manipulation. In this paper we focus on the extraction manipulation on the simulator and the analysis of the results, taking into account several parameters and using an evaluation function to obtain a g
