[Hemopleuritis in closed thoracic injuries and multiple trauma].

In examination of 817 patients with closed chest injury and multiple trauma 57% of them were found to have obvious or latent hemopleuritis. According to the character of the primary damage and the developing pathophysiological changes, 3 forms of hemopleuritis were revealed: cardiac, pulmonary, and abdominal. A single complex of symptoms forms finally: pulmonary hypertension, venous stasis in the lungs, right-ventricular cardiac insufficiency. As a result, venous blood escapes through the lymph nodes, mainly those of the right lung, and together with the lymph flows in the dilated lymph vessel

Neurovascular injuries of the thoracic outlet and axilla.

In this retrospective series of 46 patients with neurovascular injury of the thoracic outlet or axilla, our management strategy emphasized prompt recognition of trauma that may be superficially innocuous. Whenever possible, the precise site of vascular injury was identified arteriographically to help plan an incision that would provide proximal vascular control as well as expedient injury exposure. Vascular reconstruction was accomplished with either end-to-end primary anastomosis (n = 17) or autogenous saphenous vein graft interposition (n = 26). No case of ischemic limb loss occurred, althou

Computed tomography of thoracic and lumbar spine injuries.

CT scans of 73 patients with acute thoracic/lumbar spine injuries from T3 to L5 were reviewed. Injuries were classified as burst fractures (48), fracture-dislocations (ten), wedge compression fractures (11), and seatbelt-type injuries (four). Thirty-one (42%) had motor deficits due to spinal cord or nerve root damage. Such neurologic deficits were present in all patients with fracture-dislocations, and 60% of those with burst fractures. Seven patients, four initially normal, developed progressive neurologic impairment early after injury. Burst fractures, one with dislocation, were the spinal i

Thoracic vessel injury.

Thoracic trauma occurs in an estimated 25% of all trauma victims, with approximately 50% of trauma mortalities attributable to these injuries. Only 4% of thoracic traumas involve injuries to thoracic vessels, including the aorta, innominate veins and artery, left carotid artery, internal mammaries, and pulmonary hilar vessels. These vascular injuries can range from minor to major with advances in prehospital management increasing the number of patients surviving in the field and presenting to the hospital with life-threatening vascular injuries. Rapid assessment during the initial survey and i

Decelerational thoracic injury.

BACKGROUND: Among the five major decelerational thoracic injuries [myocardial contusion (MC), traumatic aortic disruption (TAD), sternal fracture (SF), flail chest (FC), and tracheobronchial disruption (TBD)], coexisting injuries are seemingly rare. METHODS: To test this hypothesis, we reviewed the records of all patients, with final diagnosis (FDX) codes of these injuries, treated at our Level I trauma center for the 10 years preceding 1997. RESULTS: Among 142 patients, all victims of motor vehicle crashes, there were 38 MC, 36 TAD, 33 FC, 28 SF, and 7 TBD. There were six coexisting injurie

Evolution of treatment for traumatic thoracic aortic injuries.

OBJECTIVE: To review the evolution of traumatic thoracic aortic injury (TTAI) treatment at a single institution. METHODS: Retrospective analysis of all patients included in an institutional trauma registry and vascular surgery database who underwent treatment of TTAI between January 1999 and January 2011. RESULTS: Ninety-one patients (69 males) were treated for TTAI. The mean age was 38.5 years (range, 16-79 years). Forty-one patients underwent open repair (OR) and 50 thoracic endovascular repair (TEVAR), 37 with thoracic stent grafts (TSG) alone, 11 with infrarenal aortic extender cuffs (AE

Brachial plexus injuries complicating video-assisted thoracic surgery.

OBJECTIVE: Video-assisted thoracic surgery (VATS) is a less-invasive alternative to open thoracotomy. According to evidence-based medicine methodology, VATS is associated with better outcomes and the same complication rate as open thoracotomy. CASE DESCRIPTION: Two women (19 and 21 years old) underwent VATS for treatment of pneumothorax. In an attempt to perform hemostasis, the subclavian arteries had to be occluded. Total sensorimotor deficit in both patients on the side where surgery was performed was noticed postoperatively. After 3½ months in patient 1, nerve roots C5 and C6 were neurolyz

Thoracic spinal injuries: operative treatments and neurologic outcomes.

Between January 1983 and December 1997, 29 patients with either a fracture (11 patients) or a fracture-dislocation (18 patients) of the thoracic spine were treated operatively. All patients underwent posterior decompression and stabilization within a mean time of 4 days after injury (range, 0-45 days). Patients with complete paraplegia had no postoperative improvement in neurologic status, whereas all patients with incomplete spinal cord lesions improved in neurologic status after surgery. There was no significant association between time from injury to operation and final neurologic outcome.

Thoracic vascular injuries: a major problem in trauma.

OBJECTIVE: We present a review of our experience in the management of thoracic vascular injuries during the past 10 years in one of Sweden's busiest emergency departments, with morbidity and mortality as the main outcome measures. DESIGN: Of eight patients who sustained chest trauma with thoracic vascular injuries, six presented with shock. Angiography was the gold standard in diagnosing aortic and subclavian injuries. RESULTS: Lesions of the inferior vena cava, left internal mammary artery, and intercostal vessels were detected only at surgery. Two patients died of exsanguination. Managing

[Rupture of the thoracic aorta and multiple injuries].

Traumatic rupture of the thoracic aorta is an extremely severe injury, and is usually fatal. The injury is most often seen in forceful deceleration trauma. This complicates the priority of diagnosis and treatment in relation to other severe injuries in the same patient. During the period 1984-1991, nine patients underwent operation for this injury at the Department of Thoracic Surgery. Two patients succumbed, because of free rupture in the operating theatre. The others survived. Three of these were treated by directly suturing the rupture, and four by introducing a prosthesis. The prognosis in

[Ambulatory evaluation and treatment of blunt thoracic injuries].

307 patients with chest injuries were treated in an outpatient clinic during a three month period. Ten patients developed complications such as hemothorax, pneumothorax and lung contusion, or late complications such as atelectases and pneumonia. 21 patients were hospitalized after initial evaluation. Two patients died. Pain was a symptom in 306 of the 307 patients. Other symptoms were coughing, hempoptysis, fever, nausea. Complications increased in 40 patients, with other symptoms or signs in addition to pain. These other symptoms had a 40% positive and 95% negative predictive value as regards

Thoracic aortic rupture in patients with multisystem injuries.

This report attempts to delineate management priorities in patients with thoracic aortic tears and severe multisystem trauma. Nineteen patients with a mean age of 40 years were treated after motor vehicle accidents. An average of five major systems were injured and the majority of patients sustained head injury, severe intra-abdominal injuries, and major fractures, in addition to aortic tears. Hypotension was present in 17 patients. Chest roentgenograms showed a widened mediastinum that subsequently led to angiography in 18 of the 19 patients. Of the 11 patients who required both an explorator

[Thoracic injuries in the field of aviation medicine].

In civil and war traumatology, 33% of deaths are due to chest traumas although this involves only 25% of cases. Examination of the physiopathy clinical medicine and treatment of the various traumatic lesions of the chest and their complications shows that rapid transfer of such patients to suitable places of treatment following adequate first aid is indispensable. The use of fully equipped aircraft is thus vital. Transport criteria are dictated by the environment, first aid resources, meterorological conditions, cruising altitude, available equipment [an automatic respirator and aspirator are

Penetrating Thoracic Injury.

This article discusses the role of radiology in evaluating patients with penetrating injuries to the chest. Penetrating injuries to the chest encompass ballistic and nonballistic injuries and can involve superficial soft tissues of the chest wall, lungs and pleura, diaphragm, and mediastinum. The mechanism of injury in ballistic and nonballistic trauma and the impact the injury trajectory has on imaging evaluation of penetrating injuries to the chest are discussed. The article presents the broad spectrum of imaging findings a radiologist encounters with penetrating injuries to the chest, with
