Periodontal pathogens in the placenta and membranes in term and preterm birth.

INTRODUCTION: Preterm birth is a common cause of adverse neonatal and childhood outcomes. It is commonly associated with infection of the maternal-fetal interface. The relationship between periodontitis and preterm labour is controversial. METHODS: Control placental tissues from uncomplicated term births were compared with those from spontaneous preterm births for incidence of common periodontal bacteria. A chi-square analysis was used to compare the populations, with significance determined at p=<0.05. RESULTS: The study group comprised 29 control women who had an uncomplicated term birth,

Prevalence of Late Preterm and Early Term Birth in Portugal.

INTRODUCTION: Late preterm birth (defined as birth between 34 and 36 complete weeks' gestation) and early term birth (defined as birth between 37 and 38 complete weeks' gestation) have become a topic of recent discussion as the morbidity associated with delivery at these gestational ages has become increasingly evident. Our objective was to evaluate the characteristics of late preterm and early term birth in Portugal. MATERIAL AND METHODS: We developed a survey questionnaire that was sent to the Obstetric Department of all public hospitals in Portugal. The questionnaire consisted on questions

The Methodological Pitfalls Complicating the Analysis of Early Term Births.

BACKGROUND: Evaluating the success of policies aiming to reduce early term birth (37+0-38+6 weeks' gestation) is analytically challenging. OBJECTIVES: Determine the best method of reporting temporal changes of early term birth rates. METHODS: We analysed 54,646 singleton births at Western Australia's main tertiary hospital (2009-2019). Using interrupted time series, segmented and nominal regression, we assessed how varying gestational age outcomes, denominator, or statistical method affected conclusions regarding temporal changes (pre and post-Initiative implementation) in the timing of birt

Term twin birth - impact of mode of delivery on outcome.

INTRODUCTION: The main aims of this study were to compare maternal and neonatal outcomes in term twin birth according to the planned mode of delivery and to study the effects of chorionicity and inter-twin delivery time on neonatal outcome. MATERIAL AND METHODS: A single-center cohort study of 495 women with twin deliveries at ≥37+0 weeks of gestation. Term twin deliveries were divided into a trial of labor group (TOL, 69.3%) and a planned cesarean section (CS) group (30.7%). The primary outcomes were maternal and neonatal morbidity. RESULTS: 80.8% of women attempting TOL achieved vaginal bi

Burden of elective early-term births in rural Appalachia.

OBJECTIVES: Infants delivered at ≥ 37 weeks' gestation are considered full term, but research has demonstrated those born at 37 to 38 weeks (early term) have a higher risk for poor birth outcomes than deliveries at 39 to 41 weeks (full term). Despite this, many deliveries occur electively (scheduled, no medical indication) before 39 weeks. This study examined the risks of elective early-term delivery in a disadvantaged, rural sample and compared these results with national findings. METHODS: Data were available for 638 rural women, recruited prenatally from three counties in rural southern Ap

Biochemical markers for the prediction of spontaneous pre-term birth.

OBJECTIVES: The aim of this study was to identify predictive biochemical markers for preterm labor. METHODS: In this prospective study we included 225 asymptomatic Caucasian women consecutively enrolled at 24 weeks of gestation. The following data were collected only once at 24 weeks of gestation: vaginal pH, vaginal fFN, cervical and serum concentration of IL-6, IL-8 and TNFalpha, maternal blood serum, ferritin. Student's t-test, the chi(2)-test and multiple linear regression were used as statistical methods. RESULTS: There were no differences between the age of patients, parity and gestati

Temporal association of maternal weight gain with early-term and preterm birth and low birth weight babies.

BACKGROUND: Low birth weight and preterm or early-term babies may have a higher risk of poor health. One of the main factors is the weight gain of a pregnant woman during gestational weeks in the second and third trimesters. Changes in weight over a month in a pregnant woman might also have an impact on infant outcomes. This study aimed to investigate the association between maternal weight at different time points and low birth weight and preterm or early-term babies (premature babies). METHODS: A total of 156 pregnant women were recruited. Maternal weight was collected at different gestatio

Gestational Hypertension as a Mediator of Prenatal Ozone Exposure and Term Low Birth Weight: Birth Cohort Study.

BACKGROUND: Ambient ozone (O3) exposure has been found to be associated with gestational hypertension, which, in turn, increases the risk of term low birth weight (LBW). As such, gestational hypertension acts as a potential mechanism mediating restricted fetal growth; however, few epidemiological studies have quantified this specific mediation pathway. OBJECTIVE: This study aims to examine whether gestational hypertension serves as a mediator of the association between prenatal O3 exposure and term LBW. METHODS: We conducted a population-based cohort study using the Cheeloo Lifespan Electron

Impact of maternal thyroid autoantibodies positivity on the risk of early term birth: Ma'anshan Birth Cohort Study.

PURPOSE: We aim to investigate associations of maternal serum anti-thyroperoxidase autoantibody (TPOAb) with duration of gestation. We aim to investigate whether maternal TPOAb positivity is associated with the risk of premature or early term birth. METHODS: This was a prospective birth cohort study performed in an iodine sufficient area of China. Serum samples were collected from 2931 women at both the first and second trimesters of pregnancy. Thyrotropin (TSH), free thyroxine (FT4), and TPOAb levels were measured. Data on gestational age at birth was obtained from delivery records. RESULTS

Exhaled carbon dioxide in healthy term infants immediately after birth.

OBJECTIVE: To measure exhaled carbon dioxide (ECO2) in term infants immediately after birth. STUDY DESIGN: Infants >37 weeks gestation born at The Royal Women's Hospital, Melbourne, Australia were eligible. A combined flow sensor and mainstream carbon dioxide (CO2) analyzer was placed in series proximal to a facemask to measure ECO2 and tidal volumes in the first 120 seconds after birth. RESULTS: Term infants (n = 20) with a mean (SD) birth weight of 2976 (697) g and gestational age of 38 (2) weeks were included. Infants took a median (range) 3 (1-8) breaths before ECO2 was detected. Cardiovascular risk factors at age 30 following pre-term birth.

BACKGROUND: Recent epidemiological evidence has shown increased rates of cardiovascular mortality and associated risk factors in those born small. However, scarce information exists concerning cardiovascular risk factors in adulthood following pre-term birth, or distinguishing the relative contributions of length of gestation and fetal growth to small size at birth. METHODS: Prospective follow-up of 458 30-year-olds whose mothers took part in a randomized controlled trial of antenatal betamethasone for the prevention of neonatal respiratory distress syndrome (147 born at term, 311 born pre-te

Outdoor air pollution and term low birth weight in Japan.

INTRODUCTION: Evidence has accumulated on the association between ambient air pollution and adverse birth outcomes. However, most of the previous studies were conducted in geographically distinct areas and suffer from lack of important potential covariates. We examined the effect of ambient air pollution on term low birth weight (LBW) using data from a nationwide population-based longitudinal survey in Japan that began in 2001. METHODS: We restricted participants to term singletons (n=44,109). Air pollution concentrations during the 9months before birth were obtained at the municipality level

Long term respiratory outcomes of very premature birth (<32 weeks).

Many very prematurely born infants develop bronchopulmonary dysplasia (BPD), remaining oxygen dependent for many months and requiring frequent rehospitalisations. Troublesome, recurrent respiratory symptoms requiring treatment and lung function abnormalities at follow-up are common. The most severely affected may remain symptomatic with evidence of airways obstruction even as adults. Data from adolescents and adults on the respiratory outcome of extreme prematurity, however, are usually from patients who have had 'classical' BPD with severe respiratory failure in the neonatal period. Nowadays,
