PURPOSE: Historically, a chest tube or drain has been left following a thoracic operation to allow drainage of air or fluid in the postoperative period. OBJECTIVE AND METHODS: In order to clarify the utility of video-assisted thoracoscopic surgery (VATS) for mediastinal disease, we analyzed retrospectively 34 thoracoscopic surgical cases (seven lymph node, nine thymic, eight cystic and 10 cases with solid lesions that excluded thymic diseases) between February 1993 and July 2002.

OBJECTIVE: To evaluate the accuracy of radiological tests and the outcome after thoracoscopic evacuation of retained haemothorax.
DESIGN: Prospective study.
SETTING: Academic trauma centre, USA.
SUBJECTS: Of 703 patients with thoracic injuries, 58 (8%) were evaluated for and 15 (2%) were found to have, retained haemothorax.
INTERVENTIONS: Plain chest radiographs (CXR) and thoracic computed tomograms (CT) 48 hours after admission, and thoracoscopic evacuation within 3-6 days of admission.
MAIN OUTCOME MEASURES: Accuracy of radiological tests, morbidity, and mortality.

Video-assisted thoracic surgery (VATS) has enabled more complex procedures previously requiring thoracotomy to be accomplished in lung cancer management. Subtotal esophagectomy was attempted by right thoracoscopy on 13 patients, 10 having cancer and 3 long caustic stenosis. We report what is probably the first case of para-oesophageal bronchogenic cyst treated by video-thoracoscopy. The patient was a 26-years old woman who presented with dysphagia to solid foods. Secondary spontaneous pneumothorax (SP) is an indication for thoracoscopic exploration and treatment just like primary SP. The same guidelines apply to both. Thoracoscopy was performed in 50 patients with pleural exudates of unclear etiology. Indications for thoracoscopy were: unclear character of accumulation of exudate, ineffective treatment including pleural punctures in case of hydrothorax. Visual examination of the pleura and chest organs supplemented by cyto- and histological study of tissues taken from involved places of the pleura allowed to arrive at an exact diagnosis in 90% of cases.

The increasing use of thoracoscopy performed under local anesthesia has made contributions to the diagnosis of pleural disease with effusion. During the past 7 years, we have performed 100 such thoracoscopy procedures using a flexible fiberoptic bronchoscope. OBJECTIVE: To assess the value of thoracoscopy in undiagnosed pleural effusion.
METHOD: By thoracoscopy, 114 cases of unknown origin pleural effusion were examined. Abnormal pleurae were biopsied in sight of thoracoscopy and compared with blind needle biopsy for pleurae.
