Somatosensory evoked potentials and magnetic resonance imaging in intraspinal neoplasms.

Median and posterior tibial somatosensory evoked potentials (SEPs) were studied on 25 patients with pathologically proven intraspinal neoplasms, and the results were compared and correlated with the details of clinical examination and the information derived from magnetic resonance imaging (MR). MR was abnormal in all cases and in 23 of 25 (92%) demonstrated an intraspinal expansile lesion. SEP was abnormal in 19 of 25 patients (76%). Abnormal SEPs were found in 18 of 19 patients (94%) with cervical or thoracic neoplasms but only in 1 of 6 patients (16%) with the tumor in the thoracolumbar or

Primary mediastinal tumors: part II. Tumors of the middle and posterior mediastinum.

Lymphoma, mediastinal cysts, and neurogenic neoplasms are the most common primary middle and posterior mediastinal tumors. Lymphoma may involve the anterior, middle and/or posterior mediastinum, frequently as lymphadenopathy or as a discrete mass. Foregut cysts are common congenital mediastinal cysts and frequently arise in the middle mediastinum. Pericardial cysts are rare. Schwannoma and neurofibroma are benign peripheral nerve neoplasms, represent the most common mediastinal neurogenic tumors, and rarely degenerate into malignant tumors of nerve sheath origin. Sympathetic ganglia tumors inc

[Diagnosis and surgical treatment of mediastinal neoplasms].

The authors share their experiences with examination and surgical treatment of 163 patients with various neoplasms of the mediastinum. The first place among the neoplasms is occupied by tumors of the lymphatic apparatus (33%), the second--by tumors of the thymus (21%). The video-thoracoscopic technique was used in 17 of 131 operations performed. Complications after the operations took place in 6 patients (4.5%), two patients died (1.5%). A conclusion is made that the patients of this category must be treated not only by thoracic surgeons but also by hematologists, neurologists, neurosurgeons a

Metastatic Endocrine Mucin-Producing Sweat Gland Carcinoma to the Lung: A Case Report.

Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare, low-grade neuroendocrine neoplasm previously believed to be indolent in nature. There have only previously been six reported cases of metastases and none of thoracic structures. This case shows a metastatic EMPSGC in a 72-year-old male with a complex oncologic history and is the first reported case of metastases to the lung. As increased recognition of this entity continues to grow, it is important to consider it as part of the differential in mucinous and/or neuroendocrine neoplasms for appropriate management. This case adds

Thoracic outlet syndrome secondary to a cavernous hemangioma of the first rib.

Bone hemangiomas are slow-growing benign tumors, with rib involvement being exceptionally rare. Thoracic outlet syndrome caused by a bone neoplasm of the first rib is even more uncommon, with only a few cases documented in the literature. We present the case of a patient with a clinical diagnosis of neurogenic thoracic outlet syndrome secondary to a bone tumor of the first rib, treated using a combined approach of video-assisted thoracoscopy and posterior access for resection. To our knowledge, this is the second reported case in the literature of a young man with thoracic outlet syndrome seco

Thoracic-abdominal approach in primary pulmonary synovial sarcoma.

We report a case of primary synovial sarcoma of the lung. The patient was a 32-year-old male who presented with a mass in the right hemithorax invading the peritoneal cavity. The neoplasm was resected through a thoracic-abdominal approach. The patient is doing well 21 months after surgery. Within the last decade thoracic localizations of synovial sarcoma are an emerging histopathological entity thanks to the molecular analysis of the SYT-SSX fusion gene transcript.

Unusual presentation of posterior mediastinal chordoma in a 2-year-old boy.

Chordoma is a rare and slow-growing malignant neoplasm that arises from the embryonic notochord. It is rare to see a thoracic chordoma presenting as a posterior mediastinal mass with pleural seeding in a child. The authors report a chordoma of the thoracic spine with posterior mediastinal extension and pleural seeding in a 2-year-old boy who presented with asymmetric bilateral hand temperature and lower limb paralysis. The clinical course progressed rapidly, resulting in death.

Paraneoplastic pemphigus herpetiformis with IgG antibodies to desmoglein 3 and without mucosal lesions.

BACKGROUND: Pemphigus herpetiformis (PH) is a rare clinical entity that combines the clinical features of dermatitis herpetiformis and the immunopathologic features of pemphigus. The target antigen is usually desmoglein 1, with exceptional cases manifesting autoantibodies against desmoglein 3. More recently, it has been found that many patients with PH also demonstrate autoantibodies against desmocollin. The association of PH with a malignant neoplasm is rare. OBSERVATIONS: We describe a patient with PH and a lung neoplasm. Immunologic studies demonstrated IgG antibodies to desmoglein 3 and t

Molecular Pathology of Well-Differentiated Pulmonary and Thymic Neuroendocrine Tumors: What Do Pathologists Need to Know?

Thoracic (pulmonary and thymic) neuroendocrine tumors are well-differentiated epithelial neuroendocrine neoplasms that are classified into typical and atypical carcinoid tumors based on mitotic index cut offs and presence or absence of necrosis. This classification scheme is of great prognostic value but designed for surgical specimens, only. Deep molecular characterization of thoracic neuroendocrine tumors highlighted their difference with neuroendocrine carcinomas. Neuroendocrine tumors of the lung are characterized by a low mutational burden, and a high prevalence of mutations in chromatin

[CT guided selective pneumomediastinography in thoracic imaging].

We established new approaches for CT guided selective pneumomediastinography; paravertebral, pretracheal and anterior mediastinal approaches. CT would seem to be useful method for assessing the invasion to the neighboring structures in the thoracic neoplasms. But scanty fat in mediastinum often interferes with correct evaluation of mediastinal structures in conventional CT. Pneumomediastinal CT with our methods overcomes the weak point and provides additional information to enable exact diagnosis. The feasibility and security of these methods were confirmed through applying to many cases. Base

Fine-needle aspiration diagnosis of intra-thoracic and intra-abdominal lesions: review of experience in the pediatric age group.

Review of fine-needle aspiration (FNA) smears from 121 pediatric patients with intra-thoracic and intra-abdominal lesions revealed 42 (34.7%) cases of neoplasms, 35 (28.9%) cases of tuberculosis, 12 (9.9%) cases of non-tuberculous inflammations, 4 (3.3%) cases of benign cystic lesions, and 28 (23.1%) inadequate/inconclusive cases. The age of the patients ranged from 20 days to 18 yr. Ultrasound and/or CT study done in 105 cases localized the lesions in following common sites: lungs (19 cases), mediastinum (22 cases), liver (14 cases), intestines (11 cases), and lymph nodes (17 cases). A thoracic benign mesenchymoma in association with hemihypertrophy.

A thoracic benign mesenchymoma was removed from an 18-year old caucasian male who also had hemihypertrophy. The very large benign mesenchymoma which was removed was found to consist of an admixture of cytologically mature vascular spaces, adipose tissue, and smooth muscle. After removal of the tumor, the patient has had no recurrence with one year of follow-up. Patients with hemihypertrophy appear to be at high risk to develop neoplasms. Malignant neoplasms of the adrenal gland, kidney, and liver are most common. It is suggested that patients with hemihypertrophy should be followed periodicall

[Value of computer tomography of the thorax and abdomen in children].

Today CT of the pediatric chest and abdomen is an accepted and established diagnostic modality. Detection of pulmonary metastases and the staging of malignant lymphoma of the mediastinum are main indications for thoracic CT, whereas in other neoplastic mediastinal or thoracic wall lesions, which are much rarer, CT is complementary to very valuable conventional examinations. However the cross-sectional display free of superposition and the possibility of density-measurement are outstanding advantages for both thoracic and abdominal CT. Abdominal extension of malignant lymphoma is accurately det

Giant fibrous tumor misdiagnosed as traumatic hemothorax.

Solitary fibrous tumors of the pleura are neoplasms of the mesenchymal tissue of the pleural mesothelium. The most frequent symptoms are dyspnea, coughing and chest pain. A 45-year-old female patient presented after a thoracic contusion. A radio-opaque image was evidenced on chest X-ray. At the initial hospital, a hemorrhagic fluid was aspirated at thoracocentesis and the patient was transferred to our hospital with diagnosis of traumatic hemothorax. A thoracic CT showed a tumoral formation filling two-thirds of the left hemithorax. The transthoracic biopsy finding was compatible with a fibrou

Thoracic neuroblastoma of the neonate.

Five cases of neonatal thoracic neuroblastoma were either clinically asymptomatic or presented as uncharacteristic respiratory distress. Chest radiography demonstrated a spectrum of abnormalities varying from a lobulated, apical thoracic mass with or without evidence of expansion, to a spindle-shaped paraspinal soft tissue widening, or a homogeneously dense hemithorax. In the newborn a neoplasm in the posterior mediastinum most likely represents a neuroblastoma, either primary or metastatic. In a high percentage of cases the primary tumour as well as possible skeletal metastases have an increa

Complete anterior-posterior minimally invasive thoracoscopic robotic-assisted and posterior tubular approach for resection of thoracic dumbbell tumor.

Thoracic dumbbell tumors are relatively uncommon neoplasms that arise from the neurogenic elements. Surgical resection can be challenging as the tumor involves both the spinal canal and thoracic cavity. Historically, thoracotomy and laminectomy were utilized for the resection of these tumors. Although single-stage removal of such tumors has been described recently, there is no prior description of a total minimally invasive single-stage resection of a thoracic dumbbell ganglioneuroma. The current report describes a completely minimally invasive surgical resection for such a tumor performed usi

Image-guided thoracoscopic resection of thoracic dumbbell nerve sheath tumors.

BACKGROUND: Surgical removal of dumbbell nerve sheath tumors (NSTs) remains challenging because these neoplasms occupy ≥ 2 spinal and extraspinal spaces. The presence of intraspinal extension, tumor dimension, and/or its location within the thoracic cavity have previously made the resection of these types of neoplasms difficult. OBJECTIVE: To describe the feasibility of performing minimally invasive thoracoscopic surgery, as facilitated by an image guidance system (IGS), to achieve gross total resection of select dumbbell NSTs located in the thoracic spine. METHODS: The 3 cases presented her

[A case report of chronic traumatic thoracic aneurysm (thrombotic closure type) which appeared as a mediastinal tumor in preoperative findings].

Chronic traumatic thoracic aneurysm is considered to be an uncommon disease. A case of 59-year-old man who had a history of blunt chest trauma with a traffic accident thirty years ago is reported. He has no symptoms and no signs since the accident. The roentogenogram of his chest showed widening of upper mediastinum, but aortography did not reveal the aneurysmal dilatation of the aortic arch. CT and MRI study showed an inhomogenous mediastinal tumor simulating neoplasm neighboring the aortic arch. At surgery, it was found to be a thoracic semispherical pseudo-aneurysm (thrombotic closure type)
