Thoracic Outlet Syndrome: A Narrative Review.

Thoracic outlet syndrome comprises a group of disorders that result in compression of the brachial plexus and subclavian vessels exiting the thoracic outlet. Symptoms include pain, paresthesia, pallor, and weakness depending upon the compromised structures. While consensus in diagnostic criteria has not yet been established, a thorough patient history, physical exam, and appropriate imaging studies are helpful in diagnosis. General first-line therapy for thoracic outlet syndrome is a conservative treatment, and may include physical therapy, lifestyle modifications, NSAIDs, and injection therap

Contemporary management of thoracic outlet syndrome.

PURPOSE OF REVIEW: The diagnosis and management of thoracic outlet syndrome (TOS) has been surrounded by controversy since this disorder was first recognized. Recent evidence from observational studies has helped us better understand the pathophysiology of different TOS subtypes and guide clinical decision making for this disorder. RECENT FINDINGS: The identification of anatomic anomalies involved with the cause of different TOS subtypes has been made easier by contemporary diagnostic techniques. This includes the injection of neuromuscular blocking agents into anterior scalene muscles to hel

Current management of thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a condition caused by compression of the neurovascular structures leading to the arm passing through the thoracic outlet. There are three distinct types of TOS: neurogenic (95%), venous (4%-5%), and arterial (1%). Treatment algorithms depend on the type of TOS. Although statistically the most common type, neurogenic TOS can often be the most difficult to diagnose and treat. We have good follow-up data indicating that appropriately selected patients benefit from surgical intervention. Arterial and venous TOS often present more urgently with arterial or venous t

Arterial complications of thoracic outlet syndrome.

Arterial complications of thoracic outlet compression have serious potential implications; however, these complications rarely appear. Between 1990 and 2006, prospectively collected data on 27 patients with arterial complications of thoracic outlet syndrome were analyzed. The causes of arterial compression were cervical rib (20 [74.1%]), abnormalities of the first thoracic rib (three [11.1%]), soft tissue anomalies (two [7.4%]), and hypertrophic callus after clavicle fracture (two [7.4%]). In all cases, a combined supraclavicular and infraclavicular approach was used. Decompression was achieve

[The asymptomatic thoracic outlet compression syndrome].

The thoracic outlet compression syndrome has a great number of clinical variations. Arterial and venous perfusion impairment is an associated symptom, nerve irritation can occur with or without vascular problems. The degree of nerve damage ranges from transient irritation to permanent motoric and sensory defects. The lack of space in the supracostoclavicular compartment is the cause for nerve compression. The degree of neural damage depends on the degree and duration of the compression. Anatomic variations between the clavicle and first rib are frequent causes for the TOS: accessory ribs and m

Vascular complications of thoracic outlet syndrome.

Vascular complications of thoracic outlet syndrome are uncommon but may result in significant long-term disability. This report documents a retrospective review of 17 such patients. Ten patients presented with acute onset of upper extremity swelling and axillosubclavian vein thrombosis. One patient presented with chronic, intermittent arm swelling and subclavian vein stenosis. Three patients presented with acute symptoms of upper extremity emboli, and three presented with chronic arm claudication. Cervical ribs were discovered in four patients with arterial symptoms and in no patients with ven

Predicting outcomes in thoracic outlet syndrome.

OBJECTIVE: To test the hypothesis that somatosensory evoked potentials (SEP) and results of arterial photoplethysmography (PPG) predict outcome after surgery for thoracic outlet syndrome. DESIGN: A retrospective case series which correlated surgical outcome with preoperative SEP and PPG results using Fisher's exact test. PPG results were considered abnormal when there was complete loss of arterial pulsation, and SEP results when amplitudes were reduced and latencies delayed. SETTING: Royal Prince Alfred Hospital, between March 1984 and February 1992. PATIENTS: Forty-six consecutive patients

Thoracic outlet-syndrome with vascular complications.

1. In thoracic outlet-syndrome arterial lesions--such as poststenotic aneurysm with or without peripheral arterial emboli--are mostly caused by permanent compression of the vessel. In 48% of cases arterial emboli show to be the first clinical manifestation of a subclavian artery lesion. 2. Venous lesions usually classified as "spontaneous axillar vein thrombosis" are probably initiated by intimal lesions of the axillary vein caused by compressionof this vessel in the costoclavicular space. 3. Unilateral ischemia of fingers or hand--especially in young patients--should be considered first of al

Reoperation for recurrent thoracic outlet syndrome.

Recurrent thoracic outlet syndrome that requires reoperation accounts for 1% of first rib resections. Symptoms in a series of 30 patients were mainly neurological and consisted of pain and paresthesia involving the neck, shoulder, arm, and hand and were severe and unrelenting. Recurrence of symptoms ensued from one month to seven years following initial rib resection, with the majority appearing within the first three months. Nerve conduction velocities were diminished to an average of 51 m per second, well below the normal of 72 m per second. Reoperation was required after a period of extensi

Long thoracic nerve paralysis associated with thoracic outlet syndrome.

Two cases of long thoracic nerve palsy associated with thoracic outlet syndrome are reported. Both patients had abnormal posture, with low-set shoulders and winged scapulae. Clinically there was weakness of the serratus anterior muscle with partial denervotion on electromyography. The diagnosis of thoracic outlet syndrome was based on positive vascular tests and brachial plexus nerve compression symptoms induced by the vascular testing positions. An orthosis that held the shoulder in an elevated position was used in both cases. Complete recovery of shoulder function and relief of the symptoms

[Compression syndrome of the upper thoracic outlet (thoracic outlet syndrome)--report on a 13-year-old girl].

The thoracic-outlet-syndrome, a symptom complex consisting of arterial, venous and neural disorders of the upper extremity, is caused by compression of the neurovascular structures between the clavicle and the first rib. Other reasons are congenital abnormalities like a cervical rib, an abnormal first rib, acquired anatomic lesions like fractures and traumata or functional factors. The younger-and middle-age groups are most commonly involved. The symptoms are depending upon the point at which compression occurs and include pain, paresthesias, numbness, fatigue, pallor, coolness or swelling and

Venous thoracic outlet syndrome and Paget-Schroetter syndrome.

Venous thoracic outlet syndrome represents a relatively rare but important diagnosis in the adolescent population with increasing recognition. Compression of the subclavian vein within the costoclavicular space can lead to episodic venous outlet obstruction in the upper extremity, with edema, rubor and functional symptoms. Over time, cumulative injury and compression can lead to thrombosis of the vein, referred to as "effort thrombosis" or the Paget-Schroetter syndrome. This progression can lead to the need for acute management of the venous thromboembolism, requirement for thoracic outlet dec

Arterial thoracic outlet syndrome in an adolescent.

Arterial thoracic outlet syndrome (aTOS) is rarely diagnosed in children. A 15-year-old boy with recent-onset claudication was found to have aTOS on investigation. Bony abnormality other than the cervical rib might also cause aTOS. Enhanced awareness of aTOS as a cause of vascular symptoms in children is warranted.
