Thumb posterior rotation deformity: a neglected deformity in children with thumb polydactyly.

PURPOSE: To investigate the incidence and risk factors of thumb posterior rotation deformity in children with thumb polydactyly. METHODS: A retrospective analysis was conducted on the clinical data of 479 patients (521 thumbs, mean age 13.1 ± 12.7 months) with thumb polydactyly. Clinical data including gender, age, and affected side were collected. Clinical appearance photographs and x-ray images of all patients were obtained. Thumb posterior rotation deformity was determined on clinical appearance photographs. The ulnar deviation angle of the thumb was measured on anteroposterior thumb radio

Duplicated thumb with enormous soft-tissue oedema - pacifier type of thumb duplication.

Here we presented the first case of pacifier type thumb duplication. A newborn Japanese girl with no family history had a duplicated thumb on her left hand. The duplicated thumb showed a very large, oedematous soft-tissue nubbin in its appearance and was resected on the fifth day after birth. X-ray showed hypoplastic phalanx bone, suggesting type II polydactyly. Histology of the resected thumb showed enormous oedema in its connective tissue with cartilaginous and neural elements. This case was quite similar to literary reported cases of pacifier polydactyly in post-axial polydactyly, and its p

Video-computer quantitative evaluation of thumb function using workspace of the thumb.

This study develops the basis for video-computer quantitative evaluation method for measuring degree of thumb impairment. The system evaluates the three-dimensional space (workspace) within which the thumb-tip moves and computes its surface area. This study is intended to be the basis of a subsequent study which will compute the percentage of actual use compared to the "ideal" value for the given thumb length. Based on this quantitative measurement, the movement function of the impaired thumb can be identified objectively. Repeatability testing confirms that the video motion analysis system is

The Diagnostic Utility of Lateral Thumb Radiographs in Detecting Thumb Carpometacarpal Arthritis.

PURPOSE: Thumb carpometacarpal (CMC) arthritis is commonly evaluated using radiographs to guide treatment decisions. No consensus exists on optimal radiographic views or interpretation methods. The purpose of this study was to determine whether lateral thumb radiographs demonstrate more advanced degenerative changes than Bett's anteroposterior (AP) and oblique views in patients with thumb carpometacarpal arthritis. METHODS: We retrospectively reviewed preoperative Bett's AP, oblique, and lateral radiographs from consecutive patients undergoing thumb CMC arthroplasty (June 2023 to March 2025).

Ultrasound of Thumb Muscles and Grasp Strength in Early Thumb Carpometacarpal Osteoarthritis.

PURPOSE: The pathophysiology of thumb carpometacarpal (CMC) osteoarthritis (OA) involves complex interactions between the ligaments and muscles supporting the joint. Factors such as muscle volume and strength may be more relevant in early disease. We used ultrasound as a noninvasive method to explore differences in the intrinsic hand muscles of patients with early CMC OA, as determined using physical exam and radiographs, and healthy controls. We also assessed differences in grip strength. METHODS: A convenience sample of postmenopausal women with early CMC OA diagnosed using a physical exami

Management of Thumb Metacarpophalangeal Hyperextension in the Setting of Thumb Basal Joint Arthritis.

Thumb metacarpophalangeal (MCP) joint hyperextension is a well-established sequela of advanced carpometacarpal (CMC) joint arthritis. This deformity results in poor patient function because the ability to perform key pinch is negatively affected. For this reason, surgeons must consider the presence of an MCP deformity when addressing CMC arthritis. A variety of nonsurgical and surgical interventions have emerged. Surgical treatments can be grouped into four main categories: (1) volar plate advancement/capsulodesis, (2) tendon transfer and tenodesis, (3) sesamoidesis, and (4) arthrodesis. Surgi

Bowler's thumb: ultrasound diagnosis of a neuroma of the ulnar digital nerve of the thumb.

Bowler's thumb is a rare traumatic neuropathy of the ulnar digital nerve of the thumb. We present a case of bowler's thumb in a 21-year-old male recreational bowler who presented with a painful mass on the ulnar side of the right thumb. Magnetic resonance (MR) imaging of the hand was inconclusive. However, subsequent ultrasound (US) showed asymmetric enlargement of the ulnar digital nerve of the thumb with marked epineural thickening corresponding to the palpable mass, confirming the clinical diagnosis of bowler's thumb. Although this condition is typically diagnosed clinically, imaging can he

[Forum: reconstruction of the traumatic thumb. Esthetics and post-traumatic reconstruction of the thumb].

The authors deal with the aesthetic problems of traumatic thumb reconstructions from two complementary points of view: surgical and psychoanalytical. From the surgical point of view, they discuss the aesthetic factors of repair ("aesthetics equals function") in the hand and in the donor site. The features of a "good" reconstructed thumb are recalled, together with the ways of achieving this result, taking into account several imperatives related to the various techniques: the residual scar, the proportions of the thumb (length and diameter) and the appearance of the nail. The morphological seq

A Customised paediatric thumb web splint for management of adduction contracture of thumb.

The thumb web has a unique anatomy, the integrity of which is vital to both form and function of the hand. Thumb web space adduction contractures are a common consequence of hand burns and are a progressive disabling deformity and prevention or early intervention is advisable. This case describes the use of a customised removable web splint in children with scarring and gradual contracture of the first web space causing pain with movement and significant restriction in the hand function to maintain the thumb web expansion. This inexpensive and effective appliance can be safely applied to an in

Update on the Therapeutic Efficacy of Tregs in IBD: Thumbs up or Thumbs down?

Crohn's disease and ulcerative colitis, the 2 major forms of inflammatory bowel disease (IBD) in humans, arise in genetically predisposed individuals because of an abnormal immune response direct against constituents of the gut flora. Defects in counter-regulatory mechanisms are supposed to amplify and maintain the IBD-associated mucosal inflammation. Therefore, restoring the balance between inflammatory and anti-inflammatory pathways in the gut could contribute to halt the IBD-associated tissue-damaging immune response. Various suppressive T cell (Tregs) subsets have been characterized phenot

Stability of the Basal Joints of the New Thumb After Pollicization for Thumb Hypoplasia.

PURPOSE: To investigate the presence or absence of union of the new trapezium to the retained metacarpal base after pollicization and to relate this to stability of the new trapezium and the new carpometacarpal joint. METHODS: Thirty-six patients (46 pollicizations) were assessed at clinical review. Mean time from surgery to review was 96 months (range, 9-260 mo). Clinical assessment measured range of motion (ROM) at the carpometacarpal joint, stability of the carpometacarpal joint, and extrinsic and intrinsic strength of both hands. Radiological review evaluated 3 parameters: bony union betw

[Verification of thumb-specific muscles in a triphalangeal thumb with magnetic resonance imaging].

The presence of three phalanges in the first digit is considered to be a relatively rare congenital hand malformation. Six groups of this deformity can be distinguished: some digits are opposable, others resemble a non-opposable five-fingered hand. In cases of a hypoplastic thenar region with a restrained opposition, a clear verification of thumb-specific musculature has been hardly possible. We report of the possibility of a non-invasive identification of thumb-specific muscles by means of magnetic resonance imaging.

Association between hyperflexibility of the thumb and an unexplained bleeding tendency: is it a rule of thumb?

A bleeding tendency manifested by petechiae and ecchymoses is one of the most common causes for referral of patients to haematology clinics. Vessel wall pathology is not usually considered to be a cause for deranged haemostasis, although coexistence of increased capillary fragility and joint hypermobility have been reported. We determined the frequency of thumb hyperextensibility and scored the findings in a series of 44 patients referred because of ecchymoses and petechiae, as well as 261 control children and their mothers. All 44 patients had normal coagulation studies. Thumb flexibility sco

[Forum: reconstruction of the traumatic thumb. Toe as a donor site for reconstruction of the thumb].

One method of reconstruction of the thumb consists of microsurgical transfer of a toe. After a brief anatomical and technical review, the authors describe the various possibilities of thumb reconstruction using toe transfer: transfer of the 2nd toe, transfer of the great toe, partial toe transfers. Traumatic amputations must be distinguished from congenital amputations in terms of the indications for toe transfers. The indication for toe transfer in a case of traumatic amputation may be based on the level of amputation.

A Biosynthetic Thumb Prosthesis.

UNLABELLED: In cases of severe damage to the extremities, the function and structure of compromised biological tissues must be replaced. If biological reconstruction using autologous tissue is not feasible, amputation and replacement with a synthetic prosthesis is often the next best option. Currently, the synthetic materials available for prostheses are limited, especially in their ability to restore skin sensation. Here we show a biosynthetic prosthesis that combines the versatility of titanium with the rich sensory capabilities of biological skin, to both increase movement and restore sensa

Paediatric Trigger-Locked Thumb.

Paediatric trigger-locked thumb (PT-LT) is a common paediatric thumb condition. It is an acquired flexion deformity of the interphalangeal joint because of a size mismatch between an enlarged flexor pollicis longus tendon and a stenotic oblique pulley. It may present clinically as paediatric locked thumb which is more common or infrequently as paediatric trigger thumb. Paediatric locked thumb is classified into two types based on the presence of metacarpophalangeal joint (MCPJ) hyperextension. Type I does not have MCPJ hyperextension, whereas type II is associated with MCPJ hyperextension. The
