Extracorporeal biventricular support through left mini-thoracotomy.

Herein, we present the case of a 23-year-old woman with fulminant myocarditis that led to severe biventricular failure and cardiogenic shock. Despite high-dose inotropes and mechanical support with venoarterial extracorporeal membrane oxygenation plus and Impella CP, the patient's cardiac function did not recover. After upgrading to extracorporeal left ventricular assist device support, persistent right heart failure prevented weaning off venoarterial extracorporeal membrane oxygenation. Chest computed tomography revealed a leftward ascending aorta, prompting left mini-thoracotomy for extracor

Extracardiac Fontan Operation Through a Right Thoracotomy.

An extracardiac Fontan operation through a right thoracotomy incision is an alternative approach in children who have an ascending aorta that is in close proximity to the sternum, as in aortic atresia or transposed great vessels, and in those who have had prior mediastinitis.

Bilateral staged thoracotomy for multiple lung hydatidosis.

BACKGROUND: Hydatid cyst disease is still a problem in many countries. Surgical removal is currently the generally accepted choice of treatment for lung hydatidosis. However, operating on bilateral widespread lung hydatidosis is still controversial. The aim of this retrospective study was to evaluate the results of surgical treatment in bilateral multiple hydatid disease of the lung. METHODS: In this study, we reviewed our experience in the surgical treatment of 17 (3.7%) patients with bilateral, and at least three, lung hydatid cysts. These 17 patients (8 male, 9 female), with an average age

Societal costs of inappropriate emergency department thoracotomy.

BACKGROUND: Emergency department (ED) thoracotomy can be lifesaving. It can also lead to resource waste and exposure to blood-borne infections. We investigated the frequency with which ED thoracotomy was performed for inappropriate indications and the resulting societal costs. STUDY DESIGN: This retrospective cohort study examined all trauma patients admitted directly from the scene of injury from 1992 to 2009 who underwent ED thoracotomy. The main outcomes included inappropriate ED thoracotomy. Secondary outcomes included resource use and societal costs for performing ED thoracotomy for impr

Aortic root replacement through right anterolateral thoracotomy.

A 59-year-old woman had undergone total arch replacement, followed by the mediastinal omental flap installation because of postoperative mediastinitis. One year later, she was diagnosed with annuloaortic ectasia with mitral regurgitation and underwent aortic root replacement (modified Bentall procedure) plus mitral valve annuloplasty through the right anterolateral thoracotomy. Her postoperative course was uneventful. The approach could be an alternative for the aortic root replacement in patients with previous median sternal wound complications.

Acute pain management for patients undergoing thoracotomy.

Management of thoracotomy pain can be difficult, but the benefits of effective pain control are significant. A variety of modalities for treating postoperative pain after thoracotomy are available, including systemic opiates, regional analgesics, and new oral and parenteral agents. This work provides a review of the literature and recommendations for the clinician.

Left thoracotomy reoperation for coronary artery disease.

Twenty patients underwent reoperative coronary artery bypass grafting (CABG) through a left thoracotomy since 1971. This was their second CABG in 16 patients, third in three patients and fourth in one patient. Surgery was performed from 1 to 16 years following the initial procedure. Demographic data showed no significant variation from patients undergoing standard reoperative CAGB in this institution. Ejection fraction varied between 30% and 73%. Cardiopulmonary bypass technic has gradually developed since 1971, using the left femoral artery and vein. For venous cannulation a 50-cm long cathet

Oesophagectomy without thoracotomy. Experiences from 30 cases.

In 26 patients with malignancies of the oesophagus or cardia and three with benign lesions the intrathoracic oesophagus was resected via abdominal and cervical incisions only and reconstructed with the stomach, a colon interposition or jejunal loop. The operation was chosen to omit the thoracotomy in patients with reduced tolerance to surgery or to secure a safe resectional margin in patients with easily resected tumours. With the exception of two patients, the method was not used for removal of mid-oesophageal tumours. Awareness of the possibility of intrathoracic complications such as bleedi

[Oesophagectomy without thoracotomy. 5 cases (author's transl)].

The authors describe an original technique of excision in severe caustic burns of the upper digestive tract: oesophagectomy without thoracotomy. The new procedure avoids the wide pleural opening of the thoracic approach, which leads to frequent and often fatal respiratory and infectious complications, whereas extrapleural drainage of the mediastinum has always proved effective and safe. Five technically successful operations were performed in one year. One patient died on the 10th post-operative day of tracheal necrosis related of the burn and 4 patients survived. These preliminary results are

Pulmonary artery sling repair via left thoracotomy.

Pulmonary artery sling is a rare congenital anomaly in which the left pulmonary artery arises anomalously from the right pulmonary artery. With rigorous pre-operative assessment, we successfully repaired isolated pulmonary artery sling using left thoracotomy in a highly selected group of children. In this modern series, we have achieved satisfactory surgical outcomes without using cardiopulmonary bypass support.

Resuscitative thoracotomy: What you need to know.

Resuscitative thoracotomy (RT) is a time-critical procedure performed in the setting of traumatic cardiac arrest, primarily to address cardiac tamponade, control intrathoracic bleeding, and reduce infradiaphragmatic blood loss. Advances in trauma care have refined the indications and improved the outcomes of RT in select patient populations. This review focuses on current techniques, indications, outcomes, and future directions. The physiologic objectives are to release pericardial tamponade, repair cardiac injuries, control intrathoracic hemorrhage, perform open cardiac massage, and provide a

Thoracotomy for Esophageal Pseudodiverticulum Following Cardiac Surgery.

Esophageal pseudodiverticulum (EPD), a rare benign condition, is typified by the presence of outpouching flask-shaped lesions across the esophagus. EPD, often secondary to a motility dysfunction, is associated with various condition, however, has not be reported following mitral valve replacement (MVR). We report a case of a 45-year-old male with progressive dysphagia to solids and liquids over 1 month, along with regurgitation and vomiting. Twelve months ago, he had undergone thoracotomy for MVR. Physical examination revealed signs of malnutrition and epigastric tenderness. Upper gastrointest

Left thoracotomy approach for aortic root surgery.

Marfan syndrome is an autosomal dominant disease of connective tissue affecting approximately one in 5 000 people. However, the majority of patients (60-80%) with Marfan syndrome have some degree of aortic root dilation or thoracic aortic aneurysm that can lead to aortic dissection and rupture, which has long been recognised as the leading cause of death in Marfan syndrome. The funnel breast (pectus excavatum), often seen in patients with Marfan syndrome, radically complicates the access during aortic root or arch interventions, forcing cardiosurgery practitioners to seek alternative approache

[Modified "trap-door" thoracotomy for pediatric patients].

INTRODUCTION: Surgical approach of the cervicothoracic junction has been traditionally done by cervicotomy and/or thoracotomy. Nevertheless, this access does not allow a suitable control of vasculonervous structures. Due to this we present our experience with a variation of the "Trap-door" thoracotomy which gives the best access to this area applied to pediatric patients METHODS: We present 4 patients of 2.8 +/- 1.9 years of age treated in our hospital by a cervicothoracotomy transmanubrial approach without clavicular luxation. One patient presented a stage IV cervicothoracic neuroblastoma, 1

Subglottic stenosis after thoracotomy--a case report.

Traumatic intubation, reintubation, intubation with endotracheal tube of inappropriate size, and failure to firmly secure the tube may contribute to the development of subglottic stenosis. Systemic factors such as sepsis, hypotension, autoimmune and granulomatous disorders have all been implicated as contributing causes. In addition, a risky circumstance that might be considered important in the development of airway damage is the occurrence of gastreoesophageal reflux (GER), particularly in thoracotomy operations, where the patients are placed in the lateral position. The purpose of this repo
