BACKGROUND: Isolated fetal complete heart block is an uncommon finding, with a mortality rate of 20-30%. Various treatment modalities have been reported, with no consistent success.

Three out of nine unstable asthmatics whose asthma is subjectively and objectively better controlled on continuous subcutaneous terbutaline infusions (6-12 mg/24 hours) developed tender indurated subcutaneous swellings at the infusion site. In the two cases reported here in detail, histologically the lesions were areas of panniculitis and deep dermal collagen necrosis. Further investigation of the possible causative mechanism for these lesions--drug solution pH, osmolality, or impurities--suggests that impurities in the solution are the most likely cause.

A method is described for the determination of unconjugated terbutaline in plasma. The assay is based on gas chromatography chemical ionization mass spectrometry with ammonia as the reagent gas. Terbutaline, a selective beta-2 adrenergic receptor stimulator, has been used to decrease myometrial activity and improve uteroplacental blood flow in 15 patients with acute intrapartum fetal distress. In all cases electronic monitoring gave evidence of partial or total fetal recovery after therapy. The effect of terbutaline, a "selective" beta2-receptor stimulator, was compared with that of placebo in 30 patients in premature labor. Treatment consisted of an intravenous infusion for at least 8 hours, and then of subcutaneous injections four times daily for 3 days together with peroral treatment, which was continued until the end of week 36 of pregnancy. In 12 of 15 terbutaline-treated patients (80 per cent) premature labor was arrested beyond the treatment period, compared with three of 15 in the placebo group (20 per cent).
