Simulation training in cesarean delivery.

Surgical training has always been an essential element of obstetrics. Skills acquisition for cesarean delivery relies heavily on apprenticeship-style training, with a notable paucity of formal simulation training before taking an active role in surgery. Patient safety is compromised when surgeons possess poor surgical skills and inadequate knowledge to handle acute emergencies. Experience comes with "real-world" exposure or can be obtained in a simulated setting, where scenarios can be practiced without the risk of patient harm. As part of a larger Intensive Course in Obstetric Emergencies, a

The relationship between grit and selected personality measures in medical students.

OBJECTIVES: To test the hypothesis that scores on a Grit scale are positively associated with personality measures that are conducive to relationship building (Empathy, Self-Esteem, Activity, and Sociability), but inversely associated with personality measures that are detrimental to interpersonal relationships (Neuroticism-Anxiety, Aggression-Hostility, Impulsive Sensation Seeking, and Loneliness). METHODS: Convenient sampling was used that included 241 medical students at Sidney Kimmel Medical College at Thomas Jefferson University who participated in this ex post facto research. Validated

[For a communication addressing populations' health literacy].

The concept of "health literacy" has been widely used in English-speaking countries for about 20 years. However, its meaning has evolved since its first definition as "cognitive and social skills which determine the motivation and ability of individuals to gain access to, understand and use information in ways which promote and maintain good health", to be most often centered on the narrower meaning of "functional health literacy", which is the ability to read and understand written medical documents. This narrow definition excludes illiterate populations and don't take into consideration the

Reporting Skill: The Missing Ingredient in Concussion Reporting Intention Assessment.

BACKGROUND: Extant literature suggests that a substantial portion of athletes may not report a possible concussion and that concussion knowledge is insufficient to predict concussion reporting behavior. One area that has not been explored is reporting skill; that is, mastery of the actions required to report a concussion. This study evaluated the relationship between reporting skill and reporting intention, introducing a measure of the reporting skill construct. HYPOTHESES: Reporting intentions will be more closely associated with reporting skill than with concussion/symptom knowledge. The re

"They recognize me as a doctor": A peer mobilisation training programme to promote oral HIV self-testing and referral for acute HIV infection screening among gay and bisexual men and transgender women in coastal Kenya, an exploratory study.

BACKGROUND: Targeted peer mobilisation can improve access to HIV testing and care and may impact onward HIV transmission. We describe a qualitative exploration of the experience with a peer mobilisation training programme for oral HIV self-testing (OST) and referral for acute HIV infection (AHI) testing among gay and bisexual men (GBMSM) and transgender women (TGW) in coastal Kenya. METHODS: The training programme covered five modules: 1) safe sex, 2) OST, 3) AHI, 4) HIV partner notification services, and 5) mobilisation skills. Mobilisers attended two training sessions and weekly meetings be

Correlates of HIV/AIDS problem behaviors and incarceration status among inmates in Georgia.

This study examines the relationship between health self-efficacy, measures of sexual risk taking, health communication and negotiation skills, condom use, and substance use behaviors with history of prior incarceration among a sample of inmates in Georgia. The analysis herein is based on the completed survey responses from 188 inmates. The t test or simple chi-square tests were employed to examine differences in participant response means based on prior incarceration status and responses regarding HIV/AIDS problem behaviors for the entire population. The only statistically significant differe

Interaction of feedback frequency and task difficulty in children's motor skill learning.

BACKGROUND: Providing adults with knowledge of results (KR) after each practice trial (100% KR) usually is found to be detrimental to motor skill learning compared with conditions in which feedback is less frequently provided. The effect of 100% KR on children's learning is less clear, with research showing that children with cerebral palsy benefit from less frequent KR, whereas children with typical development do not. OBJECTIVE: This study was designed to examine the interaction of KR frequency and task complexity on the acquisition, retention, and transfer of a novel throwing skill in four

Is preschool language impairment a risk factor for dyslexia in adolescence?

The literacy skills of 56 school leavers from the Bishop and Edmundson (1987) cohort of preschoolers with specific language impairment (SLI) were assessed at 15 years. The SLI group performed worse on tests of reading, spelling, and reading comprehension than age-matched controls and the literacy outcomes were particularly poor for those with Performance IQ less than 100. The rate of specific reading retardation in the SLI group had increased between the ages of 8 1/2 and 15 years and there had been a substantial drop in reading accuracy, relative to age. However, over 35% had reading skills w

[Clinical reasoning and decision making in the practice. A woman with abdominal complaints].

A female patient aged 28 years suffered from recurrent episodes of diarrhoea. Giardia lamblia was isolated once. By rigorous application of basic clinical skills, such as thorough history taking and physical examination, together with laboratory tests the correct diagnosis was established (in this patient systemic lupus erythematosus). Two important aspects of clinical reasoning, viz. returning to the patient when new information becomes available (cyclic way of working) and pattern recognition ensured a fruitful diagnostic process.

Effect of ethylic alcohol on attentive functions involved in driving abilities.

The burden of injuries due to drunk drivers has been estimated only indirectly. Indeed, alcohol is considered one of the most important contributing cause of car crash injuries and its effect on cognitive functions needs to be better elucidated. Aims of the study were i) to examine the effect of alcohol on attentive abilities involved while driving, and ii) to investigate whether Italian law limits for safe driving are sufficiently accurate to prevent risky behaviours and car crash risk while driving. We conducted a cross-over study at IRCCS Fondazione Santa Lucia Rehabilitation Hospital in Ro

Feasibility and Preliminary Efficacy of Empowered Relief in Patients With Chronic Pain Taking Methadone or Buprenorphine: Single-Arm National Pilot Study.

BACKGROUND: Approximately 45% of individuals taking methadone or buprenorphine have chronic pain. These medications are commonly prescribed for chronic pain or opioid use disorder (OUD). To optimize pain management as well as reduce opioid-related symptoms (eg, craving) and risks (misuse and overdose), there is a critical need for a brief, effective, and accessible pain skills intervention for this population. OBJECTIVE: This single-arm study aimed to examine the feasibility and preliminary efficacy of online Empowered Relief (ER), a 1-session pain relief skills class, for individuals with ch

A novel educational strategy to enhance internal medicine residents' familial colorectal cancer knowledge and risk assessment skills.

BACKGROUND: Internal medicine residents are deficient in their knowledge about familial colorectal cancer (CRC) and thus unable to comply with appropriate screening guidelines. The objective of this study was to evaluate the effectiveness of a mixed educational program that incorporates both a didactic lecture (DL) and interactive, case-based seminar (ICBS), plus distribution of a personal digital assistant (PDA)-based risk assessment tool. METHODS: Internal medicine resident continuity care teams were randomly assigned to an intervention (9 teams; 43 residents) or control (11 teams; 39 resid

Association between FT3 Levels and Exercise-Induced Cardiac Remodeling in Elite Athletes.

BACKGROUND: Previous studies demonstrated that variations of fT3, even within the euthyroid range, can influence cardiac function. Our aim was to investigate whether thyroid hormones, even within the euthyroid range, are associated with the magnitude of exercise-induced cardiac remodeling in Olympic athletes. METHODS: We evaluated 1342 Olympic athletes (mean age 25.6 ± 5.1) practicing different sporting disciplines (power, skills, endurance, and mixed). Athletes underwent blood testing (thyroid stimulating hormone, fT3, and fT4), echocardiography, and exercise-stress testing. Athletes taking

Accounting for Intraindividual Profiles in the Wechsler Intelligence Scales Improves the Prediction of School Performance.

IQ scores are often used to predict school performance. However, for children with learning disabilities, the predictive validity of IQ scores appears to be low. In the fourth and fifth versions of the Wechsler Intelligence Scales for Children (WISC), new ancillary indices were introduced. The General Ability Index (GAI) is a broad measure of fluid reasoning with verbal, visual-spatial, and figural test items. By contrast, the Cognitive Proficiency Index (CPI) combines different executive functions known to be frequently affected in children with dyslexia, ADHD, or combined learning disorders.

A Study of Knowledge and Recognition on Postoperative Delirium Among Postanesthesia Care Unit (PACU) Nurses.

PURPOSE: Postoperative delirium (POD) is a syndrome that often occurs after surgery. Nursing staff are the first-line personnel who have the most time and close contact with postoperative patients. If delirium is not recognized immediately, it will affect the patient's prognosis and increase clinical care requirements and health care costs. This study explores postanesthesia care unit (PACU) nursing staff's knowledge and ability to recognize POD in patients. DESIGN: A cross-sectional study was conducted with purposive sampling to recruit nurses from hospital PACUs in central and southern Taiw

Persistence of literacy problems: spelling in adolescence and at mid-life.

BACKGROUND: Developmental reading problems show strong persistence across the school years; less is known about poor readers' later progress in literacy skills. METHOD: Poor (n = 42) and normally developing readers (n = 86) tested in adolescence (ages 14/15 years) in the Isle of Wight epidemiological studies were re-contacted at mid-life (ages 44/45 years). Participants completed a spelling test, and reported on educational qualifications, perceived adult spelling competence, and problems in day-to-day literacy tasks. RESULTS: Individual differences in spelling were highly persistent across
