Robot-assisted surgery is rapidly expanding in the field of thoracic surgery, including procedures for lung cancer. However, robot-assisted airway reconstruction-particularly sleeve bronchoplasty-remains a highly challenging surgical procedure. Approximately two decades ago, robot-assisted airway reconstruction was investigated as my doctoral research topic, providing an early foundation for the development of this technique. This article summarizes the evolution of robot-assisted airway reconstruction techniques, from procedures initially developed through animal experimentation to those curr

The function of the respiratory muscles (RM) is affected positively or negatively by a variety of surgical procedures. Cardiac, thoracic and upper abdominal surgery impair the RM function and lead to postoperative complications such as hypoxia, atelectasis, aspiration and infections. Preoperative assessment of RM function is cardinal to avoid or attenuate these complications. Three types of surgical procedures, lung transplantation, lung volume reduction surgery and surgery for obesity have been shown to improve RM function. A mechanism by which these types of operation have shown beneficial e

Iatrogenic complications of thoracic and cardiovascular surgery are relatively uncommon, but contribute to potentially significant patient morbidity and mortality. The incidence of iatrogenic disease reflects the complexity of surgical procedures, including lung resection, esophagectomy, coronary artery bypass grafting, thoracic aorta repair, and cardiac valve replacement. Some iatrogenic complications are minor and common to all procedures, whereas others can be potentially devastating and are associated with precise technical components of specific surgeries. Multimodality imaging plays an i

A complex relationship exists between health care disparities and large databases among the thoracic surgical patient population. Using the example of thoracic malignancies, the ability of investigations leveraging large databases and novel analytical approaches to highlight disparate access to care and discordant outcomes following treatment is illustrated. Large, widely used databases may not be representative of the thoracic surgical patient population as a whole, and caution must be used when interpreting and generalizing results gleaned from such database analyses. Ensuring appropriate re

PURPOSE: Although Damage Control Thoracic Surgery (DCTS) has become a provocative alternative to treat patients with chest injuries who are critically ill and physiologically depleted, the management approaches of chest-packing and the measurement of clinically relevant outcomes are not well established. In this paper, we systematically reviewed the available knowledge and evidence about intra-thoracic packing during DCTS for trauma patients. We furthermore inform on the management approaches, surgical strategies, and mortality associated with this intervention.
METHODS: We identified article

Rib fractures are frequent in trauma patients, being most of them managed on a non-surgical way. However, in selected cases, it is advocated. Chest wall stabilization (CWS) only recently has been best characterized. Available data shows plenty of benefits related to CWS versus non-surgical treatment in selected cases. Even though, it is only performed in a small number of patients according to some national databases. There are lots of topics to define concerning CWS such as the subgroups that benefit most, the time of surgery, which ribs should be stabilized and which incision should be perfo

Evaluation of the patient's medical history and a physical examination are the cornerstones of risk assessment prior to elective surgery and may help to optimize the patient's preoperative medical condition and to guide perioperative management. Whether the performance of additional technical tests (e.g. blood chemistry, ECG, spirometry, chest x-ray) can contribute to a reduction of perioperative risk is often not very well known or controversial. Similarly, there is considerable uncertainty among anaesthesiologists, internists and surgeons with respect to the perioperative management of the p

Thoracic surgical procedures are increasing in recent years, and there are different types of lung resections. Postsurgical complications vary depending on the type of resection and the time elapsed, with imaging techniques being key in the postoperative follow-up. Multidisciplinary management of these patients throughout the perioperative period is essential to ensure an optimal surgical outcome. This pictorial review will review the different thoracic surgical techniques, normal postoperative findings and postsurgical complications.

Sleeve lobectomy remains one of the most challenging procedures in thoracic surgery. To date, reports on robotic-assisted thoracic surgery (RATS) sleeve lobectomy are limited, with most literature focusing on right upper lobe reconstructions. Notably, even Cerfolio's landmark reports do not detail bronchial wedge resection or sleeve lobectomy of the middle and lower lobes. Using a 0° scope with an upward-viewing technique allows for the observation of target bronchi in the middle and lower lobes from a low intrathoracic angle. Although the interlobar pulmonary artery often obstructs the bronch

OBJECTIVES: Thoracic pathologies are important causes of morbidity in solid-organ transplant recipients because of chronic immunosuppression, transplant-related complications, and diverse underlying etiologies. We aimed to evaluate thoracic pathologies requiring surgical or interventional management after solid-organ transplantation, present our clinical outcomes, and propose an anatomical management framework for these complex conditions.
MATERIALS AND METHODS: We conducted a retrospective cohort study at a tertiary referral center of solid-organ transplant recipients who underwent surgical

We propose a systematic review that compares outcomes after intentional coverage of the left subclavian artery during thoracic endovascular repair with and without prior surgical revascularization. According to Preferred Items for Reporting of Systemic Reviews and Meta-Analyses guidelines, PubMed and Embase searches identified studies reporting patients with left subclavian artery coverage during thoracic endovascular repair and their subsequent outcomes. Analyzing a varied patient cohort, there are nonstatistically significant trends that favor left subclavian artery revascularization in prev

Chest trauma is the leading cause of death in people under 40. It is estimated to cause around 140,000 deaths each year. The key aims are to reduce mortality and the impact of associated complications to expedite recovery and to restore patient's conditions. The recognition of lesions through appropriate imaging and early treatment already in the emergency department are fundamental. The majority can be managed in a non-surgical way, but especially after traumatic cardiac arrest, a surgical approach is required. One of the most important surgical procedures is the Emergency Department Thoracot

Infectious complications of open and endovascular procedures for descending thoracic aortic disease are relatively rare, affecting 1% to 6% of treated patients. However, the number of thoracic aortic procedures, especially endovascular, is increasing continuously, and infectious complications involving the graft or endograft have been observed more frequently in recent years. Several causative factors may play a role in thoracic aortic prosthetic infections, including hematogenous seeding, local bacterial translocation, and iatrogenous contamination. In addition, the development of a fistula b

Background The COVID-19 pandemic disrupted global healthcare systems, leading to major shifts in surgical case prioritization. This study examines how thoracic surgical trends changed at a tertiary care hospital in Peshawar, Pakistan, specifically comparing oncologic and non-oncologic procedures, patient demographics, and surgical outcomes. Methods We conducted a retrospective comparative study of major thoracic surgeries performed between December 2018 and July 2021. The study period was divided into pre-pandemic (December 2018-March 2020) and pandemic (April 2020-July 2021) phases. Surgical

Single port thoracic surgery, reduced port surgery and needlescopic surgery attract attention as one of the minimally invasive surgery in thoracic surgery recently. Single port thoracic surgery was advocated by Rocco in 2004, it was reported usefulness of single port thoracic surgery for primary spontaneous pneumothorax. The surgical procedure as single (or reduced) port thoracic surgery is roughly divided into the following. One is operated with instruments inserted from the single extended incision, and the other is operated with instruments punctured without extending incision. It is not ge
