This is a cross-sectional study of 227 cocaine dependent individuals in six different therapeutic communities (TCs) within a single treatment network in Andalusia (Spain). The primary aim of the study is to examine the prevalence of lifetime psychiatric comorbidity in this sample using the Psychiatric Research Interview for Substance and Mental Disorders (PRISM). Diagnoses were assessed 15-20 days after admission. The data indicate that more than 65% of the sample experienced a lifetime co-occurring psychiatric comorbidity. Substance-induced mood (21.6%) and psychotic (11.5%) disorders were mo

INTRODUCTION AND AIMS: Odyssey House Victoria's Therapeutic Community (TC) accepts substance-dependent clients, including those with co-occurring mental health issues. American data suggest that TCs are effective in rehabilitating such clients; however, Australian research is limited. The aim of the study was to examine Minnesota Multiphasic Personality Inventory-2 (MMPI-2) profiles of Odyssey House TC residents early in their residency and again after 4 months, to chart changes in MMPI-2 profiles and compare them with norms for psychological health.
DESIGN AND METHODS: The sample comprised 3

Three different groups of therapy leavers and current residents (total n = 80) from Dovegate Prison's Therapeutic Community (TC) were compared on various demographic, criminogenic and psychometric attributes. Statistically significant findings distinguished the leaver groups from current residents and differentiated between type of leavers. Those leavers judged as unsuitable for therapy were characterised by high scores of Psychopathy, Neuroticism, Criminality, Addiction, Venturesomeness, and Avoidant and Schizoid Personality Disorders. Those leavers who had elected to leave the TC were more l

Reflecting on early-career experiences working with patients with severe personality disorders in a therapeutic community, this article explores the counter-transference challenges faced by novice clinicians. I discuss how intense patient projections can activate unresolved conflicts within therapists, often influencing their clinical responses. Countertransference is examined as a co-created experience shaped by both therapist and patient dynamics. Emphasizing the importance of self-awareness, personal analysis, and supportive environments, this perspective offers insights to help early-caree

BACKGROUND: The Therapeutic Community (TC) is a common treatment modality for incarcerated individuals with substance use disorders. TCs rely on peer group processes to promote lasting behavioral and identity change, yet prior research has not adequately tested the peer influence mechanisms underlying the theoretical model. This study applied dynamic network analysis to estimate peer influence processes central to TC philosophy.
METHODS: A stochastic actor-oriented model (SAOM) was applied to ten months of social network data collected from prisoner surveys within a TC unit (N = 62) in a medi

The objective of this study is to explore the stability of personality dimensions in a selection of psychoactive substance abuse patients during treatment in therapeutic communities. The baseline assessment was conducted 15 to 20 days from the start of therapeutic community treatment, and the second assessment was conducted 3 months after admission. The Millon Clinical Multiaxial Inventory III was used to assess personality dimensions. At 3 months, a reduction was observed in the mean severity scores of most personality dimensions in both patterns and clinical syndromes. Caution should be exer

Despite several targeted antiviral drugs against SARS-CoV-2 currently being available, the application of type I interferons (IFNs) still deserves attention as an alternative antiviral strategy. This study aimed to assess the therapeutic effectiveness of IFN-α in hospitalized patients with COVID-19-associated pneumonia. The prospective cohort study included 130 adult patients with coronavirus disease (COVID-19). A dose of 80,000 IU of IFN-α2b was administered daily intranasally for 10 days. Adding IFN-α2b to standard therapy reduces the length of the hospital stay by 3 days (p < 0.001). The le

This article explores some of the difficulties inherent in managing risk in those who have committed sexual or other violent offences and how this may impact on their ability to re-integrate into the community. It discusses the work undertaken at HMP Grendon, a prison-based therapeutic community (TC), to address these difficulties and help residents develop a pro-social lifestyle starting in custody and continuing on release. The article aims to describe how the TC model offers a containing and adaptive environment, and how this can provide opportunities to address offence-related behaviour, w

BACKGROUND: New therapies that could modify the disease course of Huntington's disease (HD) are entering clinical trials. However, conceptions about clinical research from the HD community are unknown. This knowledge could help inform patient-clinician discussions surrounding clinical trial participation.
OBJECTIVE: The purpose of this study was to assess clinical trial attitudes and understanding in the HD community.
METHODS: We developed a survey incorporating two measures of trial understanding and attitudes and the impact of therapeutic route of administration on hypothetical trial parti

Background: Community-acquired pneumonia (CAP) remains a leading cause of pediatric morbidity and mortality worldwide, particularly in low-resource settings. Although antimicrobial therapy is the cornerstone of treatment, disease severity is often influenced not only by pathogen burden but also by dysregulated host inflammation. Systemic corticosteroids have therefore been investigated as adjunctive agents to reduce inflammation-mediated lung injury, but their role in children remains controversial. Methods: This narrative review summarizes current evidence on systemic corticosteroid use in pe

Harm is a concept that permeates behavioral and public health discourses on addiction. Examining addiction recovery services in settings beyond the OECD led me to the question: What does harm mean in an un-urban, un-Western, and un-democratic space? While some emphasize the human rights potential of reducing harm, others speak to the violence of cure. My ethnographic research in a Therapeutic Community (TC) for drug treatment in Southwest China pushed me to consider how the potential for reducing the harms of illegal substance use balance with the complex psychological demands of cure. The all

Data from naturalistic samples provide an important complement to findings from randomized trials of psychotherapy. A random national sample of US clinicians provided data on 145 completed treatments of patients with bulimic symptoms. We attempted to characterize the nature of treatments in the community and to examine the relation between treatment variables and outcome. Clinicians of all theoretical orientations report using interventions with polysymptomatic cases designed to address clinically significant personality characteristics and interpersonal patterns. Whereas cognitive-behavioral
