Anterolateral approach for posterolateral tibial plateau fractures.

The posterolateral tibial plateau fracture is an uncommon intra-articular injury and mostly needed surgery. However, its surgical approach remains controversial. This manuscript describes an anterolateral approach to treat posterolateral tibial plateau fractures and evaluates the patient's functional outcomes. From June 2018 to July 2021 seventeen patients with posterolateral tibial plateau fractures were surgically treated through an anterolateral approach. The intraoperative and postoperative follow-up indicators were recorded. The reduction quality of fractures was assessed using Rasmussen

Tibial spine fractures: State of the art.

Tibial spine fractures (TSFs) are avulsion fractures at the site where the anterior cruciate ligament inserts onto the tibial eminence. TSFs typically affect children and adolescents aged 8-14 years. The incidence of these fractures has been reported to be approximately 3 per 100,000 per year, but the rising involvement of paediatric patients in sporting activities is increasing the number of these injuries. TSFs are historically classified on plain radiographs according to the Meyers and Mckeever classification system, which was introduced in 1959, but the renewed interest in these fractures

Management of High-Energy Tibial Pilon Fractures.

➤ Pilon fractures in the younger patient population are frequently high-energy, intra-articular injuries and are associated with devastating, long-term impacts on patient-reported outcomes and health-related quality of life, as well as high rates of persistent disability.➤ Judicious management of associated soft-tissue injury, including open fractures, is essential to minimizing complications. Optimizing medical comorbidities and negative social behaviors (e.g., smoking) should be addressed perioperatively.➤ Delayed internal fixation with interval temporizing external fixation represents the p

[Osteosynthesis of extra-articular proximal tibial fractures].

The treatment of extra-articular proximal tibial fractures is a therapeutic challenge due to the frequently significant soft tissue injury, the effect of the deforming forces and the need for an exact restoration of the bony alignment. Various methods of osteosynthesis are available for surgical stabilization. The locking plate osteosynthesis is the most frequently used procedure because of its good biomechanical stability, especially in osteoporotic bones, and the protection of the periosteal blood flow. Depending on the extent and stability of the defect zone, especially in the case of a med

Screw Fixation of Pediatric Tibial Tubercle Fractures.

BACKGROUND: Tibial tubercle fractures are rare injuries that account for <1% of physeal fractures. These fractures are thought to be increasing in frequency, particularly in young, adolescent males who participate in basketball and other sports with repeated running and jumping. The tibial apophysis becomes mechanically vulnerable as the proximal tibial physis closes from posteromedial to anterolateral, enabling the quadriceps to overpower the chondroepiphysis and avulse the proximal tibial epiphysis from the tibial metaphysis. DESCRIPTION: Position the patient supine with the leg on a bump o

Outcomes and Review of Periprosthetic Tibial Fractures.

Periprosthetic fractures around total knee arthroplasties (TKA) are well documented in the literature. Fractures of the tibial side occur least frequently and often pose reconstructive challenges to the treating surgeon. As use of arthroplasty continues to rise, periprosthetic fractures of the tibia will likely become more prevalent. Periprosthetic fracture management is based on recommendations (Mayo classification) made 20 years ago. With newer design technology, the adjuvant use of locking plates in the presence of implant stable periprosthetic tibial fractures (Felix types IIA and IIIA) is

Proximal Tibial Fractures in the Pediatric Population.

Injuries about the proximal tibia are quite common in children. Understanding the mechanisms of injury and treatment strategies is imperative for practitioners caring for these patients. Similarly, appreciating the anatomy of the proximal tibia and the adjacent neurovascular structures can assist a practitioner in recognizing the potential associated risks following fracture. Patients with injuries about the proximal tibia require long-term follow-up, and knowledge of the natural history of these fractures is imperative.

External fixation of segmental tibial shaft fractures.

PURPOSE: Tibial shaft fractures are the most common type of large long-bone fractures. Segmental tibial shaft fractures are severe injuries and its treatment can be followed by a high incidence of complications, nonunion, delayed union, malunion, infection or compartment syndrome. The most common treatment strategy is intramedullary fixation. Results of the unilateral external fixation treatment for segmental tibial shaft injuries are presented in this paper. METHODS: Patients with segmental tibial shaft fractures, AO/OTA 42-C2 were treated as urgent cases by unilateral external fixation. Mit

Surgical fixation methods for tibial plateau fractures.

BACKGROUND: Fractures of the tibial plateau, which are intra-articular injuries of the knee joint, are often difficult to treat and have a high complication rate, including early-onset osteoarthritis. Surgical fixation is usually used for more complex tibial plateau fractures. Additionally, bone void fillers are often used to address bone defects caused by the injury. Currently there is no consensus on either the best method of fixation or bone void filler. OBJECTIVES: To assess the effects (benefits and harms) of different surgical interventions, and the use of bone void fillers, for treatin

Tibial stress phenomena and fractures: imaging evaluation.

This study aims to evaluate the various imaging modalities used to diagnose tibial stress-fractures/phenomena and determine which of these are most useful and definitive. The plain film, computed tomography (CT), magnetic resonance (MR), and nuclear medicine findings in a 20-patient cohort, ranging from ages 10 to 21 years with an average of 16 years, were reviewed. The male to female ratio was recorded as was the incidence of right or left, or bilateral extremity involvement. Thereafter, each imaging modality was evaluated for positive findings. Twelve of the patients had pretibial swelling o

Review of distal tibial epiphyseal transitional fractures.

The closure of the distal tibial physis occurs over an 18-month period between ages 12 and 15 years. During this time period, children are susceptible to several transitional fractures, so labeled because they are transitioning to skeletal maturity. In the setting of an external rotation force, triplane and Tillaux fractures can occur. These fractures, which present similarly to other Salter-Harris growth plate injuries, do not fit neatly into any 1 classification scheme and are not easily evaluated on plain radiographs. Computed tomography scans are required to optimally assess these fracture

Arthroscopy- assisted surgery for tibial plateau fractures.

Like other intra-articular fractures, the tibial plateau fracture is challenging for orthopedic surgeons because of its severity of trauma, associated soft tissue injuries. Open reduction incurs serious complications, especially wound healing after traditional dissections. Unsatisfactory results often occur in complex or bicondylar tibial plateau fractures. Traditional surgical methods achieved satisfactory results in 70-80% of cases. However, these methods have a high incidence of complications including loss of reduction, infection, and septic arthritis. The advantages of arthroscopy-assiste

Bilateral tibial stress fractures: a case report.

A stress fracture can be defined as a fracture of a bone caused by repeated (rather than sudden) mechanical stress. They have been reported in almost all bones of the body, with the lower extremity weight bearing bones, especially the tibia, tarsals and metatarsals, being affected most often. These injuries have a broad spectrum of severity and prognosis. Although the pathology of this injury is understood, the aetiology is less agreed upon. This can make it difficult for clinicians to diagnose and treat this common injury. Stress fractures of the proximal tibiae are common in elderly patients

Ilizarov treatment of complex tibial pilon fractures.

We treated 21 consecutive patients between 1998 and 2002 with complex tibial pilon fractures, eight type B and 13 type C, using percutaneous reduction and fixation with the small diameter Ilizarov apparatus. The average patient age was 34+/-5.6 years (range 28-52 years). Nine of the patients had open fractures (two type I, four type II, and three type IIIA). The patients were followed up regularly at 6-month intervals for 2 years. All fractures united. The fixator was removed at an average of 26.6+/-4.2 weeks (range 20-34 weeks). The average American Orthopaedic Foot and Ankle Society ankle-hi

Management of ipsilateral femoral and tibial fractures.

This is a retrospective study of 18 patients who had ipsilateral femoral and tibial fractures. They were treated by the retrograde femoral and antegrade tibial intramedullary nail from a single incision in the knee. The average time for union of femoral shaft fractures was 27.6 (18--40) weeks. One patient required antegrade nailing with a bone graft due to metal failure after using the short nail. Two tibial fractures required bone grafting due to bone loss, with an initial open fracture. The average time for union of tibial fractures was 24.5 (18--30) weeks. Functional results using the Karls

Arthroscopically assisted osteosynthesis for tibial plateau fractures.

BACKGROUND: Options for management of tibial plateau fracture are numerous, and the outcome depends on several factors. With the inherent advantages of arthroscopy, the results of tibial plateau fracture have been greatly improved. METHODS: Thirty-one patients were treated with arthroscopically assisted reduction for tibial plateau fracture, with an average follow-up period of 3 years. Evaluations were performed with the Hospital for Special Surgery knee score clinically, and with the Ahlbäck scale for osteoarthritis radiographically. RESULTS: Eighty percent of cases were Schatzker type II,

[Miniinvasive plate osteosynthesis of distal tibial fractures].

The end result after open reduction and internal fixation in distal leg fractures is jeopardized by soft tissue complications. Functional conservative treatment of the same type of fractures often results in delayed and non-union, malunion or non-optimal functional result concerning ankle mobility. Mini-invasive plateosteosynthesis was performed in ten patients with sustained juxta or intra-articular fractures of the distal tibia. In all patients, stable osteo-fixation could be achieved and soft tissue complications did not affect the final result in any of the patients. Mini-invasive plateost

Arthroscopic assistance for unselected tibial plateau fractures.

Nine closed tibial plateau fractures in nine patients were treated with arthroscopic-assisted reduction and percutaneous fixation or buttress plate. These procedures were performed between 1993 and 1994 and represent an exclusive and nonselected, consecutive series. The mean follow-up period from time of operation was 10.6 months (range, 6 months to 1.5 years). The results were satisfactory. All the reductions remained roentgenographically anatomic with good to excellent clinical function. Concomitant knee pathology included three cases of partial tear of the anterior cruciate ligament that re
