Cardiovascular and non-cardiovascular concerns with proton pump inhibitors: Are they safe?

The introduction of proton pump inhibitors (PPIs) has been a cornerstone in the treatment of acid-related disorders, such as gastroesophageal reflux and peptic ulcer, and their use has increased rapidly during the last decades. Being highly lipophilic drugs, they may potentially affect several pathophysiological pathways involved in cardiovascular and kidney morbidity, immune response and infections, absorption of selected nutrients, bone metabolism and cognitive function. Clinical epidemiology evidence coming from well-designed analyses of observational data consistently reported that long-te

Cangrelor: review of the drug and the CHAMPION programme (including PHOENIX).

Platelet inhibition is the main goal of ancillary pharmacologic therapy during percutaneous coronary interventions (PCI). Thienopyridines and ticagrelor are oral drugs developed for this purpose. Cangrelor is an intravenous, non-thienopyridine antagonist of the P2Y12 receptor with a rapid, potent, predictable, and quickly reversible effect. Cangrelor has been studied in a broad population intended to receive PCI in the CHAMPION program, where it was compared with different clopidogrel regimens. The first two trials, CHAMPION PCI and PLATFORM, failed their primary objective, likely for challeng

Use of antiplatelet therapy after percutaneous coronary intervention with bare-metal stents and different types of drug-eluting stents.

Dual antiplatelet therapy (DAPT) with a thienopyridine and aspirin has been the standard of care post coronary stent implantation. DAPT has been shown to reduce the risk of stent thrombosis (ST) and complications of myocardial infarction and death after placement of a drug-eluting stent (DES) and bare-metal stent (BMS). This article reviews the available clinical efficacy and safety data of antiplatelet therapies. The aim of this review is to highlight not only the importance of antiplatelets in the prevention of early and late thrombosis but also emphasize the importance of newer more potent

Rash with both clopidogrel and ticlopidine in two patients following percutaneous coronary intervention with drug-eluting stents.

OBJECTIVE: To describe 2 cases of rash that occurred following oral administration of both clopidogrel and ticlopidine and to review previously published case reports. CASE SUMMARY: Two patients developed maculopapular pruritic rashes that began on the abdomen and spread to the back, neck, and face following clopidogrel administration after placement of a drug-eluting intracoronary stent. Following recurrence of the rash after ticlopidine was initiated, thienopyridines were discontinued, and the patients were treated for 3-6 months with aspirin, cilostazol, and enoxaparin or warfarin for prev

Ticagrelor for Refractory Migraine/Patent Foramen Ovale (TRACTOR): An open-label pilot study.

OBJECTIVE: After finding that the thienopyridines clopidogrel and prasugrel reduced migraine headache (MHA) symptoms in some patients with patent foramen ovale (PFO), this small pilot study was undertaken to determine whether ticagrelor, a nonthienopyridine P2Y12 inhibitor, would have similar MHA effects and might be better suited for a future randomized trial. METHODS: MHA patients were screened for PFO. Participants with documented right to left shunt (RLS) and ≥6 monthly MHA days received ticagrelor therapy for 28 days. Those with ≥50% reduction in monthly MHA days were deemed responders a

Antiplatelet drug interactions.

Both laboratory studies in healthy volunteers and clinical studies have suggested adverse interactions between antiplatelet drugs and other commonly used medications. Interactions described include those between aspirin and ibuprofen, aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs), and the thienopyridine, clopidogrel, and drugs inhibiting CYP2C19, notably the proton pump inhibitors (PPI) omeprazole and esomeprazole. Other interactions between thienopyridines and CYP3A4/5 have also been reported for statins and calcium channel blockers. The ibuprofen/aspirin interaction is thou

The potential role of prasugrel in secondary prevention of ischemic events in patients with acute coronary syndromes.

Acute coronary syndromes (ACS) are life-threatening manifestations of coronary artery disease, occurring when a thrombus forms at the site of atherosclerotic plaque rupture or fissuring. Almost all patients discharged from the hospital after an ACS (myocardial infarction or unstable angina) in the United States receive antiplatelet therapy. Current recommendations for post-ACS antiplatelet therapy are aspirin 75 to 162 mg/day indefinitely for all patients, plus a thienopyridine (currently clopidogrel 75 mg/day) for > or = 12 months in those receiving stents unless there is a high risk of bleed

Meta-Analysis of the Role of Cangrelor for Patients Undergoing Percutaneous Coronary Intervention.

Inhibition of the P2Y12 receptor by an oral P2Y12 inhibitor with loading doses along with Cyclooxygenase-1 inhibition by aspirin is considered a first-line treatment strategy in patients with the acute coronary syndrome and patients undergoing percutaneous coronary intervention (PCI). Limitations associated with oral P2Y12 receptor inhibitors include a requirement for in vivo conversion (thienopyridines), delayed onset of action, suboptimal inhibition, irreversible inhibition (thienopyridines), and delayed offset. In the acute setting, therapy with potent platelet inhibitors that have a fast o

A post-sulfonated one-pot synthesized magnetic cellulose nanocomposite for Knoevenagel and Thorpe-Ziegler reactions.

The development of biodegradable and active cellulosic-based heterogeneous catalysts for the synthesis of different organic compounds would be attractive in pharmaceutical and petrochemical-related industries. Herein, a post-sulfonated composite of one-pot synthesized magnetite (Fe3O4) and cellulose nanocrystals (CNCs) was used as an effective and easily separable heterogeneous catalyst for activating the Knoevenagel and Thorpe-Ziegler reactions. The composite was developed hydrothermally from microcrystalline cellulose (MCC), iron chlorides, urea, and hydrochloric acid at 180 °C for 20 h in a

[Update on antithrombotic treatments in arterial diseases].

In most cases, arterial diseases are characterized by atherothrombosis causing organ or limb ischemia. Antithrombotic agents are a cornerstone in the therapeutic management of this pathology. Present approaches include anticoagulants and four types of antiplatelet drugs: aspirin, thienopyridines, dipyridamole and the anti-GPIIbIIIa molecules. Several questions are still pending, in particular concerning treatment tailoring according to clinical presentation and the duration of the antiplatelet treatment. This review focuses on the indications of the currently available antithrombotic agents in

Haemorrhagic stroke during anti-platelet therapy.

Drugs that inhibit platelet function are widely used to decrease the risk of occlusive arterial events in patients with atherosclerosis. There are three families of anti-platelet agents with proven clinical efficacy: (1) cyclo-oxygenase inhibitors, such as aspirin; (2) adenosine diphosphate receptor antagonists, such as the thienopyridine compounds ticlopidine and clopidogrel; and (3) glycoprotein IIb/IIIa antagonists. All these drugs are used during coronary interventions and in the medical management of acute coronary syndromes, while only aspirin and thienopyridine compounds are used in the

Antithrombotic treatment in patients undergoing transcatheter aortic valve implantation (TAVI).

Transcatheter aortic valve implantation (TAVI) is an established treatment option for symptomatic patients with severe aortic valvular disease who are not suitable for conventional surgical aortic valve replacement. Despite improving experience and techniques, ischaemic and bleeding complications after TAVI remain prevalent and impair survival in this generally old and comorbid-rich population. Due to changing aetiology of complications over time, antiplatelet and anticoagulant therapy after TAVI should be carefully balanced. Empirically, a dual antiplatelet strategy is generally used after TA

Clopidogrel in acute coronary syndromes (unstable angina and non-Q-wave myocardial infarction).

Given the importance of thrombosis in acute coronary syndromes, antithrombotic therapy has become standard treatment for these conditions. This article reviews the mechanism of action and the major evidence supporting the clinical use of clopidogrel, a potent antiplatelet agent of the thienopyridines class, focusing on its role in the setting of acute coronary syndromes without persistent ST segment elevation (unstable angina and non-Q wave myocardial infarction). Some unanswered questions relating to this medication are also highlighted. Finally, current updates on clinical guidelines for the

The use of clopidogrel in patients with coronary artery disease.

Coronary thrombosis plays a central role in the development, progression, and complications of atherosclerotic heart disease. As a result, pharmacologic manipulation of the hemostatic system has been the mainstay of treatment for coronary artery disease. Since platelets are the most important cellular element in the development of arterial thrombosis, many of the most effective therapies have involved the use of various antiplatelet agents. This article focuses on clopidogrel, an antiplatelet agent belonging to the class of drugs known as the thienopyridines, in the treatment of patients with

Prasugrel.

Prasugrel is a third-generation thienopyridine which selectively inhibits the platelet P2Y(12) receptor more rapidly, more potently, and with less interindividual response variability compared with the second-generation thienopyridine clopidogrel. Large-scale phase III clinical testing showed that in high-to moderate-risk acute coronary syndrome patients undergoing percutaneous coronary intervention, prasugrel translates into a greater reduction in ischemic events, including stent thrombosis, in the short and long term compared to clopidogrel. Prasugrel, however, is associated with an increase

Discovery and structure-activity relationships study of thieno[2,3-b]pyridine analogues as hepatic gluconeogenesis inhibitors.

Type 2 diabetes mellitus (T2DM) is a chronic, complex and multifactorial metabolic disorder, and targeting gluconeogenesis inhibition is a promising strategy for anti-diabetic drug discovery. This study discovered a new class of thieno[2,3-b]pyridine derivatives as hepatic gluconeogenesis inhibitors. First, a hit compound (DMT: IC50 = 33.8 μM) characterized by a thienopyridine core was identified in a cell-based screening of our privileged small molecule library. Structure activity relationships (SARs) study showed that replaced the CF3 in the thienopyridine core could improve the potency and

Monitoring persistent platelet reactivity in patients with unprotected left main stenting.

OBJECTIVE: This study sought to determine the rate and potential clinical impact of persistent platelet reactivity (PPR) in unprotected left main (ULMD) stenting. BACKGROUND: PPR under aspirin or thienopyridines is associated with acute events after angioplasty. METHODS: We prospectively included 125 patients referred for ULMD stenting. For the first 64 patients (ALMA-1), angioplasty was performed under aspirin and clopidogrel without platelet reactivity assessment. For the last 61 patients (ALMA-2), platelet reactivity was assessed before angioplasty: in patients with aspirin-related PPR, a
