Deutetrabenazine (Austedo, Teva Pharmaceuticals) is a deuterated form of tetrabenazine. It is the first deuterated drug to receive US regulatory approval and is approved for treatment of chorea in Huntington's disease and tardive dyskinesia. Two oral single dose studies comparing deutetrabenazine (25 mg) with tetrabenazine (25 mg) in healthy volunteers evaluated the impact of deuteration on pharmacokinetics of the active metabolites, alpha-dihydrotetrabenazine (α-HTBZ) and beta-dihydrotetrabenazine (β-HTBZ), metabolite profile, safety, and tolerability. In the two-way, cross-over study, the me

Hyperkinetic movement disorders after routine cardiac procedures are rare yet disabling and often misdiagnosed. We report the case of a 76-year-old man who developed abrupt, generalized choreo-ballism with severe akathisia one day after coronary angiography. Metabolic and inflammatory serum studies were unremarkable, brain magnetic resonance imaging (MRI) demonstrated no acute causative lesion, including no basal ganglia abnormality, and cerebrospinal fluid (CSF) analysis was non-inflammatory with negative infectious, autoimmune, and paraneoplastic testing. Low-dose tetrabenazine produced rapi

Physical activities can have intrinsic motivational or reinforcing properties. The choice to engage in voluntary physical activity is undertaken in relation to the selection of other alternatives, such as sedentary behaviors, drugs, or food intake. The mesolimbic dopamine (DA) system plays a critical role in behavioral activation or exertion of effort, and DA antagonism or depletion induces anergia in effort-based decision-making tasks. However, little is known about the neural mechanisms underlying the decision-making processes that establish preferences for sedentary vs. activity-based reinf

Vesicular monoamine transporter 2 (VMAT2) transports monoamines into storage vesicles in a process that involves exchange of the charged monoamine with two protons. VMAT2 is a member of the DHA12 family of multidrug transporters that belongs to the major facilitator superfamily of secondary transporters. Tetrabenazine (TBZ) is a non-competitive inhibitor of VMAT2 that is used in the treatment of hyperkinetic disorders associated with Huntington disease and Tourette syndrome. Previous biochemical studies suggested that the recognition site for TBZ and monoamines is different. However, the preci

The possibility that hypothalamic noradrenaline (NA) plays a role in the formation of gastric ulcers under stress via ventral bundles (VB) was examined in rats after bilateral destruction of VB. After a 3-h restraint stress ulcerative changes were observed in the stomach of sham-operated rats. NA contents in the hypothalamus and cerebral cortex decreased as compared to the respective contents before stress. The VB-lesioning inhibited the stress-induced decrease of NA in the hypothalamus. Ulcer indices in the animals 1 or 7 days after the VB-lesioning, however, did not differ from those of cont

Huntington's disease (HD) is a hereditary neurodegenerative disorder that causes chorea and motor dysfunction due to a mutation in the Huntingtin (HTT) gene. Tetrabenazine (TBZ) is used to treat HD-related chorea, but its efficacy and safety require further investigation. This systematic review aims to assess the efficacy and safety of TBZ in reducing chorea and improving motor function in HD patients. A comprehensive search was conducted across multiple sources, including PubMed, PubMed Central, Cochrane Library, Wiley Library, and Google Scholar. Medical subject heading (MeSH) terms were use

INTRODUCTION: Huntington's chorea (HC) is commonly managed with neuroleptic medications, though there is little evidence to support their use. This study aimed to perform a real-world comparison of the efficacy of risperidone and olanzapine to tetrabenazine (TBZ) for HC.
METHODS: The Enroll-HD database was used to perform a propensity score-matched comparison of risperidone and olanzapine to TBZ, regarding their efficacy in controlling chorea. Participants with motor manifest Huntington's disease (HD) were grouped according to their use of risperidone, olanzapine, or TBZ. For the three groups

AIMS: Dopamine replacement therapy is the backbone of Parkinson's disease (PD) treatment. However, long-term levodopa (L-DOPA) administration can lead to the severely disabling motor complication L-DOPA-induced dyskinesia (LID), for which standard, effective therapy is currently lacking. This study was conducted to characterize the distinct neural electrophysiological patterns involved in the progression of PD and to examine the efficacy of tetrabenazine, a vesicular monoamine transporter-2 inhibitor, in alleviating dyskinesia and its underlying electrophysiological mechanism.

INTRODUCTION: Chorea, a common clinical manifestation of Huntington's disease (HD), involves sudden, involuntary movements that interfere with daily functioning and contribute to the morbidity of HD. Tetrabenazine and deutetrabenazine are FDA-approved to treat chorea associated with HD. Compared to tetrabenazine, deutetrabenazine has a unique pharmacokinetic profile leading to more consistent systemic exposure, less frequent dosing, and a potentially more favorable safety/tolerability profile. Real-world adherence data for these medications are limited. Here, we evaluate real-world adherence p

Tetrabenazine is used in the treatment of chorea, tardive dyskinesia, tics, and dystonia. It rarely causes acute eyeball dystonia and the description of this complication in Gilles de la Tourette syndrome is limited. We provide a description of an acute oculogyric crisis caused by tetrabenazine in a patient with severe tics. The patient had never developed acute dystonic reactions, although he was previously exposed to numerous dopamine receptor-blocking agents. After 8 days of therapy with tetrabenazine at a dose of 62.5 mg daily, the patient developed involuntary movement of the eyeballs. Wi

BACKGROUND: Tetrabenazine is a monoamine depleter with a well-documented effect against chorea associated with Huntington disease (HD). There is a paucity of data about how reduction in chorea relates to better performance on motor, gait, cognitive, and psychiatric assessments.
METHODS: We designed an open label tetrabenazine withdrawal study to test performance using validated scales. The following instruments were used to assess cognitive, behavioral, and motor function in 10 patients with documented HD: The Montreal Cognitive Assessment, Beck Depression Inventory II, Dynamic Gait Index (DG

INTRODUCTION: Chorea and behavioral symptoms in Huntington's disease (HD) have been treated with neuroleptics and related drugs for over 50 years, yet direct randomized comparisons are scarce. This study assessed the efficacy and safety of olanzapine, tetrabenazine, and tiapride in HD, with the Independence Scale as the primary outcome and components of the Unified Huntington's Disease Rating Scale as secondary outcomes.
METHODS: We conducted a pragmatic, randomized, open-label trial across 11 centers of the French-Speaking Huntington's Network, enrolling 179 patients with Huntington's diseas

PURPOSE: Chorea is a key motor feature of Huntington's disease (HD). In Germany, Austria, and Switzerland, tiapride and tetrabenazine are established and formally approved for symptomatic management of chorea in HD. However, long-term real-world comparative data are lacking, and early documentation is often incomplete. The purpose of this study was to analyze prescription patterns, tolerability, and reasons for discontinuation of both medications in a retrospective real-world HD cohort and to examine outcomes of reinitiation and combination therapy.
METHODS: We retrospectively reviewed medica

BACKGROUND: Radiotherapy is a very common treatment method for various cancers; however, it is not effective for patients with radioresistance. Accordingly, the discovery of drugs for patients with radioresistance cancer is critical. This study used a Food and Drug Administration (FDA)-approved drug library to identify candidate drugs for the treatment of radioresistant colorectal cancer (CRC). This approach to drug development benefits from its low cost and time requirements and can lead to rapid clinical translation.
METHODS: Drugs that suppress radioresistance in CRC cells were screened. E

Tetrabenazine (TBZ) is prescribed for the treatment of chorea associated with Huntington's disease. Via inhibition of the vesicular monoamine transporter (VMAT-2), TBZ blocks dopamine (DA) storage and depletes striatal DA; this drug also has been shown to induce Parkinsonian motor side effects in patients. Recently, TBZ was shown to induce tremulous jaw movements (TJMs) in rats and mice. TJMs are an oral tremor that has many of the characteristics of Parkinsonian tremor in humans. The present study focused upon the ability of the well-established antiparkinsonian agent deprenyl to attenuate th

A new capillary electrophoresis method for the enantioselective analysis of cis- and trans- dihydrotetrabenazine (diHTBZ) after in vitro metabolism by human liver microsomes (HLMs) was developed. The chiral electrophoretic separations were performed by using tris-phosphate buffer (pH 2.5) containing 1% (w/v) carboxymethyl-β-CD as background electrolyte with an applied voltage of +15kV and capillary temperature kept at 15°C. Dispersive liquid-liquid microextraction was employed to extract the analytes from HLMs. Dichloromethane was used as extraction solvent (75μL) and acetone as disperser solv
